They're really trying to say "it shouldn't matter whether you're conventionally attractive or not" while casting conventionally attractive actors. Like girl by doing that you're saying "you shouldn't change yourself because you're already beautiful" instead of "you shouldn't change yourself because it doesn't matter if you're beautiful".
The Twilight Zone episode "Number 12 Looks Just Like You" tackles the subject of forced plastic surgery really well and it's from 1964. In the story people only have a select number of models to chose from and there's also psyhcological effects from the surgery. It's a very compeling episode and surprisingly relevant with "everyone looks the same nowadays" being a big topic.
I AM AN UGLIES APOLOGIST, at least the book. Maybe the movie didn't describe the concept well. But it doesn't really matter what the people look like before or after the surgery because the idea of " pretty" and "ugly" is arbitrary. It is a way to make people feel inferior EVEN WHEN THEY AREN'T, much like our society today. The allure of becoming pretty is used to control the population, and to stop them from thinking critically about their government and societal norms.
