I'm a eager Japanese student of English language. Could I ask a question? Does just "head" implies 'giving head' thing? Is the slang so common that all of them seem to understand it? And also why shiori started such dirty joke from nowhere? Google couldn't answer them.

If you look up "head" in some dictionaries it will be listed as an alternative meaning (Slang: Vulgar. fellatio or cunnilingus). Giving head/getting head/receiving head are common phrases so Shiori saying "not head?" was enough for everybody to know which head she was talking about. Why did Shiori make such a dirty joke from nowhere? Because Shiori is Shiori.

気になって辞書で調べたのですがそれ程悪い意味は載っていませんでした。(この文脈でヤバそうなのは船のトイレくらい)

スラングの説明に別のスラングがあったりしてかなり読みづらいことが多いですけど...
