    I am narrating this video while I am treating this 36 year old man who complained of a large central 'dark 'blob' plus other squiggles and lines. When I went out to the waiting area and saw that he was young, I had already prepared my " I see your problem, but I can't treat you" speech, which is the expectation and prediction with younger patients. It is just unusual that they are ever treatable as their floaters are usually microscopic fibers and strands sitting too close to the retina. I was flabbergasted and very surprised to see a large, central weiss ring floater with associated hazy, frosted-glass vitreous cortex sheets that were probably responsible for the 'squiggly lines' he was also describing. It was the appearance of floaters that I would expect in an older myope.

    The Floater Doctor, James H. Johnson M.D. is Medical Director of the only medical practice in the world specializing in, and exclusively treating bothersome spots, shadows, and clouds that affect your vision. Although common, especially changes such as posterior vitreous detachment (PVDs) as we age, these vitreous eye floaters are rarely offered treatment by your local and well qualified eye care providers, even at top specialty institutions. Dr. Johnson uses a specialized FDA-approved YAG laser to vaporize and destroy the proteins in the vitreous that are responsible for your spots, shadows, and moving blurred areas in your vision. He has been doing so since 2007 making him one of, if not the most experienced in this very specialty niche area in ophthalmology, a pioneer in this field.

    I'm only 30, but this year I developed a huge floater in my right eye. From what I can tell, it seems to be a Weiss ring, albeit a broken one (it appears as a sort of semicircle, but sometimes opens to be more straight or closes to be almost circular, depending on how it moves). It's been there for nearly half a year now. It's very annoying, and makes me sensitive to light because the brighter my surroundings, the more visible and distracting the floater is. I can't stand to look at a bright computer screen anymore. I use dark mode on everything now.
    I live in Mississippi. I don't know what treatment options there are here, or how much they cost (I also have no health insurance).

    I have had PVD in both eyes. I was the same age. My doctor was so used to seeing this in older patients and has always stated that the Weiss ring's would go away soon. To my understanding the vitreous in younger people are at a different consistency. Mine has remained in place for 5 years so far. I have already had lens exchange surgery due to other eye issues and am definitely in need of floater removal to optimize my vision. I'm glad to hear how knowledgeable you are and how you show compassion for the ones you are trying to help. Thanks for the video!

    Hi, do Weiss rings typically clear up after a certain amount of time? I've had mine for 6 months and the ring that I can see has become slightly less noticeable when I blink. Reading text close up is better with less blur, but longer range vision still seems a bit blurry when compared with my other eye. Could it even take 2 or 3 years for the Weiss ring to move away from my central vision? I have no myopia either. Thanks for your help!

      @Thefloaterdoctor1  2 роки тому +2

      Weiss Ring floaters do not go away (physically), but if they start out closer to the retina and gradually shift further away the edges and outlines can soften up and possibly become less noticeable, but they do not go away.

    Hi Doctor. I am 35 and started seeing floaters 2 years ago. One day when I woke up, a small cob web started flying in front of me. I was really scared until I saw people online sharing floaters in their vision and that its normal especially when you're nearsighted. I got my eyes checked, and the eye specialist told me that I dont have PVD yet, what I have is Vitreous Syneresis and my eyes are perfectly fine. I have yet to consult another eye specialist for a second opinion and better explain my eye condition. And also, I am now sensitive to negative after images, when I read a paragraph (white text on a black background) and look away, I see horizontal lines for several seconds. I am not sure if that has something to do with Vitreous Syneresis. I am really worried that it gets even worse eventually. If you have encountered this same situation with a patient, please shed some light atelast for a peace of mind and hopefully I can see you to further improve my vision. Thank you so much. I appreciate it.

    • @Thefloaterdoctor1

    I got told by my opthalmologist that I am having an ongoing PVD as a result of repeated blunt force trauma to the temples, and I'm "only" 24... Not asking for medical advice, I'm just curious about the condition.
    Is the Weiss ring itself rare in that age or is it only rare because PVD's are rare at such an early age?

    • @Thefloaterdoctor1
      @Thefloaterdoctor1  Рік тому +1

      Weiss Ring floaters at a young age are very very rare because the Weiss ring is part of the membrane that peels away during the PVD. PVDs are very uncommon in that young age group. There are many many examples in my practice where someone came to me. Haven't been told they have PVD and on examination they did not.

  • @StarlifeStudios

    I'd love to say 'great' but surely the reality here is that this shouldn't have happened to a person so young. As a young person myself (with floaters), I find it hard to believe that some kind of stress induced from the LASIK didnt have an impact on the Weiss ring formation. This video and the amazing work you do also highlights how every eye is different and why you shouldn't shut your door to potential young patients. Can I ask if the position of the weiss ring (anterior or poserior) was any different than normal from your experience with older patients?

    • @Thefloaterdoctor1
      @Thefloaterdoctor1  4 роки тому +5

      The LASIK was years ago. I don't think that was a factor, but more likely the myopia that pre-existed. The hazy, hazy vitreous cortex sheets are more common in the very nearsighted. The position of the floater complex was typical: mid-vitreous to slightly anterior to mid-vitreous.
      And yes, I agree with you that I may have to not take such a hard stance about refusal to see younger patients.

    • @Thefloaterdoctor1
      @Thefloaterdoctor1  3 роки тому +2

      PVD just describes a 'shift' of fluids with a separation of the posterior hyaloid membrane from the retina. PVD is not the same as floaters. You can have floaters without PVD and PVD without bothersome, specific floaters. Even if you have/had a PVD I can't really make a generalization as to whether you would be treatable even with a PVD.

    • @Thefloaterdoctor1
      @Thefloaterdoctor1  2 роки тому +1

      "Younger people floaters" do not graduate to become the more-likely-treatable "Older people floaters". They are bot aggregations or re-distributions of the same collagen proteins, but these small floaters close to the retina will not predictable move to a more treatable area, and because of their small size, if they did migrate', you probably would no longer see them and wouldn't need treatment.

      The laser is NOT a single beam, but instead it has a cone-shaped profile. The energy is delivered to a single point in space. As long as that focal point of energy is kept safely away from the retina and lens there should not be any energy delivered to these structures - even if the target were missed. • @Thefloaterdoctor1
      @Thefloaterdoctor1  Рік тому

      There is an association of eye floaters and retina holes, tears, and detachments. Floaters don't cause detachments. Especially the small, thin fibers that younger people usually experience. But, when the vitreous peels away from the retina as with a Vitreous detachment, the traction as it peels away can cause a retina problem. Like pulling painter's masking tape away from the wall. The risk is only when it is being pulled away.

      I can't say this though if you have had a true posterior vitreous detachment or PVD then it is someone more likely that I will be able to treat you. A PVD in someone at your young and tender age is really quite rare. This is one reason I posted this video where I said I've never seen a troop PVD in somebody so young. I have seen many other times were somebody was told they had a posterior vitreous detachment when in reality they did not. It shows that my profession really doesn't have a good standardized way of describing eye floaters. Sometimes the doctors will see a cluster or group of some hazy aggregated proteins and call it a PVD.

    Hi Dr. Johnson...My name is Kevin; I'm 37 years old and have seen my eye floaters in both of my eyes since I was a teenager. I have two questions; first is can eye floaters or "PVD's" happen from a sudden blow/impact to an eye? I use to play racquet ball and had a rubber ball about the size of a base ball impact my right eye with quite a bit of force and wonder if that could have caused a PVD? The second question is can eye floaters "worsen" or grow in size over time? I have noticed over time my eye floaters to change and become worse bigger/longer with age. I have a "backwards C" shaped floater in my left eye and a "blob" or web floater in my right eye which is worse of the two. Since I have had them for so long I have gotten use to them but they are the most annoying when I am trying to read a book; I can easily see them floating across the white pages of any book I am trying to read. I am very interested in YAG laser treatment and would like to know what you think.

    • @Thefloaterdoctor1

      Absolutely trauma can cause a PVD by extreme distortion of the eye at the moment of impact. If your doctors have actually called it a PVD and were easily able to see and identify your floaters, then you may be that exceptional younger person who is a candidate for treatment with the laser.

It is possibly worth trying and I have been successful, notably with a radiologist patient of mine.

      I've seen and talked to many patients who are told by their well-intentioned eye doctor that they were experiencing a PVD. When I have the opportunity to see them in person they did not. I think some doctors confuse floater symptoms with pvd, but it is possible to have a PVD without much floaters and it is also possible to have floaters without a PVD. At your young and tender age, it is very, very unlikely that you have a true pvd.

Only a visit with someone like Doctor J will tell you best about your floater; they need at least 3mm of clearance from the retina. The more of a foggy blur your floater looks, the better your chances as it means it floats within the safezone away from the retina.

Actually I don't really care about most of them. I have a lot floaters near my retina. But I can live with them. I wondering about one big transparent floater in the center of my vision. It look like looooong bold transparent line. I'm totally sure this floater located in a front part or in the middle of my eye. I'll be happy if you could destroy this floater for a couple parts. The laser is NOT a single beam, but instead it has a cone-shaped profile. The energy is delivered to a single point in space. As long as that focal point of energy is kept safely away from the retina and lens there should not be any energy delivered to these structures - even if the target were missed. A transparent, bold, and well-defined floater may be more likely located very close to the retina. Your description can't really be used to ascertain the location. It ultimately takes an in-person evaluation.

    I'm 47 and newly diagnosed with a first flare of bilateral uveitis that ended in bilateral PVDs with the Weiss ring floater in each.Worse part is they over lap in my center vision. Test show negative for autoimmune disorders and HLA B27. My uveitis and retinal specialist gawked at me when I asked about eventually getting them removed since it has a great impact on my independence, especially reading and driving. I asked about doing the laser procedure to at least break them up and she got very aggressive and defensive because "I don't believe in that magical fix" and no one should ever consider that procedure, just stick to tried and true surgical Vitrectomy for floaters. But then I see videos like this and how less invasive it is and question her judgement. She is supposed to be one of the "Top Docs" in Arizona for Uveitis so then who am I to judge. I don't know. But I hate my life now. This is so disheartening to me to live the rest of my life like this, but I guess it could be worse.

I've literally never paid any mind my whole life up until a few weeks ago when I started seeing a sudden increase. These new floaters look more like black hairs, dots, or cobwebs in both eyes. They aren't visible when my eyes are stable but when I turn my head or eyes, they dart across my vision and are very distracting.
    I don't see any flashes or anything though. I've had no trauma to the eye or anything. Just out of the blue this happened.

    • @Thefloaterdoctor1
      @Thefloaterdoctor1  2 роки тому

      It is unlikely a PVD based on age alone. The are some 'inexplicables' when it comes to floaters in younger patients. Maybe is is inflammation/oxidation spilling over into the vitreous. How is your diet? Eating clean or highly process foods. Vegetable oils (especially fried foods) are high in Omega-6 Fatty Acids and contribute to body-wide inflammation. If there is room for improvement in that area consider it.

      It is not a question of approval, it is a question of candidacy. I can treat somebody young if their floaters are in an appropriate and safe place to treat. Typically young people's floaters are located closer to the retina where it is not safe to treat. Ultimately, it does take an in-person evaluation to determine that but if you were to travel a very long distance I would probably discourage you just based on the likelihood that I would be able to treat you successfully.

      I don't agree, or at least the science isn't there to support that assertion. What would be the difference to the vitreous from starting at a screen vs. staring at the road as a long-haul trucker, or just using your eyes all day? The photons should just pass through the vitreous without changing it. Could there be other factors, like systemic oxidation? Inflammation? Both of these are associated with the modern Western diet full of highly processed foods and artificial 'vegetable' oils. I don't think it is healthy to stare into a screen all day, but not because of the effects on the vitreous.
