Biography:
Dr. Leff is Professor of Medicine at the Johns Hopkins University Schools of Medicine. He cares for patients in the acute, ambulatory and home settings. His research focuses on novel models of care delivery for older adults, multi-morbidity, risk prediction, and quality improvement, with an emphasis on home-based models of care. He is Co-Lead of the Hospital at Home Users Group and also the National Home-Based Primary Care Learning Network. He is past-President of the American Academy of Home Care Physicians.
