I was always under the impression that the last theme was actually a reworked version of the Paul Hart theme - used for the last series, in an effort to revive the programme's popularity..? Finding any trace of the final season's opening credits is proving almost impossible, though...
The last theme to TW WAS a remixed and somewhat truncated version of Paul Hart's classic late 80's version of the theme, although produced by the man himself. The title sequence used, like the final couple, was nothing special just a sort of coloured holographic effect/pattern with a simple logo in black lettering.

I like this time I think it was used from 1998-2000. The same theme was used 1997-1998 when Peter Snow joined but was louder.
How needlessly overwrought and grandiose this title sequence is reminds me of 'Brass Eye's'. This always did my head in as a kid.
