    I like your attitudes....and especially your final statement. I have a grade 3 ac separation....and my first consultation was just horrible. The specialist had zero patience to explain such simple things as what actually happened to my clavicle and ligaments (from a car accident), and explaining to me the pros and cons of the choices going forward. The guy said he had the same thing.....told me the technical name for the injury...and said I should take the non-surgical option...and walked out when I had further questions. His assistants saw exactly what happened...and offered me a second consultation with a different surgeon, and it worked out very well. My brother is a doctor, so I know full well that there is the normal human variation in the communication skills of doctors....and I had a pretty awful one...lol.

    I suffered what was called a partial shoulder separation in 1974. I reinsured the shoulder twice more. Was only ever told to rest the shoulder and take time away from hockey. Was never sent for physiotherapy, which was probably an oversight. Today at 68 I still have low grade pain. It actually got a little worse after watching this great video! Thanks.

      @@jena2664first don’t be scared. 4 years ago I had the ‘tightrope’ procedure. Sounds like that’s been recommended to you. It’s for the AC joint, right? I would not just let it go and I do not know why you would want removable screws. My AC is has recovered completely. I do 2 combat sports for perspective. You could additionally shoot it with PRP, ozone, stem cells in Mexican clinic , peptides like BPC 157 for strong, fast healing.

    I got a AC-Joint grade 3 about 6.5 weeks ago, i was in terrible pain the first two weeks, i started doing rehab about 3.5 weeks ago. I have almost no pain now, and ive been going to the gym for about 2.5 - 3 weeks. I can do almost all types of lifting, except for chest workout such as bench-press. I had alot of pain doing Militarypress and dumbellpress, but about a week ago i felt no pain doing a full Shoulder workout. I can already do 12 reps of 14/16/18kg dumbellpress overhead, and 6-8 reps of 22/24/26kg without any pain or discomfort. The Only excersice that truly hurts is bench-press, so i dont do chest workout. Both me and my physical therapist are amazed about the progress, and i get stronger and better each day. Still doing my rehab two Times a day.
    I would recomend all of you with the same type of injury to do the rehab no matter how hard it feels, and start going to the gym, if an exercise hurt such as militarypress, then stop and wait a few days before trying again. i almost started crying of the pain the first couple of Days doing the rehab workout. But i feel better and better each day cause of it.
    I wont do the surgury if i dont have any trouble after a year. Good luck to you all.
    Btw, i am 23 years old and i have this wierd looking ball above my Shoulder, but i Will gladly have a wierd looking Shoulder as long as im painfree.

      @@a1expresstransportllc956 i am doing pretty well, i still have some pain while bench pressing and doing chest workout. I do feel some discomfort sometimes when The collarbone is "moving" around, not in pain but it feels pretty wierd. I will see my doctor in August/september to check how it feels Then. If Im not painfree in all motions by then, my doctor want to do The surgery. But now that covid-19 is around it might take sometime to get a surgery.
      But overall i feel good. I can do almost everything that i did before the accident. Except a painfree chest workout.
      I do heavy deadlift, i am stronger in my shoulder workouts then before, pressing 30-35kg dumbells above my head without any pain at all. Try to find a good physiotherapist and follow a rehab program before you concider to do a surgery. I dont want to do The surgery, it might get alot better, or alot worse. I Will only concider it if i am not fully restored by next year. Take care and I hope you Will get better, i got pretty depressed a couple months after injury, because The pain was not going away even if i did very Light benching. I determined to have surgery (reverse plate)and ligament reconstructction ( graft from my forearm) Dr. told me ligament transfer could have been avoided if repair surgery was done within 3 weeks of injury! The procedure would be simplier and easier! What a lesson for me!

    Had a grade 3 separation from a bad ski crash where I crashed jumping over a tree and landed on the direct left side of my shoulder. Doctor from cut rate Keizer permante said I didn’t need surgery and it would get better said there would be no difference with or without surgery. Went to a sports surgeon and said I should get surgery. Dr explained that the surgery Has the highest success shortly after injury because the connective tissue hasn’t fully healed. It’s been almost 2 years since I had my ac reconstruction surgery and I am back to 100%. Gained my strength in my upperbody
    Back and some more. The the pt hurt like hell and there where some ups and downs but it was well worth it.

    Hello, how are you doing Docs. So I had a snowboard accident a week or so ago. When I fell I felt something wasn’t right, but the pain was almost nonexistent. Went to a Dr a few days later and the said I had ACJ separation. First Dr said it looks like a grade 3 but could be a 5. 2ed dr said it was a grade 5. He gave me option of PT or surgery. He also said a lot of NHL players get it, don’t have surgery and live a fine life. My question is: If I have a grade 5 but pain is very minimal, should I just go with PT? I’m very iffy when it comes to surgery. I know my left shoulder (ACJ separated) won’t ever be “the same” but, can PT make it to where I get my range of motion back and live a very normal life?

    Just to share experience of this unfortunate injury.14 months on from the injury and no surgery which I decided would be the best option given the explanation of how the surgery would be and how it is hit and miss.Was told mesh and wire would be done but that it could fail too plus a 6 month lay off from anything using the arm.I decided to wait and see how it went realising it would not be 100pcent again but thankfully to date this grade 3 injury is 98 pcent ok.A little discomfort occassionally but I have full use.A bump sometimes on the shoulder. I can..touch wood..use the weights in the gym regularly although the lat pull down seems to aggravate it so best to avoid that one..all others seem fine though. Psychologically I am fine with it now and that is something you must overcome...lose the worry about the injury which at first is hard to do because you are dealing with the unknown.
    My advice is if you are over 35..i was 49 at the time of injury...wait and do not have the operation as you are getting somewhat older ...if below 35ish try the surgery as you have many years in life left wheras I figure realisticaly 20 years for me is not a long time really. I am also now running again after a 3 year injury ostitis pubis from which I thought I would never run again.
    My own body is showing me that the human body does have amazing powers of recovery even when you may not think so.
    I hope this helps some people out there who have the same injury as I searched for knowledge and help myself when it happened. Good luck everyone.Stay positive.

    Wow! You guys are great!
    15 years ago, I suffered an AC separation, which after a year of PT required surgery- removing the end of the clavicle and some other minor stuff that I can't remember. The results have been nothing less than excellent! Sadly, a week ago I fell while sprinting on astroturf and suffered a grade 3 separation in my other shoulder. Ive seen the surgeon, well, the surgeons PA, and explained that I have two options, surgery or PT. when I explained I prefer a more conservative approach, it it would bare good results, I was basically dismissed. The surgeon, when he came in the room basically said, I understand you want PT, see me down the road and departed... I do have a question :) so, the PA said the procedure would be a Dog Bone with a hook? Im not sure I like that procedure, and feel like the results I had from my other surgery were great so why not go with the same procedure? Do you have thoughts? thanks for the help!

    • @TalkingWithDocs
      @TalkingWithDocs  6 років тому +3

      Hi Schrodinger. Thanks for the positive feedback. Sounds like you have a very sensible approach. We would agree that if you want a conservative approach that is a great place to start and that if you required surgery in the future, a distal clavicle excision and ligament reconstruction sounds reasonable. Interesting there was an article published very recently about pro baseball pitchers and your injury. I have attached the abstract to our reply. Take care and good luck.
      Am J Orthop (Belle Mead NJ). 1997 Nov;26(11):771-4.Treatment of grade III acromioclavicular separations in professional throwing athletes: results of a survey.

      1Department of Sports Medicine, The Johns Hopkins University School of Medicine, Baltimore, Maryland, USA.
      Abstract
      Forty-two team orthopedists representing all 28 major league baseball teams were surveyed to ascertain their definitive treatment for a hypothetical starting rotation pitcher who had sustained a grade III acromioclavicular (AC) separation to his throwing arm 1 week before the season. Twenty-nine (69%) of the physicians would treat the injury nonoperatively, while 13 (31%) would operate immediately. Twenty-five (60%) of the orthopedists had actually treated a pitcher or position baseball player with a grade III AC separation in the throwing arm, the 25 treating a total of 32 patients. Twenty (63%) of these injuries were treated nonoperatively, and 12 (37%) were treated operatively. The physicians reported that 16 (80%) of the patients treated nonoperatively regained normal function and achieved complete relief of pain, while 18 (90%) had normal range of motion after treatment; of those treated operatively, 11 (92%) regained normal function, achieved complete relief of pain, and had normal range of motion after surgery.

    Question. A year ago I fell 15' onto the ground face down. My hand was stuck in a piece of equipment and was pulled back behind my body and then free as I fell. Shoulder was x-rayed, xray came back clean, had an mri done which showed tearing In my rotator cuff. Did 1 year of physio with no improvement. Had laproscopic surgery on rotator cuff done last month. Dr said I had a lot of scarring and inflammation but that the rotator cuff had healed. Swelling remains in my tricep muscle with a lot of pain. Trapezoid muscle and shoulder blade pain upon raising arm above 90 degrees. Pain is directly above where clavicle and acromion meet on the back of my shoulder and up into the back of my neck. I've been diagnosed now with a frozen shoulder. My question is, what are the chances I've been misdiagnosed and actually have a separated shoulder which is causing a frozen shoulder? I don't Return to see Dr for another 3 weeks so I'm kind of curious

    • @TalkingWithDocs
      @TalkingWithDocs  2 роки тому

      Hi A B. So a separated shoulder is easier to diagnose simply from X-ray but we suspect you mean a dislocated shoulder. A separated shoulder usually refers to the relationship between the clavicle and the acromion in the front where they meet. It usually leads to relatively localized pain that gets better with time. If your shoulder was dislocated it is usually not subtle and would be more straightforward to diagnose. You likely have a frozen shoulder from your bad fall. Hope that helps. Hi tommy. Sounds like you had a significant injury. Ultimately you should be assessed by a surgeon with a physical examination and some xrays. The main reason to seek treatment would be for pain not for cosmetic reasons. If the pain is significant there are some surgical options that may help you. Again, that is best discussed with your surgeon. Good luck and hopefully that helped.

    I had an ac separation grade 3 and it was painful. The local doctor told me to choose between operative or non operative treatment so I didn't do the surgery , there was quite improvement by the time then stopped at the 11th week, thus I gave up and went to do the surgery. The pain is greater with extreme movement like, reaching straight up above the head, reaching across the body to opposite side, pushing/pulling of any kind of weight bearing-like, lifting an object or yourself to perform daily functions. I have been told that it will heal on its own and that I need to have surgery. Going for an MRI today. What do I do? Let it 'heal' or get surgery? I'm miserable, please help!!!

      Hi Joshua. Every case is slightly different. This is an injury that will "improve" with time but not really heal. The soft tissues that were responsible for holding your clavicle down have been disrupted. Your clavicle will remain elevated. We tell our patients that if after a year, you still have significant pain then we discuss potential reconstruction and distal clavicle excision. This is something best discussed with your treating surgeon. There are many people who function very well with this specific injury and others that do go on to require surgery. Hopefully this does not muddy the waters any further. Good luck.

    Great Video. Thanks for sharing. So wanted to share my experience and get thoughts on if I should look into surgery option now.
    I am 46 y/o male and basically fell of bike and got Grade III (borderline V) AC Separation in left shoulder. No surgery. 2 Weeks in sling and then started PT and regained full range of motion and strength around 3 months. All was going OK and joint worked but always felt awkward with lots of popping and grinding especially until it was warmed up. Went to shoulder ortho doc again at 5 months and we both agreed at that time the conservative approach worked and to resume full activities. So I then tested the shoulder with my normal active lifestyle. Landscaping, chainsaw use, wood splitting, basketball, minor weight lifting for general health etc. Well it failed me at about 6 months. I tore something...most likely anterior deltoid muscle where it attaches to clavicle. Took 2 months off and restarted PT and it is back to how it was again. I am at 8 months now and I have 3 sets of xray and just got MRI. MRI showed 3 things
    1. Small partial-thickness undersurface tears of the supraspinatus. No evidence of full-thickness rotator cuff tear.
    2. Subdeltoid/subacromial bursal effusion. Consider bursitis.
    3. Moderate AC arthrosis without MR evidence of impingement.
    The joint is very unstable and when tested it failed. My body is telling me this joint isn't working right and I honestly feel like I am doing more damage then good at this point.
    I have horizontal as well as vertical instability in the joint. My strength and ROM are still very good...almost 100% and pain is minimal (2-3).
    Needless to say I am frustrated and I think surgery might be my only option left to stabilize this. Just curious what the you guys think. Thanks again for any thoughts and great job
    putting out the information.

      Hi Sharky 40. Surgery is all about your symptoms. If you have moderate arthrosis they will likely resect the distal end of your clavicle and consider reconstructing the ligament to bring it down. The choice is really all yours and if you feel like you are really limited, it is worth consideration for sure. Good luck with your decision. Thanks for watching and subscribing if you did.

Then we begin the PT process all over again. Hopefully with better results this time because everything is stable and in correct place.

      If you’ve got a bump, it’s more likely that it’s above a 1/2 separation. I’ve separated both of my shoulders but don’t have a bump on either. From my experience, I wouldn’t go back to lifting weights until the pain is 100% gone. The shoulder is the most complicated joint in the body, and when you injure it, it takes a long time to heal. This summer and in the past week, I decided to do some extra exercising on my arms and my legs. Usually, the arm exercises just consisted of arm circles, sometimes pushups, and other things like that, since I wanted to slim down and tone my arms more (I also focused on some shoulder/collarbone area exercises to make my collarbone more defined). Nothing harmful happened. But today, I just looked in the mirror and noticed that there is a small bump near my left AC joint than connects with the clavicle and seems to be sticking out more than my right side one (my right one looks quite normal, nothing is sticking out on this side). It seems relatively new, and I don’t think the left sticking out has been there before. I usually notice little things about my body when there’s changes so I can almost 100% guarantee that it probably wasn’t like this until recently. There’s no pain or anything, but I can’t quite tell where it might’ve come from. Do you guys have any ideas on if it was from the exercising I did or maybe anything else? Gday from Australia. I have an AC separation suspected to be about a type II. How difficult is the surgery (in terms of complexity for the surgeon), if I were to elect it, depending on the type of separation? Is a type I separation easier and quicker than a type II. What are the surgical techniques for various iterations of this injury type?

    • @TalkingWithDocs
      @TalkingWithDocs  7 років тому

      Thanks for the question. If your separation is acute (or new), surgical treatment would typically not be required. It is important to assess the risks and benefits of both surgical and non surgical treatment. Occasionally you would require surgery in the future if your AC joint went on to become arthritic however having surgery acutely would not necessarily prevent that. With respect to the operation itself, there are multiple sites and videos that do explain the different procedures and it is fair to say that all surgery has risk but in the appropriate skilled hands it would generally have a successful outcome. Hope that helps.

    • @Diabolous3x
      @Diabolous3x 7 років тому

      I've been taping down the resultant step I have since I got the injury with strapping tape, I think this is better than putting it in a sling; the position of the arm in the sling keeps the bone at an awkward angle which isn't as conducive to healing as other angles to save as much of the ligaments that are left in place. I have the my hand placed on my ribs with the elbow out quite sharply 'little tea-pot' style. With the taping I tape it super super tight. The problem i've had is that despite offering to pay for the surgery outright, no surgeon has wanted to do it, so it looks like i'll be left with this grotesque bump on my shoulder. I feel like a leper.

    • @TalkingWithDocs
      @TalkingWithDocs  7 років тому

      It is important to know that if you decided to trade your bump for a scar, there is still no guarantee that you would feel better - as well, you would be exposed to all of the inherent risks of surgery so unless you had significant pain or dysfunction, most surgeons would not be in favor of trying to fix it. Hope that helps.

    Is this the same as Frozen Shoulder as I’ve been in agony with mine for nearly 2 years, I had a phone consultation last year because of COVID and Doctors surgeries not being open, and a Doctor sent me out some pain killers and stomach medication but nothing worked. As I’ve got Diabetes I was told they don’t like given the injection if they can help it. So I haven’t really bothered getting in touch with the Doctor since, I make sure I use that arm as much possible as it is my dominant arm. I take Parecetamol if the pain gets to much. I had a operation on my left arm for Arthritis several years ago so don’t want to go through that again. I think this is why I’ve not gone back to Doctor I’m a 71 year old coward.

    Hey ! hopefully you guys still reply. I injured my AC joint about a year ago ( fell off my dirt bike ) and I still have pain everyday. I am a carpenter by trade so it's very important I have full range /strength of my shoulder. Most days my pain is a 6/10 but can sky rocket to a 10/10 until I do some sort of cupping therapy or massage therapy to bring the pain back down. The pain also gets really bad when I'm trying to sleep and I can't lay on the injured side. I have tried multiple different methods to try and stabilize my shoulder / reduce pain however, nothing has worked. Right now, I'm frustrated with what my next course of action should be. Any tips / advice would be greatly appreciated :)

      Hi Madison. Sorry to hear about your pain. So best to see an orthopedic surgeon and get an xray. Options would include removing the distal aspect of your clavicle to reduce pain from the AC joint and possibly to reconstruct the ligament to hold your clavicle down. Hope that helps. We tell patients if you still have pain after a year that limits you time at least to discuss surgery. Thanks for watching and good luck.

    Hey guys, I injured my left AC joint while doing squats about 3 months ago. I stopped going to the gym for about a month and the pain did not go away, so I decided to return to lifting heavy weights. About a few weeks later, i saw significant improvements in my shoulder and it was pain free. But i reinjured my ac joint, and I decided to finally go see a doctor. The doctor didn’t necessarily access what grade my injury was, but the X-rays showed about a 1.53 cm gap between my ac joint and my collarbone. However my collarbone was still in place, so the doctor recommended no surgery and said that physical therapy wont help, and he said it will take about 2-3 months to heal. I just wanted to know your guys’ thoughts? Thanks

    • @TalkingWithDocs
      @TalkingWithDocs  5 років тому +1

      Hi fred. Without examining you or your imaging it is difficult to give a specific answer. Having said that, if the lateral aspect of the collar bone is 1.5cm above the acromion that would be either a grade 2 or grade 3 AC injury. There is always a gap between the acromion and the clavicle which is what makes up the AC joint. Often these injuries do require time to heal. Physio to strengthen the muscles around the shoulder certainly can be beneficial but often no surgery is required. Hope that helps. Good luck and thanks for watching.

    Not much useful information here. Make sure you see a specialist if you have this type of injury. I saw several doctors who misdiagnosed me as a grade 3 ac joint separation when it was actually a grade 5. By the grace of God, I found a surgeon who knew what he was doing and performed a “triple endobutton” AC joint reconstruction about five weeks after the injury. I am now on the road to recovery. I wouldn’t wish this injury on my worst enemy and the post surgical pain is unlike anything I’ve ever experienced, but my reward will be a full recovery from a severe shoulder injury that most physicians seem to know very little about. Hi Danny. It is difficult to give answers specific to your question without performing a physical exam and reviewing your imaging. Having said that, it would be uncommon for your separation to increase in severity without significant trauma but this could be confirmed with xrays. With respect to surgery, the main indication for surgery is pain and/or loss of function. If you are managing, in general, it is better to avoid the risks of surgery. Always a good idea to be assessed by a local orthopaedic surgeon. Hope that helps.
