All-On-4 Treatment before and after - Anna's Transformation!

    Advanced Dental Artistry in West Perth specialise in cosmetic dentistry, dental implants and rehabilitation of the teeth, smile and bite.     Dr. Kat: Hi, it's Adrian Kat from Advanced Dental Artistry here in West Perth. I'm gonna talk through an All-on-Four case, actually, this is an upper All-on-Four case on a patient, you'll actually see that her lower teeth are quite sound, so this is Anna and we saw her in May of 2015. The reason for treatment, which she had what we called terminal teeth, which is when the teeth have actually gone beyond a point that it can be saved. You'll actually notice in her mouth that her lower teeth are actually very sound, so there's no real treatment that we need to do on the lower teeth at this stage. On the upper, however, we've got a partial plate that replaces two teeth and if the plate's taken out of the mouth, you can actually see that there's gaps. You can also, there's a lot of gum recession around her teeth too, and these teeth are actually moving, so we can't save these teeth. Her partial denture looks like that, where you can see there's two teeth on the denture, but you can actually see that there's arms, these metal arms, that actually go to her molar teeth, so that when she smiles you can see these arms actually on both sides.
    This is Anna on the day of surgery, and you can actually see we do a lot of video here, and the reason for the video is it helps us with our planning. We can see how the patient laughs and moves, and where their lips go and that allows us to get much better harmony with our cases when it comes to their smile. We have an on-site anesthetist to put patients to sleep, and that's the safest way to do treatment.
    This is actually a try-in, and so two days after treatment, this is when we removed all of the teeth, is that we do what we call a try-in. This is the All-on-Four, the teeth are actually wax. So, you can see that the pink part is very, very red, but you can see that the teeth are actually the correct teeth and what we wanting to do here is just test to see whether the bite is correct, but of course, whether the patient is happy with the way that the teeth look.
    So, this is Ana, with the try-in in place, and what we're doing here is we're just having a chat. We're looking at some photos and video, and she's giving me comments to tell me whether she feels that the teeth really suit her. She's slightly puffy, because it's only been about two days after surgery, but we're just going to listen to some of her comments.
    I don't see any problems at the moment, since everything was quite...after treatment.
    Anna: Yeah? That's good. This is the most sensitive to me now because the gum is sensitive. But it's not sore, it's not painful. You know what I mean? But really, after the treatment, no pain. I...
    Dr. Kat: Have you took some...
    Anna: I didn't touch [Inaudible 00:03:03].
    Dr. Kat: Okay. Okay. I thought you took some on the first day but no, okay.
    Anna: No, I didn't. I only took my antibiotics, like I should, and then I started taking the inflammatory after the operation.
    Dr. Kat: And soreness at the moment, is it any soreness really? Or just a slight bit of puffiness?
    Anna: Probably just puffiness but it's not sore.
    Dr. Kat: We, of course, once we do this try-in, we go back to the smile design, and we're really then going to check, how we actually corrected the problems. One of the issues here is the midline is still slightly off by about half a millimeter, so we're just going to refine that before we do the final teeth. So, this is, three days after surgery now, and this is the All-on-Four Bridge finish so you can see the teeth are the same but now we've changed the wax to pink acrylic. Now, we can actually see here with the new teeth, of course, we're going to analyze her smile again to make sure that her smile is correct.
    Midline is now perfect, and you can now see in this video the change between her on surgical day, and just two or three days after. And here's a portrait of Anna.     She looks fantastic! I had my all on four upper and lower done here in Perth WA two weeks ago, aesthetically they look fantastic, now it’s simply a matter of me adjusting to my new teeth.

    I'm in tears and shaking as I see this. I used to be such an active happy overall healthy mentally and emotional. Now I'm completely unavailable and isolated :( ... this gives hope

    I have a partial up front to fill missing teeth they look very natural and I'm happy later I'll just get the missing teeth replaced with implants one at a time that way I don't have to be put to sleep

    What is the average cost per bottom and top? I used to have text book healthy straight teeth until my wisdoms came in and overcrowded causing severe deterioration over time. I would love to start with my top.

      Hi there, yes with all-on-four we have to take the remaining teeth out first before the implants and bridges are fitted :-)

    Too expensive in Australia I would imagine. India or Mexico, Poland..best options.
    Nothing wrong with their skill etc..but money is a hassle.

      Hi Linda, unfortunately price is subjective but people have to be aware that the training, materials and treatment is often not to the high standards found here in Australia. We sometimes see great results from people who travel overseas for treatments, and unfortunately not so great results as the team here sometimes have to deal with patients who have had failed treatment aboard. As long as people are fully aware of the risks associated, it is there decision and one we respect :-)

      Agreed. You have to be careful and check out the quality of the ingredients, certification, and other aspects.
      But you are supplying a good service for many who ARE able to afford it, so thats good.

      You've heard the term, "You get what you pay for." Other countries might offer the procedure for a lot less $$ but what will you do if you encounter a problem? And maintenance: You most likely won't be able to afford a plane ticket for cleanings every six months. This is an investment: Do your research, choose a skilled surgeon who's available to you, don't be afraid to pay for quality work.

    Anybody that would get their natural teeth removed, so they can have dentures, without the natural teeth being so rotten, they have 2 b removed for medical reasons , if your doing it only for looks , u have something wrong in your head, far as I'm concerned, cause having dentures sucks ,& it's about the worst thing u can do ,& dont do it , unless u have no choice

      We don't provide this treatment to anyone that doesn't seriously need it. It is very complex treatment that isn't used just for cosmetic treatment.

    You got rid of her permanent teeth and gave her a denture, those were not terminal teeth. And her teeth look fake, you also skimped out on the lab it seems.

    i stopped smiling at age 13 after a sever beating while in a Childrens Home, my teeth were damaged, ive slowly lost so many over the years, im 52 now and still dont smile, i dream of having teeth so i can smile but an average quote of $65,000 to $75,000 from a number of dental specialists is a never happening dream for me.

    All on 4, none on three! This is an engineering disaster waiting to happen ( eventually). It is similar to crossing a desert without a spare wheel.

      "... none on three ..." What does that mean? The procedure has been in existence for at least 20 years, and the success rate is around 98%.

      First of all, what is a spar wheel? Something you left in the third grade? Dr. Misch, who coined the none on three, was a brilliant man but he went to his grave never grasping the genius of Dr. Malo and the single most successful procedure we do in implant dentistry. Try to stop regurgitating something someone said twenty years ago like it's profound. It doesn't suit you.

      Summary: This retrospective longitudinal case series study included 471 patients (women: 286, men: 185, average age = 57.7 years) rehabilitated with 1884 implants in immediate function supporting 471 fixed full-arch mandibular prostheses and followed for 10 to 18 years.
      R: Twenty-seven patients deceased (5.7%) and 149 patients (31.6%) were lost to follow-up. The cumulative prosthetic survival rate was 98.8%; the implant cumulative survival and success rate was 93% and 91.7%, respectively up to 18 years of follow-up.
