Fromsoft usually has two models of the same boss, one for proper boss fights and one for everything else (cutscenes where you're interacting with them, NPC model, etc). You can see this with bosses like Owl, Lady Maria, Sister Friede and Isshin where their boss model is slightly taller than their non-boss model. This is done so that players can see their telegraphed movesets better. You can see a significant difference between "proper" boss fights and "NPC-turned-to-boss" like Leda and her party, Champion's Gravetender and Halflight, Spear of the Church where the latter examples tend to have their moves obscured.
You can see he's physically taller than his normal self, even if you like, unfucked his spine and made him stand upright. He is like, waaaay bigger than how he was at the start.
Putting Noel (in her Nun outfit with horns) right behind Panko, who had just finished talking about guys getting drained by succubi, was a brilliant choice.
It's amazing how Noel, a long-time Dark Souls fan, was this bothered by the gore in Shadow of the Erdtree. From Software really upped the ante in that regard.

From Software is a Japanese company, so they were likely designing the game around the CERO rating system. CERO assigns realistic depictions of decapitation and dismemberment the Z rating, which is the equivalent of the ESRB's AO rating.
FS was making a statement when they had Midra withdraw the sword while impaling his own hand. The pain a normal person would've felt in their hand didn't even register for Midra because of the level of torture he's endured up too that point. It's evident when you see every barb of that sword is impaling every part of his torso and sticking out off his skull. Who knows how centuries or even millennia been impaled with the Sword of Damnation. It was apparently bad enough that he was to tear, not cut, his own head off just to get the sword out

Like, this DLC was the first time of all souls games that I've experienced fighting bosses for days and this guy just died second try, all of his attacks are readable, I died like 15 time at Golden Hippopotamus right before him and it wasn't even a remembrance boss.
