Just like a kid adopted or a foster kid becomes siblings to the other. Not by blood, but still siblings because of the parents. Let's not pretend to play semantics here.

Obviously, people will be weirded out if they find out that an adult is going for a minor, but legally nothing can be really done (it varies from country to country, but the age of consent usually starts from the age of 14, like in the country I live in, which would give me the possibility to date a 14 year old despite being 19 😬; in the case of Spain, the age of consent is 16 which gives Nicholas a chance with Noah without having to face any legal consequences).

There must be some really young people on youtube checking out these videos... If you find the step-siblings trope tough to take, then you wouldn't do well with couples like Cesare and Lucrezia in The Borgias who are ACTUALLY blood related.

OMG The Borgias was THE BEST SHOW EVER. And yeah, they had mad chemistry.
I think for me the thing that makes forbidden romances good is that the narrative acknowledges that their main couple is toxic sludge. When it comes to incest you can really bring forth how toxic family can be when trapped in a box with each other, the power imbalance and codependancy that can come with it. Especially in a movie like this, were the older step brother is taking advantage of his troubled 17 year old step sister. Nick is obviously a terrible person for that alone, and to see that explored for the awfulness it is could be a compelling story.
My best example would be The Coffin of Andy and Leyley, a story with a whole bunch of different degenerate themes including incest between our two protags. They're both horrible, sociopathic, possessive, traumatised and damaged individuals whose relationship is build on unhealthy codependancy and shared trauma. While they have a lot of chemistry, their relationship isn't ever sanitised or romantisized or not shown for the problematic mess that it is. Same goes for Happy Sugar Life (they aren't siblings but they're 16 and 8 years old respectively).
Long story short: If you're gonna make a forbidden or dark romance, actually make it as problematic as it would be. Because if the relationship is as fluffy and sweet as other normal couples then what even is the point? I haven't watched it, but if the story not only acknowledges the predatory and unhealthy nature of Nick and Noah's relationship but also properly explored it and have it be integral in the way their characterization, I think the movie could be a really nuanced story. If they just romantize Nick and the relationship, don't really address how horrible they are in a way that matters and instead present it as an ideal romance, then it's nothing more than The Kissing Booth and Twillight 2.0, another dumpster fire belonging in the pile that consists of terrible romance movies.
