- I’ve never seen Lost, but I’ve been getting all sorts of comments lately asking me to react to the medical scenes. Turns out there were a ton of medical moments throughout the series, anchored by Matthew Fox as Dr. Jack Shephard. I reacted to the opening pilot sequence on the beach, Evangeline Lilly as Kate stitching Jack’s wound, Jack saving Charlie with some chest compressions/chest thumps, Sayid sealing Charlie’s head wound with fire, Jack performing spinal surgery on Ben’s tumor, Juliet giving Jack an appendectomy, and the wild “Do Not Harm” episode where John Locke brings Boone in with a broken leg while Claire is giving birth. Actually in other medical dramas like New Amsterdam, they do do quite accurate chest compressions, perhaps using a manequinn of some kind

I feel like you could still do Correct-looking compressions and maybe use camera angles to your advantage so you don’t actually break anyone. They could put a layer of padding on the patient’s chest so it looked like the chest was going down further, just pretend you’re pressing hard without putting your full weight on them, etc. they could definitely do better than they do in these shows.

I can't believe they cut out the part when Jack gets pissed and just punches Charlie's chest and he just magically starts breathing. Gosh, I miss Lost.

Yeah the defibrillator fist was pretty unrealistic but still one hell of an emotional scene.

It’s called a precordial thump, and actually can product around 3-5 joules of electrical activity to correct an irregular rhythm, almost like a mini defibrillator. Although the chances of it working are extremely low, and the person has to actually have a correctable rhythm, it does have the small possibility of working 👍🏻
*Doctor is forced to do a surgery in a man who kidnapped people of his camp including children and is only doing it because his girlfriend asked, so he cuts Ben's kidney and starts punching everyone around to have a chance to get away*

Yeah lol they're child safety/work laws where you can't have infants under a certain age for TV/movies
One trick they used on "Call the Midwife" is to use a premature baby at the age where it should have born. That way it's about the right size. Then they put cream cheese and strawberry jelly on it to look like the goop newborns are always covered in.
Can't use newborn baby's because I think of labor laws but some of these shows use an 8 month old baby that can hold their head up and that always makes me laugh. It's just easier though.
To be fair, when he's telling the pregnant woman she'll be fine, he's probably more concerned about her panicking than he is with accurately diagnosing her.

Let’s be honest: Jack having X-Ray Vision and a clairvoyant capacity to diagnose patients would still be far from the most inexplicable thing to show up in Lost.
I recently tried to rewatch it as I used to watch in while it was airing, but idk I couldn't keep up... same thing happened this time around, I just casually stopped watching because so much weird and non-logical stuff kept happening and I was constantly losing track of it all... and that is coming from me who loves shows with 10+ seasons... but yeah I got lost within season 3 or 4... which is not pun intended....
@@KrisHe1 Yeah, the first season-and-a-half, /two seasons are good, they did tension really well, but the moment they started elaborating on any of it they just kept digging the rabbit hole deeper.

@@KrisHe1 that was the point also they had a lot of mysteries that was meant to get you LOST.... they still explained most of them in season 5 and 6 just that there were still few issues nonetheless one of the greatest drama.

He can simply say "Doctors would not perform this test or that test." Instead he exacerbates how "crazy" it is that they are having the Doctors do all of these tests. The Doctors are the MAIN CHARACTERS ON THE TV SHOW!!!!!
He thinks TV shows should be like real life.
I just wish that he would take the circumstances present in the TV show as a starting off point to explain how they are done in real life. Instead he does more criticizing of how the writers write the TV show unrealistically.
when he does not reaction videos he is VERY good about explaining how things are done and that disappoints me that he does not apply the same process to the reactions.

My dad was in the military (around 1970s) where he befriended a medic in his squad and they still occasionally hang out and tell some "war stories". He once told me that they actually trained to do this kind of "direct" blood transfusions in emergency situations. Every soldier wore their identification, blood type, chronic illnesses, allergies, etc. on dog tags around their neck and in a situation where someone's life depended on a blood transfusion (which I'd imagine would be fairly common when there's bombs and bullets flying around) and they could not do an immediate evac or didn't have blood supply on hand, they would find someone with a compatible blood type and use a special kind of tube that allowed a direct blood transfusion from a donor's vein to the recipient. As he told me he hasn't actually needed to do it himself, since they always had sufficient medical supplies at hand or some kind of medical vehicle close-by which was fully kitted out, but he does know of a different medic who trained with him and had to use this method in a hostile environment on someone who lost a leg to a landmine going off in close proximity and it saved the guy's life. It bought them enough time to move him to a safe evacuation point and allow them to transport the soldier to hospital by helicopter, almost like a action movie.
Maybe this would be an interesting guest to get for a video one day. An actual army medic to tell us of some of the stuff they have to do in combat situations to save patients from explosion or gunshot wounds.
I completely forgot about lost, such a good show "he could be a doctor, he's dressed like one, he looks like one... he's heroic"

I watched LOST all episodes, it was a weird show. The ending was weirdest.

@@SuperPriscilla92 Well the ending was about all of them being connected by the island, that even in their afterlife they were together. They didn't die all at the same time, everyone got there in their own time, the rest just waited for them. And i think that's beautiful.

That's why it's sometimes frustrating watching these real vs tv things, where you know why some things are that way and person reviewing doesn't and just shits all over it.
i always thought the chest compressions were purposely bad so that the actors don't break ribs >.>
To be fair Lost first aired in 2004. I was a freshman in high school and I'm pretty sure we were taught CPR with the mouth to mouth resuscitation back then. I do believe face guard/shields were available then too.
1) It's definitely not a good idea for Jack to have "stopped at this point" because of brain damage. Charlie had only been unresponsive for about 5 minutes, there was still some time.
2) I can't believe you didn't comment on the entire reason I clicked: Jack's notorious "smash down on people's chest" technique.
I mean if I crashed and was having contractions, the last thing I'd want to hear is "you MIGHT be okay" lmao

Studio C has made a skit about the “Breathe Charlie, breathe!” Part of Lost. When I heard that part in this video I smiled
one interesting thing I'd like to point out is that trying to save everyone and fix everything to a detriment is a pretty integral part of his character. He end up almost losing his mind at one point because he tries to control everything

I watched LOST as a kid, and wow... my parents were obsessed with the show. Zero noise while the show was on or we got in deep trouble. They would spend all week theorizing what was going on in preparation for the next episode.
In case you couldn't tell, Jack has a hero complex and does think he can save everyone :D

What I learned about Dr Mike from this video is that he will tell a frantic and worried pregnant lady that she probably won't be okay and then let her continue sitting amongst the fumes of burning plane segments

Just wanna make the point “you’re gunna be okay” is more about treating the victim psychologically than being honest with them. I have my combat life saver certification in the army and we are suppose to comfort the victims, tell them we are going to help them and that everything will be okay. The last thing you want is someone with an arm and leg blown off panicking further cause you are like “holy sh*t, I don’t think you’re gunna make it, you look pretty bad.”
Thank you for your service, for what must have been extremely traumatic for you while you helped others stay calm. Thank you is small ...what I want to say is bigger than the words I have.
@@dbopdbop Thank you for your support, but I should mention I have (fortunately) never seen combat. I have trained for it, but I have been lucky enough to not see the traumas of combat.
I think he was just saying that as a joke. It doesn't take a certification to realize that telling someone they're gonna be is so they don't freak out

To be fair to the mouth-to-mouth scene, this show originally aired in 2004, and in my research (I could be wrong) mouth-to-mouth was still used until 2009-2011.
Also, gunpowder doesn't explode, it burns hot and fast. It is actually pretty effective for cauterizing a wound.

could be. But cauterizing a wound is still some last resort until you can give proper help. Granted - it's a dire situation, but you don't do this until you have to.

Sayid told Charlie to go back to the camp because of his injury. Charlie didn't want to because he wanted to keep chasing the person who took Claire's baby. So what is the quickest way to stop the bleeding? I think what Sayid did with the gun powder was pretty smart.
True. I took cpr class and they told us that after 2011 they stopped teaching mouth to mouth in cpr classes.

LOST is my favorite TV show of all time and I think I cried a little when you weren't buying the medical scenes. Don't be trashing my Dr. Jack! Just kidding. I get it. The best chest compressions in Breaking Bad, because they actually made a cast for an actress so the don’t break her ribs.

Yes, you simply can't do realistic-looking chest compressions on a living actor.
They did the same in Grey's. In the episode when Mer fall into the water there were a lot of chest compressions. They had to to that as well so they don't break her ribs or stop her heart
Yeah it can be difficult to do realistic chest compressions, since you don't want to hurt the performer, but so many of them are just so bad. Like I can forgive them clearly not pushing too hard so as not to hurt the person, but sometimes they're barely moving at all, their hands are in totally the wrong place, or they're not even (or barely) touching the person.
But I think part of the reason Dr. Mike points it out each time is I think the average by-stander probably is going to be a bit worried about hurting the person, so it's one of those things where you do kind of want to frequently emphasize the proper way to do it in case you ever are in that situation.
Just fyi, Jack's overconfidence and need to save everyone is a thing with his character and not necessarily a TV show contrivance. Not that Lost is immune to those.

From what I can remember, I think older shows did the shallow chest compressions to avoid breaking the ribs by accident. Which I know isn't the primary concern when you need to do it in real life. Sure it could happen, but if it happened while saving the persob it's a "ends justify the means" type of thing.

I kind of wanted to see him react to Jack getting angry at Locke for not providing the right information about Boone's fall. I was curious whether that would indeed be a big deal here and if there had been any chance of a better prognosis otherwise.
You skipped some important ones, the ones I was actually hoping for :D (Jack punching Charlie's chest, Jack thinking if amputating Boone's leg can help, Jack's story about his life-defining surgery)
Jack literally just pounded Charlie back to life. IF Charlie ever wanted heroin more in his life, it would for the residual chest pain from Jack's incessant chest pummeling!
When Jack says "Youre gonna be okay" ... its just his character trait. He always does something like that through the show. :)

@@Jehty21 Im saying there is a recuring trope with Jack... him saying "I can fix it". There is even one or two episode focusing on this. Jack had serious problem with letting go, constantly trying fix everything and everyone around him. So when he says 'Everything will be allright' - there is more to this in his case than usual. Because he is convinced that he can do everything to actually fix the situation and be sure that 'everything be all right'. Even when he shouldn't say that in some situations.

It is such a character trait that they dedicated an entire flashback story to its origin.
"Her spine is crushed! I tell her everything is going to be okay, that's false hope, dad!"
"Maybe. But it's still hope."
Jack then actually makes the promise to fix a broken spine and happens to perform a once-in-a-lifetime miracle surgery... And a conflicted "man of science" is born. 😛

It's one of Jack's character traits/flaws that he tries to save literally everyone around him all of the time, no matter how hopeless or dire the situation
Doctor Mike never really understands that these shows are not medical training videos and that they have more important things to do than to spend 15 minutes showing every test being done just so a proper assessment of a tumor can be made.
@@claytoncourtney1309 I don't think you understand what this video is about, he's not there to evaluate the show on its entertainment he's there to evaluate them on their medical accuracy and that's exactly what he's doing
@@johnhostetler2167 however the shows are not trying to be medically accurate. As a watcher of a TV show I know they're not going to be accurate medically so Dr. Mike's efforts here are wasted.
He could use these circumstances to show what would actually happen, and sometimes he does, but a lot of the times he just gets mad that they did not spend 5 hours for a scene that is very minor in the overall plot.

FYI, Jack is supposed to be a super surgeon. And that scene where he cut the guy's kidney, well it might be unethical, but they were holding him and a bunch of others prisoner; so... And yes, Jack's thing is that he's heroic, always trying to save everyone. He's effectively their leader.
Lol yes I agree, what they were doing to him and his friends THAT was unethical. I loved Jack's character but in the later seasons he became kinda annoying. Sawyer was always amazing though 😂

Critiquing Lost's 'medical' scenes without having watched the show and the scenes in context isn't really productive. Just like you want to know everything surrounding an injury(that hurt ankle), everything surrounding why Jack, Kate, Sayid, etc. do what they do should also be known and taken into consideration. Also, mouth-to-mouth was still accepted practice when this show was made, Jack wasn't a general practice doctor; his specialty was spinal surgery, and actual chest compressions can't be done on actors in a television show without risk of injury. It's enough to show they are being done.
My mum, who was a nurse, used to always do the same comment “that is not a newborn” when they in tv series and films have birth scenes 😂

That’s one of my biggest pet peeves. Call the Midwife is one of the best shows I’ve seen depicting birth and newborns.

Yes. They couldn't correctly do chest compressions because they would injure the actor. Unless there is a mannequin or something over the actor. All chest compressions would be inaccurate.

He can hear someone call him during a plane crash.
He has every specialty of medicine memorized.
it's also a supernatural island that literally manipulates time, space, and reality... The island legitimately de-crippled someone the second they crashed there, so honestly I think its pretty fair that jack is able to do some abnormal things as well (seeing as the island is what is allowing him to do it anyway)

I havent watched the show in years, but I liked the island before it started getting supernaturally absurd.
The weird anomalies that made plane engines fail and government tests was cool enough imo, didn't need a sentient island.
@@TnT_F0X because the last scenes weren’t entirely real, they had died at the end and what we saw as the “flash sideways” was them reuniting to move forward after death.

Love your videos Doctor Mike. I am in search and rescue and a wilderness first responder. In wilderness medicine, we do still use rescue breaths. I'm also a CNA in home health and hospice and yes, in the situation I would NOT be using rescue breaths. When you have the modern equipment what you are trained to do is different.
I would be terrified if I had Dr. Mike as my CPR instructor I can imagine him screaming in my hear "now let's see those chest compressions, they have to be deeper, push with your body" but once passed I would feel like a superhero!

@@maribelmendez9012 It's easy in real life also. The CPR is not the hard part in real life, it's all the rest. Tossing in an IGEL or intubation, reading the 12 lead, figuring out what is wrong and what meds to give, etc.
Even that becomes mostly second nature after a bit.

I think about this every time you talk about chest compressions. I work assisted living in WV and during their CPR training in multiple companies still teach you do do mouth to mouth and will argue with you when you tell them it’s outdated. You get Red Cross certified while still being taught incorrectly it’s wild.

I love LOST and still got the impression immediately that they were not going to mess around with accuracy unless it was to illustrate that a character had some kind of medical experience. I think they knew that injury from a plane crash on a remote island was dicey no matter what and they just kept it loose. Always interesting to see a professional take on how fictional shows approach their topics of expertise. I imagine it would drive me crazy if I watched a show that played fast and loose with interpreting elements of my profession.
Next time I'd really like to see you do Outlander. There are only 4 seasons but there are a lot of medical scenes because the protagonist is a WW1 nurse forced into medieval Scotland. It's a wonderful show and from everything I've seen I think you'll enjoy it as well. Even if most of the medicines are outdated and tools primative. Can't wait to see your reaction

No show has ever made me feel as excited and intrigued as Lost.
Not a doctor, just giving my wife's experience. She was in the ER with suspected appendicitis when the pain suddenly just went away. Long story short, the loss of pain triggered one of the doctors because that WAS a sign the appendix had ruptured. Not sure how they checked or if they rushed her straight to surgery, but it was correct and they caught it very shortly after it burst.

I had never watched Lost until this last month. And while watching them do chest compressions, I had Dr Mike's voice in my head "No not hard enough, whole body. Don't do mouth to mouth. They doing it wrong". But yes 2004 I think it was still common to see mouth to mouth. I still haven't watched the rest, but based on up to season 5, the least of the concerns is how far fetched the medical part of it is. The series was called Lost, not because they were lost on an island but because most of the time, I think the viewer is lost. But still enjoying it none the less.

I love Lost and started rewatching it recently with my bf who's never seen it, and although I do appreciate the accurate medical info, it is super funny to see this kind of break down on Lost as opposed to a hospital drama, because Jack doesn't have a lot of resources lol. "How does he know that? How is he going to do that?" idk he either tries or 40 people die so he's winging it a bit. too bad another doctor didn't survive so he could have a break lmao
At 6:54 to answer you question, gun powered does blow up it just burns really quickly. so when you are shooting its in a pressurized capsule, the brass or shell, which creates a build up of more pressure behind the bullet until it essentially pops out and that pressure blows it down the barrel. so on Charlie's forehead as it has open air around it which makes no pressure to go boom.
