I think aqua had an extra layer of trauma people often overlook. He was reborn with his medical knowledge, but at the time of Ai's death, aqua as a child was helpless to use that knowledge to help. He was too small to apply pressure, fingers too short to try and reach into wound to stop/slow bleeding, and no medical tools either. He died before he could fulfill his promise to take care of her during delivery, and then he "failed" to save her again after the stabbing. Total helpless feeling.
Also, he was in the arms of her corpse slowly turning cold until he was separated from it. Though, not quite a spoiler but just in case people overlooked that.
yes he may be more mature than ruby but he still has an extra layer of trauma by whitnessing Ai's death firsthand, seeing and feeling how she slowly dies. also he may have been a doctor but he was a gyconologist. I don't think that he saw much death and dispair in that kind of field
If that descending aorta is severed, there isn't anything anyone can do unless they are in a surgery suite, and even then it's not a good chance. That was what he realized-- she was dead and there wasn't anything that could be done.
In his previous life his mother died while giving birth to him and he blames himself for her death and now in this life again his mother dies and was not able to protect her again. All that adds to his trauma and anger
You probably already know, but Ai was about to turn 20 when she died, so they would be close to 4. Now they are around 15. So the time skip is 10 to 12 years

In Akane's video they post later, there's a photo of Aqua sitting in the middle and Yuki and Mem around him, all in party hats. Assuming it was his birthday, he had just recently turned 16 prior to their talk, which tracks with 15 being the standard age for a freshman in high school. Since this episode takes place before they start high school, I think its fair to say they are both 15 here. Sorry about the whole block of text
I feel that him being a doctor before made it worse, this is my interpretation, he knew what to do to maybe be able to delay it until help got there, I think he even said something about putting preassure, but he was in a 4 years old body. Not to mention that even if doctors are more used to death, not all of them react the same, we see how he still held on to the memorie of his patient, even continuing her enjoyment of an idol group he wouldn't have known without her
Figured you would have been told by now, but at 0:48 Oshi No Ko was written by Kaguya Sama writer. Also this is the same universe, there are a few easter eggs. I or others will hopeful point them out as they happen

The reason why the official music video has gained so many views is that after watching the first episode of "Oshi no Ko," you will understand why the music is structured this way, why the visuals are designed like this, and why the lyrics are the way they are when you watch the official MV. The remarkable aspect of this seemingly unnatural English translation is that it not only matches the sounds of the Japanese language but also aligns with the meaning of the story. 4:15 One reason they cannot reveal who their mother is, mainly because that was their mother's decision. Ai was the person who decided to secretly be the twins mother while also being an idol. This kinda touches into some cultural differences, but in the Japanese image of "idols" they basically cannot have lovers, not to mention having kids. Especially active idols who are still making money out of this image. Of course, Ai is long dead by this point, so is her decision really that important to keep this a secret up to over 10 years later? That is very debatable. I would imagine they want to keep this secret simply because that's what her mother will do, even if she were still alive.
Small notes about idol stuff! - 3:17 This sounds like it would be a big idol group - maybe Morning Musume (or any of the other Hello! Project groups) or AKB48 (or any of the sister groups). - 9:02 underground idols, or chika idols, are basically indie idol groups. there's definitely big idol groups that came up from nothing (momoiro clover z started out busking, performing on the streets), but there's definitely a strong chance for sketchy management, sketchy exploitative contracts, sketchy jobs, marketing that brings in sketchy fans. some idol otaku, even foreign idol otaku, are fond of supporting these groups best they can since seeing them rise up can be awesome, but chika idol groups generally don't last long. the idol industry's long been oversaturated with aspiring talents. so that idol's complaints are honestly *very* accurate.
well ruby lied to him in episode 1 aqua notices her playing like a child in kindergarten while hes reading a book and he questions her on her age and she lies and tell him she was an adult woman when she died because she wanted to be treated as an equal and not a child

Aqua was 3 years old and didn't have the strength to apply pressure to slow the wound. Plus it was a najor artery so she was dead regardless.
