Theory: When Izuku went to the doctor and confirmed he didn't have a quirk, it could appear that he had stolen it (The doctor) because you can see that he is Shigaraki's helper. So that means that he is a Villain. Since he made All For One (The quirk i think) means that he can steal quirks and give them to other people. OR In the movie Heroes Rising: Nine said "This kid has another/multiple quirks." Only that Izuku might not know how to activate it or it can't be seen in public. (Or it's just Blackwhip) logic ;)
