Infectious Agent, an organism having the ability to cause disease. To break it, seek prompt treatment if ill and use disinfectant cleaning solution for the pathogen you have identified.
Reservoir, is where pathogens can thrive and reproduce. Table tops, door knobs, and people. To break it, washing hands, keeping a clean environment, and disinfecting surfaces.
Portal of Exit, how the pathogen leaves the reservoir, such as sneezing or coughs. To break it, wearing personal protective equipment, washing hands, properly disposing of trash, and covering your cough or sneeze.
Mode of Transmission, how pathogens are carried from one place to another, even on the hands of healthcare worker. Control airflow in negative pressure rooms, wash hands, disinfect surfaces, handle food properly, and observe isolation precautions.
Portal of entry, its the way pathogens enter the host, like breaks in the skin. Healthcare workers break this link, by practicing aseptic technique during procedures, taking proper care of wounds, washing their hands, and properly caring for catheters.
Susceptible Host, the host is the person who cannot defend against pathogens, such as the elderly or burn patients. Healthcare workers break this link, by treating the primary disease and identifying those at higher risk of infection.
