Yes saw something from Penn where he recalls his I think next visit and Dave asked them to really freak him out and not tell him before hand what they were going to do. im guessing that was an accident since it was live, especially with the quick cut back... tricasters can be tricky sometimes if you're setting up a preview of the next shot, and actually cut to it by accident
That was an attempt to get a close up shot of the trap that Teller was holding up. By the time the camera operator got the shot, Teller put the trap down. Just one of those things in live television.
Is the P&T Showtime special the one where they put some guy in a big tube that ran around the back of the stage, and then I think fed rats in behind him?

So I'd guess all the strings were set up until the point Teller did his distraction, then Teller set them all up to fire on the next try. Letterman must have known to not use his finger for that attempt. Definitely a "Don't try this at home" type trick though. I wonder how many people actually did try this themselves, only to get hurt?
I think there was only one that was safe and it was marked in some way that only P&T would know. The distraction Teller did with the blood for sure had something to do with it thought. I have seen this truck done by them several times and they do the blood thing every time so it has to be a distraction for something.

From the stories Penn has told, Letterman preferred not to know ahead of time what the trick was. So his reactions here were probably genuine.
