The man in the first scene North of the Wall is part of an organization called the Night’s Watch. People who join the Watch take oaths and the punishment for deserting the Watch is execution. That’s why Ned had to execute him at the beginning of the show. If you go back and watch the episode again you’ll hear Ned’s brother, Uncle Benjen, talking to Jon about it when he arrives at Winterfell for the feast. Jon was asking for his help convincing Ned to let him join the Watch and go to the Wall with Benjen.
I hate when new GOT reactors only pick up on surface details. You however, retained names, personal connections, and absorbed all the important information. For that good Ser, you’ve earned a subscriber 👍 Keep up the great work.

It's not a medieval times show. It takes place in an entirely different universe than ours. It could take place in the past or the future or at the same time as the time we're in now. It's a high fantasy show/book series - so everything is completely different from us. It looks "medieval" to us because the costumes are a big mashup of stuff that we think of as medieval.
