Side effects - nausea or may cause an upset stomach. (Don't worry body gets adapted but if it doesn't then prefer having fish instead of capsules)

Taking the capsule with meals results in better absorption and take it with water.
This isn't proved that milk with fish causes pigmentation but Still you should prefer taking the capsule with water.
