Did you happen to notice which end of the baby they are attending to? They are not performing surgery, they are giving this baby an enema. I’m not sure what you feel is unsanitary considering the situation. I’ve never seen anyone wear gloves to change a poopy diaper.

The worry is that the baby may have the kind of ineffective colon and/or rectum that can't or won't be "cured" by surgery/a stoma, in which case he's going to grow up suffering like...well, like a member of my own family!

And why isn't the mother holding the baby at least put the baby on top of you I understand that maybe it's better to lay them flat but sometimes a baby just needs to be held by their mother or somebody to feel safe right now this baby does not feel safe it feels violated!!
