Urologists need to learn to inform their patients of this option. I'm happy so far, 2 years out.
In my case there was significant discomfort for over a week (not really pain).
Maybe the prostate can't report discomfort but his buddy the penis can.
I had to discover PAE here on UA-cam.

I have gotten a few e mails from my posts so I will answer them the best I can. A normal prostate is 25 grams. As men get older the prostate grows and squeezes the urethra which causes all the problems. Many procedures to cut away tissue around the urethra. They cut with knifes, lasers and now water jets. Pretty good results but when you cut there is 3-6 week recovery. There is uro-lift that does not involve cutting but instead they out spacers which pushes the prostate tissue away from urethra. The “gold”standard is TURP which is cutting away tissue with a razor. All of these procedures are effective for prostates up to 70-80 grams. Simply, if prostate is too big there is too much tissue to cut away or too much tissue to use uro-lift. Having prostate partially or fully removed is tricky as large prostate equals lots of blood supply. PAE is great because it cuts off blood supply to inner part of prostate which is squeezing urethra. No cutting. Short recovery time great results. They have only done this with prostates that are bleeding (hematoria) or more than 100 grams. Probably works just as well with smaller prostates but now you are stepping on the toes of the urologists. I think this issue is political and financial. All I can say is that it should be done by a Board Certified Interventional Radiologist with specialized training. If not, find the right doctor and hospital. Lots of them around. I had fabulous result and it is relatively low risk.

PAE is great I highly recommend it for any man with BPH large prostate Mine was 120g 3x times larger and it shut me down from urination for months until PAE kicked in 2 months after doing it. I had the procedure in July. I was very uncomfortable for five days, I took a good amount of Advil. It is not really pain. It burns when you urinate, you feel you haven’t emptied and some burning near the rectum. But, by the 4th or 5th day you notice the flow is increasing and you empty your bladder. By 5th or 6th day pain is gone. Honestly, by the 8th day I felt as if I was “cured”. Three to four hours between urinating, only got up twice and the once during night. AMAZING IMPROVEMENT!

I was very uncomfortable for five days. Frequent urge to urinate and it burned. You get something called perineal pain. This is area near your rectum. I popped a lot of Advil. Psychologically, you get a little depressed because you actually feel worse. But, beginning g with the 5th day my flow increased, no pain and within 9 days I felt like I was cured. Amazing results.

This video and many of the other comments from those who have had this procedure are very helpful. I had PAE don just this past Friday and I am now in my third day of recovery. The symptoms are sever burning upon urination, difficulty with starting to urinate, fever that goes up and down all day and night. I found it encouraging to hear that it may take anywhere from 5 t0 10 days before I begin to see positive results. I sure hope that holds true because so far, I have regretted it.

I am scheduled to have the PAE procedure on 10/31/22. I took Flow Max and did not like the retrograde ejaculation side effect. My Urologist wanted me to have a Holep procedure which results in retrograde ejaculation permanently. Didn't like that option even though it is a more long term solution. The major side effect caused me to look for something else. The PAE seems to be a good choice. While not permanent it is my hope that in 5 years or so they will have yet another procedure more permanent without nasty side effects. I am 67 and feel 45. Retrograde ejaculation feels like I gave up on my sex life and feeling like a man definitely a no-go for me now with Holep and Turp procedures. Hoping this procedure is my stop gap for now to a better nights sleep (not having to get up to the bathroom so many times).

Is the PAE procedure done by a Urologist or a Radiologist? My Urologist, as far as I know, has never done a PAE procedure but has done Green Light Laser, Turp, etc. I had Green Light Laser done in 2006 for severe BPH and am still doing well although my prostate is now larger than it should be but I still uriniate fine. Considering PAE just to reduce the overall size of the prostate as it is pushing against my bladder causing lower back pain 24/7.

Any suggestions. Dr. Blake? My main problem is I don't want to destroy the prostate. So that is why I chose PAE and they used a very expensive Glue the second time instead of a Beeded solution. I've been to 2 different hospitals: Hackensack Meridian & Yale in New Haven. My insurance Highmark would not cover the procedure and I had to enroll in Medicare and Plan G.
The self-at cost at Yale was $62,000.00. There is a doctor in University of North Carolina, Ari Isaacson that does the procedure for a lot less. I would also question the doctor on how many successful operation they have performed. It's not a slam dunk: each of us has a different arterial map.
