With public being this much violent and scientifically ignorant, family physicians concept also will be useless. No one will take risk and will refer at the first instance itself which spoils the whole idea.

The discussion was candid and somewhat honest. However no solution seems to be suggested to eliminate the trust deficit between the doctor and patient, instead the doctor seems to take refuge in amniotic fluid embolism as a cause for the patients party becoming agressive in the event of any unforeseen mishap.
To say that, do you have evidence? Is the rate of maternal death due to Amniotic Fluid Embolism greater than that is as per epidemiological standards?
There should be patient counselling in a very strict way based on statistics, both local and international level.. about maternal deaths and child deaths during delivery and a reasonable time after delivery.... And this should be stated bluntly to the patient and their relatives... And they should sign under each sentence... Every possible cause of death and complications should be explained like this.
After all these explanations, if anything happens, and if the relatives act violently, they should be booked under unbailable clauses of law and should be kept in jail till the last and final judgement from a court of law... And punishment for these criminal acts should be made severe... like 10 years jail and 25 lakhs fine and all the damages done by this to the institutions and the reputation of the doctors and staff and the hospitals.
This type of patient and relatives counseling should be repeated atleast three times during the period of pregnancy and delivery..
And last point, the security staff of the institution should vigorously handle the situation and if necessary, use sufficient force to defuse the situation and the security staff and institution should be immune from this action.
All doctors should follow this procedure and IMA should do follow up in this.

സത്യം പറഞ്ഞാൽ രോഗിയുടെ , ഗർഭിണിയുടെ ബന്ധുക്കളെ ആദ്യം പറഞ്ഞ് മനസ്സിലാക്കി അല്ലെങ്കിൽ ബോധവത്കരിക്കേണ്ടത് വളരെ അത്യാവശ്യമായിരിക്കുന്നു. എന്തിനാണ് ഡോക്ടർമാർ ഇങ്ങനെ തല്ല് കൊളളുന്നത്!
പലപ്പോഴും അതിനുള്ള സമയം ആ എമർജൻസിയിൽ കിട്ടി എന്ന് വരില്ല.. Patient ഇന്റെ ജീവൻ രക്ഷിക്കാൻ ഉള്ള ശ്രെമങ്ങൾക്കവും അപ്പോൾ മുൻ‌തൂക്കം കൊടുക്കുക.. സംസാരിച്ചു നിന്ന മാത്രം മതിയെങ്കിൽ അത് നടക്കും എല്ലാം മിനിറ്റ് ഇൽ കാര്യങ്ങൾ കഴിയും

Regarding the staff pattern, and number of patient per doctor, why doctors cannot be strict in this matter...
If the ministry of health and the state administration is not responding to these requests, doctors and staff should use association powers to correct the situations...
Even the physiological processes will have complications and this should be conveyed to patients and immediate relatives..
The primary culprit in the situation is state political administration and my personal opinion is that doctors should use their professional association strength to the maximum.
If the doctors in other countries can do this, I cannot understand why Indian doctors cannot do this in India.
The one who goes to govt health set up are the poor ones. Government never give them any attention to them.
They're trying their Best.
