- Hello I'm Dr. David Richardson. I'm a cataract and glaucoma surgeon in Southern California. I've been discussing a holistic approach to glaucoma treatment. In other words, a treatment that takes into account things other than intraocular pressure.
In one of my earlier videos I mentioned that what's good for cardiovascular health should be good for glaucoma. And exercise is one of the things that we all know is good for cardiovascular health. As far as whether it's good for glaucoma that's not as clear. So, today, I'd like to discuss some of the nuances of exercise and glaucoma. So let's get going!

Thank you Dr. David, as a practitioner of Intermittent Fasting [16:8 protocol] for more than 3 years since 2020 following the Intermittent Fasting [IF] introduced by DR. JASON FUNG CANADIAN NEPHROLOGIST. I do believe IF does not increase the eye pressure of cataract and glaucoma, unlike some of the physical exercises mentioned in your video which should be avoided, if you have glaucoma.
Now comes another type of exercise which has not been mentioned by any body yet is:

At present, I am doing the “Wim Hof Guided Breathing Session - 4 Rounds Intermediate No Talking format” [ua-cam.com/video/YkE8AObpzm4/v-deo.html] in the morning after my morning prayer at 6.00 am on empty stomach as I’m on Intermittent Fasting [IF 16:8].
Having said that, I seek your expert comment whether Fasting and Breathing exercise can increase the eye pressure of glaucoma. I may be 86 years old but fasting 16:8 daily and daily breathing exercise I feel great.

I am so glad you are doing these. Thank you. This is not info that is easy for glaucoma patients to find answers to. What are your thoughts on jogging and using a rebounder?
My sense is that the cardiovascular benefits of jogging likely outweigh any transient IOP elevation that might occur during the activity. I'm not as familiar with using a rebounder so cannot comment on that activity.

Good intell
Exercise is grest for many paths, have noticed the past decade fighting this disease,.when I do strength train I always keep the head above the heart and don't strain nor squint my face. Focus on the muscle group and relax the other body portions
I was been treated for bilateral retinoblastoma when i was young (34 now). My right eye was removed and left eye treated with radiotherapy.
My vision has been stable for many years.
I begun weightlifting 4 years ago. the first year had no issues. In my second year i noticed a small change in my vision that was close to the blind spot that is caused by my scar/tuna on the retina.
I went to an ophthalmologist and had checks and they gave me the all clear saying it is likely the scaring on my retina changing with age. I have continued weightlifting and very slowly it seems the blind spot has increased over time. I thought i was in my own head about this as it has progressed so slowly, but i can't help but feel i have had some pretty significant changes to my vision, including a big increase of my field of view not being able to see, having a harder time adjusting to light when transitioning from dark environment, and noticing a change in my ability to see contract.
I have touched base with my doctor last week as i felt like again I've had a shift for the worse. I explained my history with weightlifting and was advised to stop performing heavy lifts and move to a lighter weight/higher rep range.
I'm hugely very bummed now that i potentially could saved a lot of my vision issues if i had stopped powerlifting earlier on. But I'm curious of 2 things.
When i have had my eye examined (every 6 monthly since i've noticed changes), i get confirmation there is no issue or nothing that can be seen. Is it possible to have retinal damage from acute increased in IOP without there being any signs of it from an eye exam?
I am a little scared of the idea of this progressing further, so it seems logical to stop heavy weightlifting. But could is till be putting my self at risk for further issues even if i continue weightlifting higher reps with lighter weights?

Hi Dr!! I m doing a weightlifting 2x week lower and upper days. I never use a weight higher than 15. Is that to much? I really don't want to do just cardio abd be flabby but I don't want to stress my optic nerve.

Hey doc, I'm enjoying this series to delve deeper into treating POAG beyond just IOP modulation. I'm wondering if you think that proper breathing techniques are a better alternative than having patients not lift as heavy. The benefits of strength as we age are so vast that I hate to tell a patient to back off on lifting. Would be nice to see a study on how proper breathing techniques affect lifting.
Absolutely! Proper posture and breathing techniques are essential to lifting without harming the back as well as the eye.
thks, from my experiance so far having done most execises discovered by man squating with 104, lbs get a bit of pulse in eyes, 109 much more. Hoping i adapt to that as well, i can handle the lift easy but can my eyes?
What about weight loss?
I am about 50 lbs overweight.

Dr. Richardson, thank you for your videos. They are very helpful! You mentioned that a person laying supine increases IOP, and prone increases IOP even more. How should a person sleep to best minimize their IOP? Should a person lay supine with a pillow under their upper back and head?
In general, yes. A slight elevation of the head is preferred.
I prefer list, verses dialogue while driving.

I have had Glaucoma for about 6 years I an 78 year old male. For the past few months both my eyes water after taking drops, I'm using Azopt and Ganfort.

I had to have the valsava manoeuvre last year bc my heart rate was elevated to 220 beats per min. I have glaucoma and recently my pressure in my left eye reached 59 and needed surgery.

I did aggressive exercises of eyes .Probably from frustration. I am doing this exercise for 30 years. Now my good eye seems to be dry. Can omega 3 will help me ?

Swimming should be fine unless trabeculectomy surgery has been performed (in which case it can still be done, but only with special goggles).
I used to sleep somewhat face down due to back trouble. I suspect this did not help with glaucoma.

Hello Ajay. So long as you maintain your recommended follow up appointments with your doctor you should be in a good position. When glaucoma is discovered early in the process we can often prevent significant loss of vision.

What about swimming, is it good or bad? wearing the appropriate equipment like goggles that don't increase eye pressure of course.

Yea I lift 35lbs and I have glaucoma just put in your eye drops as soon as you finish working out

Can you tell us what body toning exercises are allowed instead of the ones that aren't. That'll really help. There is a lot of clarity on doing not too intense cardio exercises. But what about toning exercises. Which can be done. Please do reply.
I think generally it's important to always prefer lighter weights & more reps instead of heavier weights & few reps... on top of that, you should avoid exercises where your head is below your heart (like bridge pose).

What if you do weights but are careful never to use valsalva? It seems the valsalva has become coupled with weight training.....
As it is important to keep one's core engaged during lifting (and this generally also results in a Valsalva), not sure how one could safely lift weights without doing so. So long as the weights are not too heavy a small amount of Valsalva should be OK for most with glaucoma.

U can tell is the pressure is high it the number is 23 and above , if it is 22 and below that fine.

Most likely, yes, during the active process of lifting. However, my sense is that typical Crossfit exercises are less of an issue than older styles of weight lifting (such as bench pressing).

Bodyweight exercises (with the exception of inversion activities) are generally preferred over "bench" weight lifting.

I am a 28 year old man. My left eye IOC pressure is 21mm and right eye is 20mm. Should I be worried?
What about glaucoma medication and excercise resistance? I find I can't break a sweat when I'm on Timilol. It takes forever to break a sweat.. this sucks.. I love to exercise.

So in essence, no weight training, no yoga, no physical activity beyond a slow death walk. This is horrible. So do nothing, take the meds prescribed and wait for inevitable blindness?! This is morbidly depressing. I was recently told that I have IOP. My life is about remaining fit, weight training - though not as intense as I used to but very intense by American standards. I also practice Krav Maga and I do intermittent fasting - as a lifestyle. KM training is intense. My routines (read: lifestyle) is what keeps me whole. Everything I’m hearing from doctors is that this all goes out the window. In exchange for what? While I understand that many Americans celebrate the idea of “not being able to work out” etc., some of us live by fitness. Again, there has to be a better way. And I’m committed to leveraging my knowledge and resources in deep tech to going beyond what’s being told to find answers. This has been the most depressing news I’ve received in years. But it tracks.

Thank you for your videos. I am 70 years old and was open angle glaucoma suspect for about five years until starting treatment with (now) 2 drops a day about a year ago. Currently on Latanaprost and Brimonidine with IOP of 11 and 13. I tried SLT at Ohio State, but that only worked less than 6 months. Would Micro Pulse perhaps be better than SLT? I enjoy walking and take care of mowing two properties. Would you also recommend against sit-ups. touching your toes and chin ups? I may be 70, but still hope to get back out to the country on 15-25 acre hobby farm again. If I lived closer to you I would love to come in for a consult, but I am in Ohio and hope to move to Tennessee or Kentucky.
Sleep elevated?
I slept with my torso elevated and it was the worst thing I have ever done. I woke up with all my weight on my legs and my belly was gigantic. I panic and never done it ever since. What could it be?
Yes but slightly elevated. They are wedge pillows for gastric reflux. They are great.

What kind of exercising were you doing?
It would really be better and safer not to make a video while driving. This is known as distracted driving because your mind is away from driving, and many people die from distracted driving....
