    How It Happened?
    The plane had taken off from Wichita, Kansas Airport
    A PSA Airlines plane, operated by American Airlines, collided midair with a Military Black Hack Helicopter at around 9 p.m. Wednesday the 29th 2025 while approaching a runway at Reagan National Airport.
    From the top Angle you can see Flight AA- 5342 was approaching from the South to Land at the Reagan Washington National Airport.
    This is surrounded by the Potomac River Running around Pentagon city vehicle to the the North you will find the US Capithol Hill where US President Trump Inuguaration took place a 3 weeks ago.

    I am a retired military helicopter pilot. I find it astounding, that the Blackhawk pilot and co-pilot did not see the jets Navigation Lights! Or, request that ATC confirm the jets altitude and directional flight path.

    Retired USAF officer here, my last seven years was with base level IG. I wrote, evaluated and debriefed major accident response (MARE) exercises, among other things. I would have loved to have had graphics like this to work with. Well done AiTelly.

      Reports are coming out that the helicoptor was at 300 feet and the maximum altitude recommended was 200.

    How is this UA-camr able to come up with much better graphics and a simulation to anything news outlets and even experts investigating this crash? The detail here is outstanding. Let alone the incredible detail of the simulation. Wow.

      1. There is no legal issue with making speculations on what might have happened in an accident. this is not a situation of libel or slander, anything will be fully covered by "here's what we think happened so far based on what we have been told".
      2. Even after they have all the facts, even weeks or months after the major networks rarely if ever create an animation this detailed and sophisticated.
      At the end of the day the real reason is because that's not Their audience there the people who watch that are usually low information and tune it on in the background. The news channels are big but in a certain way they are really dumb

      @@Laotzu.Goldbug The official television stations have to label their speculations as such, then there are no legal issues. But costs are strictly limited and one hopes that the average viewer will be satisfied with the superficial information as usual.

    Former Blackhawk pilot here…that chopper has an onboard system called a FVDNS (Full View Digital Navigation System). In simple terms, an FVDNS is essentially a 360 degree radar that surrounds the hawk in a 1/2-3/4 mile bubble of digital display radar. Meaning there is a giant 24.5” LED display right in front of the pilot that shows any object in full within that bubble. With clear conditions (no fog, snow, heavy rain etc). Could show clearly up to a full mile, but half a mile to be safe. That display will show the pilot what the object is, how big it is, how fast it is going, the direction it is heading & most importantly…how close you are to the object and if a possible collision is imminent. The ONLY way this could have happened judging by the footage, is if the pilot either had the system manually overridden and switched off (totally against regulations…some pilots do do it however when flying over wide open spaces because that screen is bright and can cause eye fatigue even through visors). Flying over a city, right over an airport is the exact spot to have it ON and pay full attention to it. Which brings me to the only other option, the system was on, functioning properly, and the pilot simply disregarded the system and flew by line of sight. Some blackhawks even have systems that will override the pilot (in case of a health emergency/loss of consciousness etc.) and evade the danger on its own. This model obviously didn’t have that unfortunately…it’s a feature usually only put on hawks going in to active war zones. But this pilot was flying in the dark, above an airport…NOT paying attention to instrumentation. That alarm should have been blaring for 10-15 full seconds at least before impact. I picture the pilot frantically looking around to see the obstruction (rookie horrible error…Blackhawks have notoriously poor visibility, which is why the panel is so important) when they couldn’t see anything they kept the status quo and continued on the steady course…right into a passenger jet. I still have yet to wrap my mind around this. Looking forward to more info coming out.

      Not sure what Blackhawk you have flown but Army helicopters don't haveradar
      and aren't referred to as choppers.

    I’m an Army veteran my job was a UH60 crew chief/ mechanic, I don’t understand how this flight path crossing situation was allowed to be so common?
    No way a flight path for military helicopters should be allowed to cross a landing flight path.
    There should have been a separation corridor for all aircraft on an approach or departure vector at any altitude below 2500 feet.
    All other aircraft should not be allowed to cross these corridors to avoid any collisions.

    Im a former Air Traffic Controller with ten years experience at one of the busiest military airports in the world during the end of the Vietnam war and several civilian airports afterwards. I was a tower and radar controller at Nas Miramar, San Diego, Ca. home of "Top Gun" at the time. I was fully facility rated both in the Control Tower and Approach Radar and controlled every type and kind of aircraft known to man at that time.
    The ATC rules and regulations that controllers apply to air traffic are written by the FAA (Federal Aviation Administration). One such rule as briefly explained in this well done video is the difference between IFR (Instrument Flight Rules) and VFR (Visual Flight Flight Rules). This is very important to understand. VFR simply means the weather is clear enough for the pilot to fly visually his aircraft and maneuver his aircraft, at will, to avoid other aircraft in his area. The pilot may not fly into the clouds or in reduced visibility. The VFR aircraft is not under positive ATC control, they can get traffic advisories if they ask ATC for it. Some pilots do, some don't. IFR means the pilot is qualified to fly in the clouds on instruments and files a "flight plan" with ATC before take off. IFR aircraft are under positive ATC control from the time they take off until they land. All commercial passenger aircraft are on IFR flight plans and provided ATC separation per the rules and regulations set by the FAA.
    When I first heard about these "corridors" along the river while commercial aircraft are on approach to that airport, I was appalled! Im sorry, a couple hundred feet crossing an active approach end of a runway with a commercial jet on short approach is inherently dangerous! Reports say this helo was at 400. With the city lights in the background and other aircraft taking off from the same airport, it would be easy to mistake what you think you see. Also I find the phraseology used by Air Traffic Controllers appalling these days and certainly in this occasion. I never heard the controller say "Pat25 Traffic 10 O'clock 1mile commercial aircraft on final runway33. Or whatever the position and distance was and keep repeating the closing distance till the VFR helo turned.
    It's often said by us that were in the ATC business, that the rules and regulations, are written in blood by the FAA. I don't mean to sound insensitive to the victims and families of this tragedy. These are the rules the FAA has instituted and most times it works quite well. However, I'll bet you see some rule changes the FAA will institute regarding VFR aircraft crossing in front of and underneath landing aircraft.

      Wow, super sad. Controller did speak to the Black Hawk what kept controller from giving exact location of BOTH of the planes close to that helicopter? Can you surmise?       @@sharonmcgraw5641 Yes he did speak to Pat25, but what I heard is not anything that I was trained to say when calling out traffic. It's very specific phraseology. I also never heard the controller give any advisory traffic to the on approach commercial jet either. That's a problem. Could have given the commercial jet the chance to see the helo below him and instituted a "go-around" by pulling up to avoid the collision. Again, VFR traffic crossing the approach end of an active duty runway and with only a few hundred feet of vertical separation between landing aircraft on short approach, is insane.

    A couple of my friends who were attending the US figure skating national development camp were aboard this flight.
    This video was especially helpful for people like me who knew some of the victims on a personal level. Everyone in the USFS community is still really sad about the sudden tragedy, but I feel that this video will help us all understand what had really happened to our skaters, and the other victims.
    I was really impressed with how well you made this; the quality of the animations, the explanation, and how fast you were able to get this out there. This was amazing!

    I have 41 years of experience in air traffic control and I can affirm that it is not good practice to allow unrestricted operations to any VFR aircraft in dense traffic at critical moments of arrivals and takeoffs! It's dangerous!

      It's literally a helicopter flight route. They were slightly high, but it's a normal flight route for them in DC.

    For the life of me, as a guy who works with Air corridors for the US Army. I still don't understand why the Blackhawk went from 200ft elevation to 300ft elevation before the crash in an area where 200ft was the ceiling in that corridor.

      It is as good as a civilian could explain, but lacks the details and evidence that we professionals demand before making judgements. As with my military pilot peers here, human factors not yet established were the primary failures - in the tower and other ATC buildings, who were those in charge and on the horn with the aircraft? If they were more of what we have seen as DEI hires, the former DHS in charge of the FAA should be prosecuted along with those responsible for hiring and training these tower flowers.

      the air traffic control should say which direction the plane ahead is flying so they know wen they pass "behind" it it's the correct aircraft. If 2 planes are flying opposite directions the aircraft passing between will always pass "behind" the aircrafts. So to knw they should state the direction of the aircraft they are passing behind. I think they thought the aircraft they were supposed to pass behind was clear so they took off and boom.

    OK to clear some of this up. The CRJ was about 4 miles out at 1200 descending. The tower told PAT25 and asked if they had it in sight. They not only said yes, but they requested visual - which means they are taking responsibility to maintain separation from the landing jet. When the jet was turning final, the ATC saw them getting close and asked the PAT25 if they had the jet in sight - they responded yes again, and again requested visual - they were responsible to maintain separation. The tower responded for them to pass behind the jet, that was about 6 seconds before impact. It should be noted, Military helos in that corridor are to maintain 200' altitude and hug the east bank of the Potomac. This is to maximize vertical separation for aircraft on approach to land. They were in fact at 400' and in the center of the river. 2 big takeaways - the PAT25 was flying on VFR (visual flight rules) and the inbound jet on IFR (instrument flight rules) - IFR always has precedence to VFR traffic; and perhaps more importantly, they told the ATC 2 times they wanted visual - which translates to "we are taking responsibility to maintain a safe distance" - which obviously they did not.

      Assuming this is correct, it seems like very important information.
      1) Abandoning safety regulations, as you describe them.
      2) The two craft are flying straight towards each other, how does the helicopter pilot not see the nav lights of the plane?
      Although one obviously shouldn't jump to radical conclusions, it does seem very strange that this should happen..

      @Dailymailnewz the current theory is that when ATC asked the helicopter pilot if he saw the plane the pilot responded in the affirmative. It is believed that the pilot was referencing the plane that was taking off in front of him not the plane that was landing behind of him.

      @Shriman-ql5uj in general, in this case, they train to ensure safety protocols should they need to transport government officials. One example would be continuous government. Most airports have strict perimeters and no flying of any kind is permitted. There seems to be 3 mistakes made. 1 - policy that uses a 3 man crew instead of 4. The plane was approaching from behind outside of the pilots vision. The 4th crew member would be the safety measure by designation of providing visual that's outside of the pilot's view when flying under VFR. 2 - the pilot flying under VFR seems to have erred in his altitude which wouldn't have happened under IFR. 3 - ATC should have confirmed that he saw both planes. Like this video my response is an hypothesis based on available information and my years of experience as the head of operations for a major airline.

    As a pilot I can tell this much, regardless of whether or not the helicopter misidentified the jet, the ATC is equipped sufficiently to recognize the dangerous proximity of two aircrafts and should have never allowed the helicopter to be that close of an aircraft which is on final approach.

      you will be surprised that many H60 don't equip ATC. I know some of military pilots buy a portable commercial ATC which is cost around $1000 and carry it in the cockpit when flying. but if it is a training fly, the instructor may ask the trainee not use it.

The same premise applies in Army Helicopter Combat Readiness training. Tragic as it is its partly a must to dust of the shock and awe factors.     This is really really great and absolutely true because a retired black hawk female pilot explained on CNN that in crowded airspace it's best to have 4 in helicopter in a congested air space over water, which refects lights crazily!! You were correct, they mistook the other plane taking off, she said the air traffic controller should have given the EXACT PHYSICAL LOCATION of the flight they were asking the helicopter if they saw, then they would have realized. Example: "Do you see flight ### at 5:00 o'clock?? Tragic, so pray for that controller as I'm sure he's feeling pretty awful now too. Goodness.

      The information about the plane crash in Aktau is false. The black boxes have been decrypted. When landing in Grozny, the plane had problems with visibility due to weather conditions and the instruments started to lie. They weren't showing the same altitude as the dispatcher had. Therefore, another approach was made, and then it was decided to change the airport. After that, the aircraft was damaged and the control devices began to fail. The only right decision was chosen - not to land in a mountainous area, but to fly to Aktau, where the landscape is flat, otherwise the pilots would not have been able to land the plane.
      All this time, the plane was guided and monitored by dispatchers. However, no one knew exactly what had happened on board. According to the pilots, the plane collided with birds and the oxygen tank exploded. They found out about the real damage after the accident.

      Another channel said there is supposed to be one air traffic controller for the choppers and one for airplanes, and because of governments cuts, only one guy was doing both. And the claim is that the control tower was understaffed. But there no doubt will be other theories as well, once the accident is further dissected.

    I was a Blackhawk pilot. You covered it very well. VFR does mean you take responsibility for your own aircraft avoidance, however, that is controlled airspace and they had to squak their transponder. That means ATC can see their position, altitude, airspeed and heading. Once the tower or ATC identified the converging tracks, they should have instructed PAT25 to make a left turn, no delay and instructed the jet to go around.

His colleague warned him that he’s not properly set to dive. The veteran instructor yelled at him by saying I’m a veteran I know what I’m doing, been doing this for many years. Sure enough that very same dive something terrible goes wrong and they crash into someone’s backyard-DEAD! Now if you ask me I say his arrogance caused him his life which I care less about but the young kid who signed his life away on a piece of paper died tragically and unjustly. Wonder, if atc would have gotten same tongue lash for trying to correct the course?

      It's a confusing statement though to say the slower-moving thing crashed into the faster-moving thing. It's a vague distinction which one hit the other one, that could imply which had the more speed, which made the final move that made collision inevitable, which has the most responsibility for the accident, etc. All of which could have different answers. So no point getting hung up on such a vague notion as if it's an objective fact.

    Helicopter pilot here. I believe the Helo pilot is at fault. If I was flying that corridor, I would assume to know the active runway in use before it was changed. The problem and confusion surrounding this incident is that ATC did not inform the Helo of the runway change. The Helo pilot would not have known this at the time. He would have been looking for aircraft on late final for 01, not 33 . What isn't clear is that helo pilot understood that there was a change of runway from 01 to 33, which may have had him thinking he had more time than he actually did to respond to the aircraft. In saying that, was he looking in the direction of the 01 final approach? or at 33 final, or somewhere else? I guess we will never know. If he was using NVG this would have made visual sighting of the aircraft even more difficult. The Helo pilot acknowledged he had visual of the aircraft and he would keep separation, he may have been confused as to which aircraft and to what direction it was at. Its a tragic accident and should never have happened. My condolences go out to the families of all the victims involved.

      FWIW to anybody, as a PPASEL who has flown in similar corridor airspaces under VFR, I will listen to ATC but if I know I'm crossing such a runway, especially when intersecting the usual glideslope, I will be looking very, very carefully for inbound traffic as well as departing traffic, regardless of what they tell me. Not ascribing blame here; just sayin'. It's a big paved runway, in the middle of a major city. There could be a NORDO emergency dragging it into there, regardless of Tower's instruction or anything heard on that frequency, or current ATIS.

      I would be curious to see if it was a blind spot caused by the windshield frame similar to “A pillars” on cars - when pedestrians walk perfectly inline with a vehicle’s A pillar - they essentially becoming invisible to the driver.

      The helicopters should fly lower. Closer to the river at their own peril. Away from normal traffic. Usually they do. This is their fault ultimately. Only so much ATC can do. They aren't the ones flying on VFR.

      Exactly! And nobody questions why was that training helicopter AT NIGHT flying in that area? Whatever the reason, it proves now that it is an area with a tragic background.
      A similar accident occurred in Japan on January 2, 2024, at Haneda International Airport, between a landing civil aircraft and a small military rescue plane heading towards the area affected by a strong earthquake hit the day before. Miraculously, all passengers from the civil aircraft were unharmed after exiting the plane in flames. Not the same fate for all in the military plane.

    I've been an airline pilot for over 20 years and have flown in and out of DCA hundreds of times. While your animation is very impressive and have made it easy for people to understand what may have happened; I have to disagree with you on a few points. 10 years ago, at my previous airline, I flew and have logged over 5000 hours in the CRJ700 and have flown this exact approach to runway 33 numerous times. At 3:45 in your video, you show the departing aircraft heading due east out of DCA, he was actually heading north initially. In the video you suggest that was supposedly the aircraft that PAT25 was looking at and had mistaken for the traffic to avoid. The runway in use at DCA at the time was runway 1 and that traffic would have been heading due north on departure and starting to make his turn to the left to about a 330 heading to avoid P56 (Prohibited area). On the real video of the crash, the camera is northwest of the airport and looking southeast. The aircraft that had already taken off was in its turn to the NW and clearly not the aircraft PAT25 was looking at. That departing aircraft would have been to PAT25's 3 o'clock position and climbing fast and starting to turn away from him. What most people don't understand is that there were multiple aircraft on approach behind AA5342 to runway 1. PAT25 is flying in a mostly southerly route and, I believe, when ATC asked PAT25 if he had the aircraft to follow in sight, he was actually looking at the aircraft (ironically another PSA CRJ700) on a 4 mile final for runway 1 that was behind AA5342. AA5342 was initially heading due north on final for runway 1 but on about a 4 mile final, had to turn right for a few miles to line up with runway 33. So AA5342 was to PAT25's left and he probably mistook the CRJ on a 4 mile final for 1 as his aircraft to follow. The controller didn't specify an azimuth or position of the aircraft that the helo was to avoid. If ATC had said "Do you have the aircraft at your 10 to 11 o'clock in sight?" I think that would have helped the helicopter crew to realize they were looking straight ahead but should be looking a little left for their aircraft to follow. Also, if the helo crew was on night vision, that severely restricts your peripheral vision, further complicating the identification of the correct aircraft to their left.

      Helicopter pilot intentionally crashed into the plane because she was flying in the direction of the plane for an extended period of time with high speed. In the last seconds the helicopter did not make any move up or down to avoid the crash. The motive was the same like in New Orleans.

      Excellent breakdown and analysis that confirms my initial thoughts about this incident. As an ATC myself I believe more detailed traffic info regarding the CRJ would have led to a different outcome. Providing only aircraft type information at night was totally insufficient tfc information. I also didn't believe that the Blackhawk pilot could have mistaken the departure as the traffic in question based on the images from the first videos shown of the accident. In my jurisdiction Tower controllers are non radar but still can provide traffic info with reference to position in the traffic circuit or distance and altitude on final approach to other aircraft.

    One correction is needed. The flight was scheduled to land on the long runway but was asked by the tower to divert over to the short runway to its right which it agreed to. Therefore, the route would have been straight into the long runway and then jogging right to align with the short runway- not a straight path as shown in the video. When you look at the approved flight path for helicopters, it is directly in line with the flight path to the shorter runway - the a/c was crossing the approved path to the bird with tragic consequences. Also it is confirmed that the bird was not to exceed 200’ in altitude and it was at 300’ at the time of the accident.

    You're kinda right. The AA flight was on an IFR plight plan but the phase they were in at the time was a visual approach, circle land runway 33. The instrument approach they flew was the ILS runway 01 but as they frequency changed from approach control to tower they were asked if they could accept a circle land to runway 33. Their acceptance of that segment of the approach indicated that they had the runway in sight and could complete the landing in visual conditions.
    Edit: The Blackhawk was supposed to be operated in a very well defined corridor below 200 feet AGL. Looks like they were well out of their corridor limits both vertically and horizontally.

    00:48 Wrong - the helicopter collided with the jet because the helicopter had the responsibility to avoid the jet. PAT stands for Priority Air Traffic This is the problem though, thinking you are the priority when you are not. FAA and Pentagon Officials who approved PAT traffic in this area should stand trial for manslaughter over this. Not matter how long they have been retired. Unaccountable government must stop.
