This totally reminds me when I read that the creator of the show said that the script for the actor of the Janitor sometimes have "Whatever Neil thinks of." because Neil(Janitor) constantly improvise for scenes (including the "I'm watching you" in the first episode.)
God of Lemmings Dude, she was trying to hold a laugh because Zach was killing this scene! Aside from that, you realize over the week of shooting for one episode there's a reading of every scene, numerous rehearsals and about eight to twelve attempts for each scene. It's possible that this was round ten or something. So unless the whole thing wasn't completely improvised of course Sarah knew what punchline was coming!
Being a Scrubs enthusiast, I go back and watch this specific episode for this specific exchange. it's honestly one of the best worded way to explain what happened on network tv. I give mega props to whoever wrote it.

As funny as this is to watch Carla's, Turk's, and Elliot's reaction to JD's unfortunate mishap, you can tell 1:39 that JD was really hoping his friends would be there for him, and it's a bit upsetting to watch this small moment of weakness in the group's friendship. That's what made Scrubs such a great show though, because they all rebounded from it... There will never be a core group as tightly woven as those characters, and it's sad to see Scrubs gone.
Ok it's hard to name the top of the best Scrubs jokes. It somehow is a matter of taste and there'll always be massive super nerdy fans around here who'll go "Noooo, not the best. Look season two, episode six, when Turk came home from work..." So you shouldn't have titled it like this. That being said, me personally, I can totally live with it! Loooove that scene.
