    Is there any reason why a doctor wouldn't do surgery for a multi level cervical fusion non-union?

    Thank you for the topic of discussion. I am healing after 360 multi level cervical fusion and wondering if there are certain movements or ways I may be holding my head since not wearing a brace that would prevent fusion from happening, I also picked up an infection and had to have a debridement and "vacuum out" of the abscess that formed after surgery that may put me at risk also for a "non union" to occur?
    Is it odd after such an extensive surgery not to feel the new hardware in my neck as I heal, I'm finding I'm tired and fatigued more easily since I had my first surgery on October 1st 2020 and I can actually feel the difference in my neck?

    Curious if Ablation or Epidural Injection would be better for Back and Spine pain following a non-union Lumbar L4-L5?

      Radio frequency ablation only works to deaden temporarily facet nerve pain. ESI is only for foraminal nerve root impingement. A non union or pseudoarthrosis requires a revision surgery to redo the interbody implant or cage and new bone graft like rhBMP2. Nowadays surgeons generally use BMP as it’s more potent than harvested iliac crest bone from your own pelvis aka autograft or allograft from another bone donor. BMP can be quite potent though and excessive use can cause overgrowth of bone into places unwanted like epidural spaces and lead to stenosis. A revision surgery for pseudoarthrosis may involve a different angle of approach to attempt to get to more endplate surface area so the second surgery succeeds in a solid fusion

      @@hkanchor just had Revision on April 13. L4/L5 the doctor came in from the front! Removed spacer put in a cage. Not sure what type of graft I am using a bone stimulator as I text.

    Doctor I need a favour if you can kindly help.
    I am an 18 year old
    I have been diagnosed with-
    Right paracentral L5-S1 disc extrusion causing thecal sac indentation with right s1 exiting nerve roots compression . Is it a disc bulge or herniation? I have recovered from pain by a lot but my doctor have given me antidepressants and too many painkillers. I feel dizzy and unwell and I think this is due to medication. Could you please suggest a solution. Should I go ask him to reduce my meds . I am asking this cause I have to travell a long distance to reach the doctor .Also there is Mild posterocentral L4-5 disc protrusion. I will be very grateful for your help sir.

    Hi Doctor ur videos r very informative and I do respect a lot. Sir I do have a query if you can respond Please, I had L5-S1 fusion due to bilateral pars fracture. I m concerned about adjescent disk diseases. I like to ask about how to minimize this risk in future and what are the option for artificial disk replacement and facet replacement. I am from India and not lot of option available for this query here. I did lot of search and found GERMANY spine specialist do lot of such surgeries...need help doctor please reply

      Do not smoke. Do not gain weight. Exercise regularly, with gentle core strengthening. Avoid significant twisting activities such as tennis or golf. Surgery should be reserved for severe pains and neurologic issues. Be patient, and stay positive. Technology continues to improve, but surgery should always be the last option. If you can, avoid being the first to try a new surgery. Unfortunately, the newest techniques have still not be followed long enough to know if there is true improvement over prior techniques, without any additional complications.
