yeah cause muichiro and tanjiro can't sense hantengu so maybe nezuko is unsure about it but she can still sense him somehow
Plus the foreshadowing of tokito asking tanjiro “what did you say?” When tanjiro said “timing was right” probably a sixth sense he didn’t even notice
those are Hantengu clones/emotions and he is UM 4, Hantengu himself is ( fear), who looks always afraid and frail, the other clones/emotions so far : Karaku ( pleasure - green eye), Sekido ( rage/anger -red eye), Urogi ( joy -yellow eye) and Aizetsu ( sorrow/sadness -blue eye), each of them have that written on their tongue.

The whole background story of Hantengu explains how he became a demon, the crimes he committed before becoming a demon ,how his demon blood art works due to his emotions.
Fun fact about him is that before he battled a demon, he was apparently 87 years old when he was sentenced to death long before Muzan asked him if he wants to become a demon.
