Health Assessment Questionnaire (HAQ) Demo

A HAQ Score (Health Assessment Questionnaire) was developed at Stanford to to allow patient to self-report their pain and function. It is commonly used for arthritis, and is often used on pre-authorization forms (special authorization, pre-approval) for expensive biologics.
This new online HAQ tool allows patients to submit their data to their care provider, or collect it themselves to track how they are doing. This video provides a demonstration on how to use it.
