Hi Dr, hope you are doing well, i recently met with an accident 22 days back and had an abdonimal pain and went through ultrasound scan and the impressions are as below
1. Irregular hyperechoic areas with out vascularity in right lobe of liver. No subcapsular hamatoma at present.
2. Linear hyperechoic areas in lower pole of right kidney without vascularity
-- consistent with liver and right renal parenchymcal injuries.
Can you please let me know whether my health condition will be back to normal and can i do exercises like before as currently when i sleep on the right side i am feeling irritation and pain below the ribs.

As per your report you shared,
You have a serious liver injury due to trauma called liver laceration as well as your kidney also affected with grade 3 laceration. It means subcapsular liver hematoma(SLH) that is spontaneous bleeding between the glissons capsule and the liver parenchyma which is involved more than 50% of the liver's surface area.
In this case what to do?
1) You required, repeated physical examinations,
2) Frequent re-evaluation of lab test,
3) Bed rest,
4) Gastrointestinal tract decompression,
5) Broad spectrum antibiotics,
This is the treatment of choice for this type of injury. If you follow up the treatment seriously within timeline and also need to consult with a good gastro doctor. It will take at least 4-6 months to heal that completely.
Currently, you can do walking for a while in your house area. Go for exercise after healing completely not now.

Surface Antigen level 1,19,238 lu/ ml means viral level is growing rapidly. Have done HBV DNA test? if yes have please share.

If liver function test of hbv patient is normal like ALT lavel is 24 and other are also normal
So what should it mean?
Sometimes ALT level found normal in liver cirrhosis patients. If dont have any other complications then it is safe.
You also need to following test in every 6 months :
1) HBsAg
2) HBeAg
3) HBV DNA
4) Liver Fibroscan/USG
5) ALT/AST
