When a horse falls behind the bit it's because the he/ she does not have to listen to the rider, because the rider is using a bit that doesn't suit the horse or the rider physically trying to pull the horse on the bit.There is no contact from the riders hands to the horses mouth. If you looked at a horse who was behind the bit ( side angle ) you could draw diagonal line down the front of his/ her face. This happens because the horse may tuck his/ her nose in to their chest to lose contact.
Often the horse will fall behind the bit when to much hand and not enough leg is used. This implies that the horse lacks forward momentum. By riding from your leg into your hand and having an educated lower leg this problem can easily be avoided.

one difficulty i see is use e of the word "bend; to someone with a dresage background that means bend in the ribs, not neck
