The only part that can't be real is when he's looking at the photos, there can't be a camera over his shoulder. I mean, he looks at the pics, but they stitch that sequence in afterward for effect. They do this in other episodes and later, the movies.
For reference the guy playing borat and Ali G Sasha Baron Cohen studied at Cambridge and is super smart.

it isn't staged but the reason the people go along with it is because they believe they are being filmed for a documentary by the Kazakstan Government and they know they are on camera so they try their best to behave themselves and be polite.
