Combined with what other people have said, i think lesley got hurt and her son david died in an accident related to a car. It could be david was in the street and she rushed to help him and they both got hit but only she survived. Lesley is in the attic of the dollhouse and there's another set of stairs going up from her floor, so we can assume she isn't the one really pulling the strings. She's also in a form of costume since no facial reconstruction surgery would use those colors and thickness of stitches, *but* her costume is different from the other "people" we've seen, or st least yg can see when he's powered up, since they are entirely covered and are meant to blend with their background. Beyond that, everything is up in the air.
