You don't hav to fill your details at all ..in the first page all details already will be there you just need 2 person i.e. referee details to be filled in 2nd page that's it ...and post that via postal address given on instruction ...that's it ... You have cracked the exam it's the most difficult thing and all other things which you are thinking what to do how to do..this will be done smoothly no need of worries 😊..by the way congratulations

I m confused as you say during DV only name of referee is only required

Try to submit another one also otherwise you may receive appointment letter late ...or you may contact to zonal/regional office and inform them about this...

No issue if you are giving a referee of your same surname..and the case is against your father not against you ..so it will not impact you ...
