So they didn’t respawn automatically? I heard people say that it takes a week in the game for everyone to spawn back…

can confirm most of the NPC´s respawn after a week, some may take a longer period of Time. If you cannot wait for your Quest objective revive them one by one at a Morgue.

Is this plague part of the story? or its possible to avoid deaths and use this item in somewhere different

Not part of the story, it’s a mechanic that pawn can caught a plague and they can infect another.
If your pawns have a symtom of the plague you have to send them back or kill them.

the plague is totaly optional bc is like pkrus, it infect randomly pawns, if it let breakout when u rest in an Inn it will elimante all npc in the town, but u can play a run without encountering it, btw there a chance that when u fight a dragon the move where it grab a pawn it can infect with the plague
