  • Dr. Ebraheim’s educational animated video describing knee arthritis and knee pain .
Causes of knee pain may be related to:
    •Sports
    •Trauma
    •Inflammation or arthritis
    •Osteoarthritis
Cartilage is a tough and flexible connective tissue that gives shape, support and provides a cushion between bones in the joints. Unlike other connective tissue, cartilages do not have its own blood supply, instead, it is composed primarily of water, chondrocytes, collagen, and proteoglycans, which produce and maintain a structural matrix giving cartilage tissue its form and function. Proteoglycans are composed of numerous glycosaminoglycans attached to a core protein. The predominant glycosaminoglycans in cartilage are chondroitin sulfate and keratin sulfate. Within the cartilage matrix, there are numerous prototype and monomers attached by link protein to a molecule of hyaluronic acid. This is all woven with collagen to form an elastic and compressible structure.
Normal articular cartilage also known as hyaline cartilage provides a smooth crystal clear gliding surface to aid in the motion of the joint.
The structure of hyaline cartilage is divided into four layers; superficial, middle, deep and calcified layer with the calcified layer representing a transition from articular cartilage to bone.
This cartilage is designed to achieve and maintain proper function over the majority of a person’s life. If the cartilage is subjected to excessive weight, overuse, improper alignment or injury, it begins to wear away leaving bone to rub on bone.
Articular cartilage does not have the ability to heal itself, however, there are several treatment options available to aid in the repair of injured cartilage.
The knee is the most common joint in the body with cartilage injury issues. The knee joint is composed of femur, tibia, and patella. There are two types of cartilage in the knee joint, the articular cartilage and the meniscus, which is a shock absorbing cartilage. There are also important ligaments within the knee joint, the anterior and the posterior cruciate ligaments.
The goal of treating cartilage injury is to reduce pain and improve the function of the affected area. Anti-inflammatory medications are commonly used to combat-injured cartilage but they can potentially cause cardiovascular or gastrointestinal toxicity.
Damaged cartilage can also be treated with a series of injections. Steroid injections such as cortisone are powerful anti-inflammatory drugs used in the treatment of osteoarthritis. Hyaluronic acid is a substance produced naturally in the body and is present within cartilage when isolated. Hyaluronic acid is a thick viscous solution that can be added to the natural hyaluronic acid of the knee joint. This solution uses purified Hyaluronic acid derived from either rooster combs or genetically engineered cells. Glucosamine is another option for treatment that can be injected directly into the joint. Glucosamine is a modified sugar that is formed by the human body and is used to form larger molecules involved in the formation and repair of cartilage. Other methods include gene therapy, platelet-rich plasma, growth factors, stem cells, drilling and debridement of cartilage may help with injured cartilage.
Occasionally the cartilage is replaced, regrowth and implanted. When the cartilage is damaged, the choice is often simple as the patient will need a total knee replacement. Many different types of designs and materials are currently used in total knee replacement surgery. Nearly all consists of three components, a patellar component, which consists of durable plastic, a tibial component, made of a durable plastic cushion often held within a metal platform and the femoral component, which is usually made of highly polished metal. Total knee replacement is the most predictable option to treat severe arthritis of the knee joint.
When the arthritis is minimal, almost all treatment methods will work, however, when the extent of arthritis is moderate, there are many different treatment options presented, but none of them are predictable.
It is logical to pursue and recognize the efficacy of platelets and stem cells in cartilage regeneration, simply because it is better for the patient to be able to regenerate their own cartilages and for it to be replaced with an artificial joint.

I had knee injury and torn my ACL, PCL, MCL and LCL. Also, i had my literal meniscus removed. I had those surgery 3 and 2 years ago. I hardly climb stair, and cant squat! Im so scared and I cried when the doctor said i showed symptoms of arthritis when i have my knee stiff sometimes, pain when i try to stand after a long sit and knee popping. Another problem is i gained a lot of weight since the surgery and im overweight now. I wish I could do something... Im not ready for this. Im just 25!!!

Knee injuries are the worst. I ruptured my articular cartilage at a soccer game. I had my surgery and half of my cartilage is gone and i have arthritis now. I was a candidate for Carticel (ligament transplant that is grown out of my cartilage) but since i am 22 yrs old my doc said that hopefully i can heal better to avoid carticel. 7 months after surgery and therapy i still have a hard time running without any pain. My flexibility is good and i have stability but running is so hard and painful.

I injured knee playing football .. went for an MRI to see if tendon was torn... luckily it wasn't just major swelling... however the MRI picked up a stage 4 8mm diameter hole in cartilidge...! I wasn't even aware I had it .. I can still squat and run etc... but im not sure how bad it is... is 8mm big compared to whole of cartilidge on knee? should I have something done?

Sometimes the back of my left knee hurts like it shot out pain for a few second and when I touch it, it hurts. Okay I think it might be because I don't exercise a lot and this pain started to appear last summer on June 2016. But I get freaked out when I searched it up saying it might be blood clot or varicose veins. I just feel relieved if someone tells me I'm going to be fine. I can't do what I want if there is a person encouraging me to do it. I'm weird right? I depend on a person's comment for my emotions :/

i have a similar problem, my knee has a pain when i bend it, climb stairs, and especially when i go into a squatting position. It feels like it'll pop or have a slight crack if i went into a complete squat position.
Unlike the person in this video, i haven't had any knee injuries before. I THINK it might have been caused from when i jumped and fell from about a 7 feet height today. I didn't think i hurt my knee during the fall, but i felt the pain later on.
    Please reply, thanks so much!!

Many osteoarthritis patients are waiting for further research into platelet and stem cell therapies. In my considered opinion, these hold the best hope for a cure for knee osteoarthritis. Currently, insurance companies will usually not pay for these therapies, because the techniques aren't yet mature or thoroughly proven to be effective.

docs inject PRP platelet rich plasma that produces growth factors that attracts stem cells that help in generation of the cartilage

I'm Afsal from Kerala. 2 years ago, I had a bike accident, my right knee and bones are broken. Doctors inserted a steel rode from the top of thigh to heel. Due to that I cant bend my knee. Is there any surgical solution to make my knee bendable? Waiting for your kind reply, thank you.

    • @bigpardner
      @bigpardner 2 роки тому

Sorry; but I am unqualified to advise you. I will just say that I was in ORs in the US often as a technician for orthopedic surgeries and never saw, or heard of, a rod inserted all the way from hip to heel. Was the ankle also injured? If not why disable it with the rod? I have seen rods from just below hip to the knee (in the femur) and from just below the knee to the ankle (in the tibia), but never the entire leg. I guess I have not seen everything. Where you are I guess is high level of medical practice. Have you consulted with doctors to see if the rod can be removed entirely are partially. Perhaps your knee was so destroyed that they can not reconstruct a functioning joint, even a total artificial knee. Good luck.

I tore my meniscus one year ago due to a hiking accident and developed a baker's cyst. An x-ray diagnosed me with osteoarthritis, but I'm certain it was accelerated by the fall on my knee. My previous ortho gave me a steroid injection which did nothing. Then he gave me tons of nsaids which are killing my stomach. I'm still in excruciating pain and have difficulty walking. Needless to say, I'm ditching my old ortho for a second opinion.

I told her about the UT Ortho and about the pain management that is available at the center. Would this be beneficial for her. Her present doc keeps telling her to wait and give it more time. She lives within an hour drive to Toledo. I thought the pain management doc could help, or maybe there is something wrong with her knee replacement.

Glucosamine is nothing to do with cartilage. You should have more chondrocytes. In other words, your adult stem cells should be turned on for differentiation as well as proliferation so that chondrocytes can be regenerated.
If you take collagen, it'll become amino acid after passing through your stomach.
Osteoarthritis only can be treated by real stem cell therpay. Real stem cell therapy is to help adult stem cells, inside of body, to turn on the switch for repair of worn-out tissue. Autologous and/or allogeneic stem cell transplantation are completely wrong. Real stem cell therapy is nano-medication that's based on:
God formed man out of dirt from the ground and blew into his nostrils the breath of life.
Hence, nano-medication won't cause any side effect and/or complication to humans.
If osteonecrosis patient can be back to normal, osteoarthritis seems to be much light disease. How to effectively treat osteoarthritis:
      1. to eradicate inflammation
      2. to eradicate MMP (matrix metalloproteinase)
      3. to turn on the switch of adult stem cells, inside of knee, for repair of worn-out tissue.
All glory does belong to God !

so how do you do this? I had an autoglous cartilage implantation and a year later still in pain. I don't think than are many more options, except for glucosmine as mentioned above

@Firestormify why would you use PRP as opposed to stem cell injections?
My answer is there no stem cells available for injection . you need to get them isolate them grow them treat them then inject them
i use adult stem cells from the patient marrow as a concentrate in nonunion and avascular necrosis
these devices are available to help to concentrate the marrow
