Fact: In the actual myths, Mjolnir could be used by Thor because not of the whole worthiness thing but because of his magic gloves and belt that allowed it to use it safely.
Not only that, but the handle actually broke during the forging process in the myth (or a larger one couldn't be made because of Loki pranking Brok and Sindri, depending on version) and Thor was the only one who could use it effectively because of the unwieldy handle size and safely because of what you mentioned.
versions of hulk have actually lifted the hammer! the way it works in the comics is that it continuously gains mass whenever the person trying to lift it is unworthy. hulk tries to lift it, it gains mass, he gets frustrated and angry. the more angry he gets, the stronger he becomes. it’s gets to the point where hulk’s rage/strength outgrows the increase of mass.

And based on the fact he was in Hell he was worthy by Hells standards, and it stopped working when he returned to earth because the standards changed. That's my opinion anyway.
