That end face is honestly the perfect expression. He is so happy to go back, to rectify his mistake. Despite his joy though, there is a twinge of sadness, knowing that this next poor man will take his place. A death so inconvenient he couldn’t have imagined it, he could be there forever. He sacrificed himself for this man without even knowing the consequences. The question is will the joy of one’s freedom overshadow the guilt left behind.
I wonder how it works. Since new people are dying that means time is passing, but if he's been there for decades how is he supposed to go back to fix things? Is it one of those "return to the moment you died" kinda things? Is the timeline constantly shifting from all the people that go back?

Another possible ending here dis that in the decades he spends in that place, he learns to come to peace with his circumstances, and no longer feels haunted by how he left things, thus making his death less inconvenient to him, and letting him go.
