Since developing SleepHQ two things have really stood out for me.
    1. The Auto Algorithms used to adjust the pressure are really bad and i will be reviewing my therapy advice in the next few weeks regarding this.
    2. The AHI is not fit for purpose as a measure of therapeutic efficacy. It's doing more harm than good! The Hypopnea / Apnea flag thresholds are too high and as such too many respiratory events are going unreported. At best the AHI should only be used as a very rough guide. It should never be used as a singular method for reporting.

Thank you so much for your brilliant explanation, I surely understood why I feel so dizzy sometimes when I wake up and yet app is awesome, I just past another sleep test at home(with oxymeter machine) and I am quite sure about what the results will be... I think I will have to readjust the pressure

I have fell out with my sleep nurses. I was having 25 per hours they said that was normal. Without machine it is 60 per hours. Was stable when I learnt to do the setting but now 15 to 30 plus. Dont know whats going on so tired at wits end! What is the oxygen sensor how much. See one as a watch worn on wrist (with app) on Amazon £100 and the ring one with a wrist box for £160. Which do you use

      Hi Nick
I was just recommended your channel today after asking a question on flow limitations.
Never heard of it before but I’ve just used Oscar to read my data on my F&P machine and I’m having FL of over 200 closer to 250 or more every night.
Is this something I need to be concerned about?
I’m waking up exhausted and my recent sleep study said that my AHI has increased to 23 more during REM and oxygen levels go down to 70s.
But looking at the Oscar data other then the FL everything else looks good.
      Hope you answer 🙏🏼
Glad I was able to find your channel. So much useful info and I’m definitely following 😊

I can relate to this 100%. I used to go to my sleep doctor and he would say I was doing a great job with using the cpap. I would tell him I was still tired and didn’t feel rested. He would pretend like everything was ok.

Thanks so much Nicko. ResMed should hire you as a consultant to overhaul their practice. This is insane that a user of CPAP is having to change the model from the ground up. Maybe you should design a CPAP/APAP/BiPAP/ASV all-in-one and blow ResMed out of the water.

Thank you so much for your content!!!! Your channel has been a life saver. I received the new Res med 11 a week ago, after being on an old Phillips for nearly 8 years. I have had to try to adjust the Res med 11 nearly every night to figure this out on my own as my Nurse Practitioner had it set on 6 and it is hard to get ahold of her. Tried that one night and felt horrible. I watched your video on 5 of you reviewing someone's data and used your suggestions for them to put mine on auto, limit the variations and raise the pressure. I used 10-13. I also had to change mask back to my regular nasal mask as the nasal pillow was oddly causing slight nose bleeds. Changed the setting on machine to nasal mask as well. I am still a bit tired so I'm concerned about my blood oxygen levels and can't afford the magic ring and not sure my California state insurance will pay for that. All in all I just wanted to say thank you for putting out this so important information. If the people from my sleep lab know of all of this, they are sure not telling me about it and I'm not even sure that the data they gathered from my last two sleep studies were accurate. All the more reason to be grateful for you and your content here. Much gratitude!!!

Yesterday was my first night using my cpap, and i am so grateful that i found you guys before i started using it. I was able to understand the data, i was able to use my cpap (airsense 10) properly thanks to your tutorials, and i feel more confident. I was feeling really bad when i got diagnosed, but now i'm starting to do the therapy and hope to improve. The first night is really compliczted, as it is a first experience at something not very pleasant, but im feeling more confident on the therapy now. I also had an apple watch to make tracking of my sleeping cycles, my oxygen levels and also compare it with the results i used using sleepHQ (really good app, it helps you a lot). When you combine all that information, you can see that things are improving. Thanks a lot for this video, i can see how all the data complements, and that you as the real time user, can track, monitor, and in cases change your therapy modes in order to get the best result. Thanks a lot!!

It's nice to have my suspicions confirmed, and even better to know if can improve me sleep therapy as well. Thanks!

I bought that O2 ring to find out if I should schedule an appointment with a sleep specialist. I spend hours sub 90% every night. I compared the results with my at home sleep study and they were basically the same. The differences in the data were so small that I am loving this ring. I am getting my CPAP on Wednesday. I can't wait to see what happens. I've had no energy my entire life and I have been successful in anything I set my mind to in spite of that, until I reach something I call "Burnout". If this can cure my chronic fatigue, depression, and anxiety I will be an unstoppable force on this world. I pray to Jesus this works. I will do everything I can to make this work. This is the first time I have felt real excitement about anything since I was a small boy. The possibility I might be able to live life like a normal person without medication actually has me feeling something. I have been emotionally dead since I was a child. Probably puberty. I don't remember most of my life. That could be as a result of this sleep issue too.

I started monitoring my blood 02 prior to using the cpap. The 0x ring was how I figured out I had sleep apnea. So I have been confused about the AHI and why it would be important to monitor if my 02 sats are dramatically improved. I went from continuously dropping into the 80'-70's to maintaining even between 95-97 all night. HR dropped as well. It is a great tool. My AHI is always between 2-4. I put the ring on my cousin the other night. She has been on cpap for 5 years. Her 02 sats were horrible! So now she has that info and can go back to the doctor for a increasing her pressure. Thanks for bringing this to light for clarification! I also find the need to monitor times I took the mask off very irritating...of course need to go to the bathroom and/or let a cat out or whatever...geez....

Very Interesting. I'd love to check my O2 stats to verify that my CPAP setting are correct. $250 shipped (USA) for the O2 ring is too steep for me at the moment. I hope in the future that insurance will either cover such a device or that the cost of the device becomes cheaper. I might have to try and make my own from one of the cheap chinese bluetooth pulse oximeters and a bluetooth arduino module. I've seen a couple such projects. I'd just be interested in using the device for a couple nights. You would think that CPAP therapy would include a trial period where they let you use a pulse oximeter to verify that your therapy settings are correct.

I don't have O2 ring but I checked my sleephq data for last 1 year almost and flow limit is 99% under 0.5, this is perfect I guess, but the question is, I still get up not refreshed and mostly tired during the day. Should I get a O2 ring? OR its not required in my case since my flow limit is all good.

The missus who works in pharmacy sleep apnea clinic had no idea that the AHI modelling that they are taught was so floored, as usual it looks like I'm going to be the test case so she can show her bosses so it looks like I'm buying an oximeter ring. Hi Nicko, thanks for the video. Good and informative as always. I just get a look to my data at SleepHQ and mentioned that I don't have Flow Limit data on the timeline. 'Sorry, there's no flow limit data for today' but maybe this is because I use Dreamstation1 I suppose. Maybe DS1 doesn't provide information about flow limit. Maybe it doesn't have such sensor or the software does not provide data for this important thing. Also about all these O2 rings. I am confused which one is good, which one gives enough precise and consistent data. All of them hust don't provide with information about accuracy. Maybe this will help me to make a decision which brand CPAP machine to buy. I almost decided to get Lowenstein, but not sure about Flow Limit and also not sure if SleepHQ supports Lowenstein's Prisma. While a Resmed AS11 is too expensive yet or not available to all providers, and AS 10 is at the end of it's life as you say. And I am wondering why the manufacturers of CPAP machines don't integrate SpO2 monitoring in their machines as a standard or maybe optional feature. Maybe too much pain in the ass with registering as medical device etc.

While addressing cpap settings don't forget to address environmental factors. Clean filters, tubes, humidifier. A then check out the dust road your machine and in your bedroom. Vacuum floors, wash curtains, bedding, pillows etc. We always assume we've done these things. Yet just as often we think we have but haven't! Are your pets hanging out in your bedroom? Shut them out day and night! Are you eating things you're only mildly allergic too? How's your diet? If your not eating enough fruits and veg your not going to eat getting enough vitamins c a natural antihistamine and anti inflamatory. Anyway, I hated how the med was making me feel so I stopped it after telling my Doc. Unfortunately, my RS10 send the data cellularly to someplace where I can then see the results on the MyAir app. If I add an SD Card, will the machine stop transmitting cellularly? I think this data needs to reach the monitoring company because my insurance company wants to see my compliance so they pay the bills.

But my HR always averages 50bpm, good or bad sleep. I set my alarm to go off if it drops below 45bpm. Last night it went off 14 times because of low HR. I'm curious if this considered normal? Do others have similar results without issues? Any insight would be greatly appreciated. Also not sure if Sleep HQ supports Resvent iBreeze 20A.

Hope to see that O2 ring soon so I can fine tune my Resmed 10 machine better. I have been tweaking almost daily and things are getting better than the sleep labs ever had me sleeping but I do find that running CP instead of auto generally results in more stable breathing but I still am a work in progress. My ideal tweaking is to get my O2 levels to stay up as I do have some breathing issues due to physical damage such as a broken neck as a baby, many brain traumas from bicycle accidents in my younger years, and Methane gas poisoning in my teens. Really looking forward to the O2 ring. Keep up the GREAT work.

G'day Nicko and greetings from Brisbane I'm now also a member of the (I guess mostly) older men & CPAP club, have been following your channel and have to say you're really doing a great job, very helpful. I'm still struggling with getting everything right in regards to my APAP stuff. My chin strap (which I need as I'm a mouth breather) keeps falling off during the night, my nasal pillow type mask does the same and so I wake up many times every night, afraid of a very uncomfy looking full face mask :-( Do you have any tip for good chin straps? Also tried a neopren type --> was itching like hell when the beard was growing and also stretched - so was quite useless...

    • @richardclary2903
      @richardclary2903 Рік тому +2

I went through similar trials when I was on CPAP. I was a side sleeper and started with a nasal pillow. I had to learn to sleep on my back to use the therapy. I tried specially shaped and all kinds of other stuff, but each time I tried, either the pillow would move and leak badly, or the mask would get very noisy because one of the side tubes would have to pass the entire volume of air needed.
Then I started opening my mouth when sleeping. I tried using several chin straps and dental guards. Even if they kept my moutn closed. I never got satisfactory results.
I decided to try a full face mask and I wish I had done that sooner. I now eleep exclusively on my side. A full face mask works much better for this. I can no longer sleep on my back, as I would have to use a chin strap and dental guard. That is just too much effort when I can simply sleep on my side.
In my opinion, chin straps work only for some people and are too much extra work. The full face mask is just as comfortable for me as the nasal pillow was and made getting to bed a simpler process. Best of luck.

Greetings from the US of A....I really like these videos. I have had to become my own Dr. when it comes to a lot of these things. So...started using mouth tape with the help of nasal tape to open the sinuses. currently using a face mask with the nasal pillows (the kind with a slit and not the nubby things that go into the nose). this seems to have helped dramatically. AHI numbers dropped to below 2 and I am sleeping through the night...despite some bizarre dreams. Never considered blood oxygen levels. I still feel sleepy when i wake up...naturally and no alarm. I am considering one of these O2 rings and see what's really gong on. I have not uploaded my data to your site yet but will do. Thanks gain. Cheers!

As I'm healing more and more...I seem to have far less drops and when I have one...it rarely drops below 90 (maybe 1-4x a week) before self correcting. All very interesting. I track my O2 nightly with the ring.

Hi Amanda, my advice would depend on how lethargic you are. If you're feeling okay and your 02 sats are above 90% then i'd advise you to just stick to side sleeping and perhaps elevate a little. If you are always tired then it might be worth checking out a MAS (Dental device) especially if you are young and dating etc. All the best my friend

@@CPAPReviews why the suggestion to elevate? How would that help with apnea?

What is the correlation between improved oxygen levels and increased AHI? Anecdotally I've noticed the same in my treatment when switching from a full face mask to nasal mask and increasing pressure settings. The flow limitation also appears to have decreased as a result. Initially this gave me some cause for concern as the AHI went up.
