"Complete transformation" LOL it's clearly Sharon in a fat suit. Christ, Matt Damon as the fat hillbilly in Deadpool 2 was almost completely unrecognizable. Not even close and acted like she looked worse than Kristen Wiig’s hair in the same film (Ghostbusters 2016), which was a godawful wig and makes her look odd. Charlize Theron really deserves her Oscar for "Monster", she completely disappeared into the character of Aileen Wuornos, that you'd forget that you were watching Theron, and not Wuornos in the flesh.
John Hurt was unrecognizable in The Elephant Man. I thought the actor's features were real after watching it when I was a kid. Charlize's monster character was also good.
Another example: Michelle Pfeiffer played an old, wrinkled witch in "Stardust". Though she only appeared like this towards the end of the film, as she used magic to look young most of the time.
I didn't know that Tom Cruise was Les Grossman until the credits roll and the most surprising thing about it is that i know that he is in the movie. I was like "where is he?" all through out the movie.

I would’ve put Valentina Cervi in Jane Eyre on this list. She’s one of the most beautiful European actresses in cinema today; I would say even more so than pretty much any of the A-listers in Hollywood. And yet when I saw her as Bertha Mason, she was so deranged that I didn’t even recognize her.
I can understand not featuring someone not well-known to English-speaking audiences, but Jonas Dassler in "The Golden Glove" (2019) is an astounding transformation from an incredibly handsome young man to balding, bulbous-nosed German serial killer Fritz Honka. The effects crew did a phenomenal job.
