You don't have to teach HIM anything - you don't even understand that this happens because of OTHER children not having been taught much about kindness and acceptance...and furthermore, why not getting surgery to change something once and for all, instead of having to go through the same kind of confrontation over and over again? Most of these facial problems can cause more serious issues as the kid gets older. The large lip can continue to grow and affect speech and eating, if it hadn't already. The facial tumor could eventually affect her eye sight, speech, eating, breathing, etc. Facial clefts can cause issues in breathing, eating, speech, eye sight, etc. Just because it's called plastic surgery doesn't mean there isn't a medical reason behind the procedure.
