I'm not sure I understand your question, the unconditioned stimulus already provokes an unconditioned response so it doesn't need to be conditioned. Maybe you're asking if an unconditioned stimulus could be trained to produce a different response, rather than the usual unconditioned response? Maybe if you could give me an example I could try to explain more clearly.
How do this theory applies in a real life counseling sessions? I understand that we make them learn a stimulus that resulted in a desired response, however that would mean we treat human just like animals who primarily follows their instinct.
This is applied in techniques like exposure therapy (to reduce unwanted associations) and aversive conditioning (to create negative associations) but as you note, things are much more complicated when we have to consider the role of cognition and social and biological factors, which is a limitation of purely behaviorist approaches.
