- Surfer's Ear is a bone growth inside of the ear canal that your body produces when exposed to cold water over and over and over again. Some surfers get it worse than others, and everyones growths are different. Mine finally hit a point of pain and discomfort that was unbearable, so I got it removed. This video is a story of that journey, and recovery, and information for anyone who's curious about getting theirs taken care of.

Congrats! I didn't know there was a newer way. I lived in Santa Cruz and heard some horror stories of the drilling. I'm glad you got to visit many of the attractions that makes SC so beautiful.

Stoked for you! Glad I saw your vlog because I began to have ear pain and pressure and saw a doctor who recommended me to see a surgeon. Even though I am deaf but enjoy listening to music soundtracks. But began to realize that it could get worse and painful if I didn’t do anything except to cleaning it. Glad you explained difference methods: drilling etc which sounds gnarly! Also my ear looks wax-free when I had it checked, it’s because of using tea tree oil which is the best. Thanks Brett!
Hey Brett, thanks so much for sharing the process. Stoked you got it done. After years of also putting it off, I'm finally heading to South Africa for the chisel surgery with Dr Martin Young. Worth a mention for those not in the US or without insurance - he does both ears for just over 4k USD. Bizarre that the drill is still the norm for places like Aus and NZ. Yew!

I had one ear done with the ear cut off around a month ago and am mid recovery, I found the ear cut to be the fastest healing part and not too bad but very jealous of your surgery getting both done at once. Your ears were sooo closed up mine were in the low 90% but yours are crazy. big fan of the channel cool to see your experience with the same thing

Brett thank you for the very detailed experience! My condition has progressed over the last 5 year and now I'm completely blocked on the right side and 90% on the left. Fortunately I've never had any ear pain, just trapped water and temporary loss of hearing. I went down the rabbit hole researching my best options and I didn't find the rabbit at the bottom of the hole but I did find Dr Hetzler! I've started the process to schedule surgery. I live in Dana Point in SoCal so I'll have to coordinate pre and post OP, but your experience has already helped me with those decisions. Looks like I'll have some time on my hands post surgery, so I guess I'll be binge watching your channel! Thanks again.
Thank you for sharing my ears have been over 90% closed for decades . I have been wanting to get the new chisel style operation done . I had friends in New Jersey that head it done with the old school rotary tool and they both said ,”dont do it”. I heard about this chisel method. Now i know which doctor to goto. Thank you Brett. Bizarrely no ENT in Australia offers the chisel method. They all drill. Your closest chisel surgeon is in Japan.
Hi Brett...I'm really glad to learn of your recent successful ear stone removal surgery in Santa Cruz...hopefully, this will improve your surfing & balance issues a lot, & keep you from falling off of your surfboard so frequently in the future...plus, as you say, it must also be really nice to just simply be able to hear normally once again...hahaha--I had you there for a minute, didn't I buddy, with that crack about "balance issues" 😂 sorry, but I just couldn't resist poking a little fun 😂...everyone knows you surf pretty good, for a haole!

As an east coast surfer ive always loved watching what your videos. I never knew surfers ear was a thing until now and i'll definitely make sure to address any issues that may come up in the future relating to surfers ear. i enjoyed hearing abt this procedure and the reason surfers where ear plugs sometimes. ❤

Great video. Last week, learned I must have surgery. Could you have gone back home after your Sunday check and had the weekly cleaning done by another doctor? Is that what you did?
Had the same surgery 6 weeks ago here in Santa Cruz. Closures were 98%+ and 95%. So thankful not to live in mortal fear of ear infections. How did your recovery go? Hetzler really is the best.
recovery was mellow... after the first 5-7 days its life as usual till you can get in the water again.
Thanks for the current info on surfer's ear surgery! I've had it done five times over the last forty years.
There was a Dr. Money who did the chisel operation in San Diego around twenty years ago, but he made so much money that he retired early. He did mine and it was better than the cut off the ear procedure that I've also had. All you younger guys, listen up, this is no fun, it is serious surgery. I got just as sick as Brett. Take care of your ears! Wear ear plugs or a well fitting hood, they will help. Also, after the surgery the bone growth will come back maybe faster than it did the first time if you don't protect your ears.

So great to see this video. I had my ears done just under a week ago, there was no pain. Just a discomfort or slight ache. Day 4-6 has been the worse as I just want to rip the padding out of my ear.
Looking forward to getting in the water again soon and not getting infections in my ears!

Ive had the same OP, defiantly the best thing for my sanity, no more trapped water.
I've had the surgery 2x, started wearing ear plugs religiously and had no more issues.
What ear plugs would you recommend?

@@ifishforfoodnottrophys7483 Docs Pro Plugs. You have to deal with figuring out what size you are, but they are best I found.
ear plugs are clutch. I use Surf Ears. Dr. Hetlzer recommended any plugs that art accordion style design.
Getting my right ear done next week and not looking forward to it, thanks for lightening the load a bit.

Hey Brett, do you have any recommendation on any good ear plug. I have tried a few but have never found one that really totally prevents water from entering the ear. I don't get ear infections but mild tinnitus after surfing.

I use the Surf Ears... I've gotten use to em and don't find them a bother, plus I can hear better than other plugs.
Hey Brett, really enjoyed the video. I have one question: Was it more difficult to find your balance after the surgery since the inner ear/vestibular system plays such an important role in balance?

I´ve suffered from ear infections since 2019, then I had it examined more carefully and found out it was "Surfer´s Ear". Every time I paddle out I need to make sure no water is trapped inside my ear (always right ear), but every now and then some is and it sucks so bad. I didn´t know the surgery was so gnarly like you said at the beginning of the video ( they cut behind the ear and fold it to remove the bone, wtf lol). If I ever find a surgeon who does the procedure like yours I´ll consider having it removed, even though mine isn´t that bad yet. Cheers from Brazil Brett!
I'd just encourage using Ear Plugs now to keep from having to get the surgery sooner than later. Ear Plugs made it better but I waited too long to start using them.

Faaaaaark, tried to get mine done in April, they are 90+% closed and insurance rejected the claim, so have to wait another 18months 🥴🥴 like a tinatus symphony up in here.

woah why 18 months? that sucks.... maybe try restructuring your insurance? we called the doc office to get procedure details and then called insurance to find what we needed to get it covered. so we were good to go by 2023.
WOW, what a difference
I fought exostosis my entire surfing career. I've had the surgery 5 times. If you don't religiously protect your ears it will come back, and in my case reoccurence is fast. Hopefully, your ears may be more resistent, but, I wore squid lids and used multiple types of earplugs. I have an ear that's still over 90% closed and could use the surgery again. Good luck, and be vigilant!
Best to wear ear plugs AND a hoodie in your situation.
I like the amount of space he created in there. We used to say we want to be able to shove a pencil eraser tip in there for size reference, post surgery.

I've spoken to more than one physician that says Hetzler did not invent this chisel tech but seems to be credited for it. No scandal, I just wanted to clarify... I have only heard good things about his work. I went to another great doc because of my insurance and got it done in my right ear. I too puked pretty hard during recovery. I'd consider it a fairly brutal couple days. But wow, so great to have that cleared up. Just wanted to say that Hetzler isnt the only doc using chisels. My recovery took a little longer as well. But I was over 90% closed up like you. Best regards.
oh interesting.... maybe I misunderstood the video I saw about his procedure and practice then. it was talking about how he made his own chisels in his garage to try and come up with a solution because the drill method was too invasive and they were seeing negative effects with patients post-op. I know hes not the only one who does it though. he travels the world teaching others the method too.
Didnt mean for this to sound like hes the only one that does it... but he is the man in regards to it. thanks for sharing.
Doc Hetzler trained most of those in the USA doing the chisel method. I had to teach myself. Doug’s greatest contribution has been to spread the word among surfers that they don’t have to be drilled anymore. As far as I know there are only about twenty of us ENT world wide with this skill. But the penny has dropped among my colleagues that they need to explore this better way of operating.

I completely understand ear issues from surfing. I got severe swimmers ear when I was in Rodanthe Salvo & Buxton surfing a series of swells that lasted nearly 3 weeks. That's all I did 7-8 hrs a day for 18-19 days straight at 15 years old. This was in 1999. Started getting irritation a week in and then swimmers ear around day 11-13. Kept surfing anyway. Turned into a ear infection which then had to go home. Got worse and I refused to see the doctor. Then I woke up from a nap a few days after coming home and my Entire pillow case and a small portion of my sheet was covered in blood. Both my eardrums burst. The ENT I went to said it was the worst I'd ever seen which scared TF out of me. I was about 80% deaf in my left ear and 95% in my right for almost 3 months and then it slowly got better over many years. Fast forward more than 20 years later and I have lost approximately 30% hearing in my left ear and 50% in my right. I wear hearing aids a lot but not constantly. It's really frustrating not being able to hear things around me adequately. Plus knowing that my stubbornness did this to myself. Oh and I haven't changed. Last year my stubbornness left me in the ER for 5 days and the hospital for 16 days. I ignored symptoms and it almost killed me. It also causes me to be quarantined to my bed for about another 3 weeks and out of work for 6 months with 110k in hospital bills. So guys listen to your bodies and put your ego aside and go to a doctor. Don't be like me. God Bless

I have not experienced either... no equilibrium issues, or tinnitus issues.

Surgery is not likely to improve tinnitus unfortunately. Perhaps in theory but I certainly would not count on it!
Sounds like you looked into getting the top ear doc but didn't think about getting a top anesthesiologist. 😉 Super interesting procedure though; thanks for making the vid!
nah just my first time under anesthesia... plus they gave me some pain med prior and I never take stuff like that. just made me nauseous for a bit. pretty normal

How are you feeling post op at this point? Any issues or so far so good?
Feeling great. I guess cause my ear is SO open… I can get water stuck in them. But maybe that’s normal for people and I’ve just been use to near total blockage. The great part is I can get the water out now if that does happen 😂 where as before it’d turn into ear infections. I’m so happy I got it done… it’s changed my life the last year.
Since you are on the East Coast, did insurance cover it or was it considered out of network since they are on west coast?
It was considered in-network. I explained all that at the end. Yes it was covered but I had to call and figure out my insurance plan the year before to make sure it’s be covered

I went to Shohet in Orange County. They fully botched the first surgery and only got rid of 80% of the growth, so now I'm looking to go get both completed and heard Hetzler was the man for the job. Did Hetzler do both at the same time? Also, logistically did you just get a hotel and recover there, I'd be driving up from Newport.
Thanks for sharing. I wanted to ask: Have you been wearing earbuds while surfing? How preventable is this condition if one always wears them?
I didnt start wearing them till late 2021... too late. yes this condition is MOSTLY preventable if you always wear them in water below 68 degree. it is still possible to get growth... but much slower and less likely to have issues.
I wear them all the time now until the water warms up.

My ear Dr kept telling me that ear plugs wouldn't make a difference, after 2 surgeries I started wearing them anyways and it was the end of trips to the ear Dr

no I hadn't... didnt start wearing them till 2years ago. but ill wear em from now on. I'm gonna get Masen into them once hes a little older. I'd rather him not worry about it for now. Ear plugs help prevent the growth though 100%
dang thats horrible they said it wouldn't make a difference... as you know, they most certainly do. but you can still have the growth come back even with plugs if you are in cold enough.

I have surgery next week. My growth is like a 1/4 may into my canal so pretty far from the drum. I’m getting the surgery done in LA. A little bit nervous. Unsure if they’re drilling or doing the chisel.
How did it go? I was just told my left ear is almost completely closed and I need surgery. I’m a bit nervous tho
@@ianhopp so far so good. They ended up drilling because my growth was deeper than they thought. I have a follow up next week to remove some packing.
It def hurts first week. Pain killers will help with it, but I woke up in somewhat manageable pain (just hurts but not like debilitating) and had to pop a painkiller.
After the week it’s fine only problem is the packing. If you’ve experienced blockage in your ear (which is likely considering the circumstances) it’s extremely annoying and you’re pretty much stuck in that blocked state for a few months with all the packing. It sometimes gets worse as it scabs up and you have that hollow sound when hearing your own voice. Extremely frustrating.
The day before and leading up to the surgery, I was super nervous. Blood pressure through the roof due to my nerves. They gave me some Valium before hand and omg it made me feel so good and ready - def recommend that. I felt so good, I asked for a little more when I got into the operating room and they hooked me up.
Then they said “alright time to go to a happy place” and I literally said “oh yea, I can def go to a happy place” with a big smile on my face as I closed my eyes and boom woke up with a bandage.
Anyway all and all it’s not that bad. The scar sucks and my ear still feels funny but so far it’s worth it
Just had this surgery 5 days ago. Ringing in that ear is now pretty intense. Has anyone else experienced this? How long did it last?
Did you have it chiseled or drilled? Shouldn’t have ringing.
Common thing for first timers to be nauseous after surgery. Your head wants to come out, but you’re body wont. Just have to let it ride and keep your eyes closed. I’ve been under the knife 5 times in the last year (same friggin hip) and the anesthesia was the best part of each surgery.

If they had a negative experience they likely did the drill method… this changed my life. I’m so grateful and it’s been amazing
Water in my area is in the 50s year round. I always wear a hood, even in summer (keeps sun off my bald head). Do I still need to wear ear plugs?
Yes, I would
I don't wear plugs with hoods... but ive heard not all hoods are equal. I only use O'Neill and don't get flushed much unless I fall face first hard, so when im' surfing with a hood not much water touches my ears. Even if so, it's minor. I can't deal with plugs AND a hood... so uncomfortable, I'll just be wearing plugs when its below 68, but above hood temps. makes it annoying when you want to wear a hood, but maybe take it down sometimes. so I just suck it up and keep it on...
wearing plugs is still prolly recommended even with a hood, but its all about ear protection. so the hood IS protecting.
Will you be wearing earplugs every time you surf now? to prevent the bone from regrowing
Every time the water is below 68... thats Dr. Hetzlers recommendation. I don't wear them with a hood on, because the hood is the protection, and I experience minimal water inside my hood. But I've worn em every day since, and will anytime waters not warm and I don't have a hood on. they aren't needed in warmer water.

How did you figure out how to maneuver your insurance around to pay for most of it?
When I called to schedule the appointment the first time the woman made me known of the possible issue of my insurance not covering it. That was in October 2022, so we began looking to make sure our Insurance covered it before surgery in January.

So I've gone thru this rigmarole before, and it's pretty convoluted. Is there any details you can provide to help? I'm guessing going from HDHP over to PPO is better, getting the Code #'s, etc? It seems like they (insurance) can't tell you until AFTER the surgery sometimes how much it'll be which is crazy.

I had the opposite reaults for mine. Here in Australia they only do the drill. Its been a year and the scar on the back of my ear is so sore and sensitive still i cant even sleep on that side.
2. The tinnitus is still really bad
3. 10 weeks out of the water and I'm ment to get the other ear done now... Think ill push through and not get it done.
dang I'm so sorry to hear that... ya Tinnitus is a common issue with the drill method. 10 weeks a long time. hang in there man... once its all over it'll be worth it.
Stories like yours make me really mad that ENT surgeons are still pushing on with the drill method. It’s crazy when those of us who have bothered to learn the chisel technique has shown so competently that it is a method that is significantly better for guys needing the surgery.
@@brettbarley Update since November, I'm finally about to spearfish again and comfortable at 25m. Surfing with surf ears in is OK. Still hurts a bit to sleep on and ringing leveled out a bit. My ear has gone back to original shape and I no longer looked like a one eared dumbo! (maybe lol)
I'd urge anyone to get the chisel method if they can. Here in Australia they don't do it. I was living in Santa Cruz on and off for 3 years, wish I got it done then.
@@DrMartinYoung They write it off and say it''s not a thorough method and the drill is the only way to get it all out. Seems like a cop out to just use the only method they have studied. With more and more surfers getting it I would have thought there would be more choices here in Australia.
@@trevor_osborn they are absolutely wrong of course. I get results as good as any chisel surgeon without permanently hurting my patients, and doing the worst of cases as well.
Have you tried surf ears or plugs?
Yes I started using Surf Ears in late 2021 but it was too late. The growth was closing and beyond saving with plugs at that point. Helped for a year… but I was too far gone and needed surgery. Ill always wear them now as I mentioned in the video.
What’s water warm enough to not need plugs?

@@brettbarley I'm in San Diego,…. I've been hooding and usually ear-plugging since the 1980's (though I rarely surf anymore),… I've been a cooler/lower body temp type in general,… never had ear problems. But when I'd go without ear protection if the water's below say, 68F,…. the inside of my ears gets very uncomfortable, like it hurts sort of, as a reaction to the cold. I've been a hooder since the late 1990's when my hair was 'going',… except if the water's in the 70's with warm weather.

I do exactly the same surgery, but here is the crazy thing. In South Africa where I live and operate the WHOLE PROCEDURE in my hands comes in at around $4000. Same quality of care, same procedure, but one eighth of the price!

Just had Surfers Ear surgery on my left ear....MF that shit hurt when they woke me up from the surgery. I still need to do the right ear but glad I'm getting this done!
Well, crap - I'm getting the same procedure for both of my ears...they are screwed.
