  • Do I Have a Forceful Let Down? In this video - Part 1 of 3 in a series I'm working on about FL/OS - we discuss how to know if you have this condition. We'll go over signs and symptoms in both Mom and Baby that may indicate a FL/OS. Remember that you may not see all of these signs, but if you see three or more, there is a good chance that you have FL/OS.

    9 weeks now and my baby has these symptoms:
    Choking at the breast when i get my letdown
    Spit up, sometimes projectile
    Gurgling after feed
    Clicking noise
    Bad frequent gas
    Goopy/mucus poop (only started at 8 weeks)
    My symptoms:
    Engorged frequently
    Clogged ducts
    Milk sprays out
    At our 2 week appointment, the Dr said it sounded like CMPA due to the stool, but the mucus-y stool only began around 8 weeks. I was in the hospital at 7 weeks, and was pumping frequently to keep my supply (baby wasnt allowed in the hospital and i didnt have any stored milk- i exclusively breastfeed). When i was reunited with my baby my supply seemed low initially, but its back to being engorged every 2-3 hours and baby unable to empty the breasts completely!
    Im so glad to know it may not be a dairy allergy, but upset to learn theres new complications with being an oversupplier 😭

    Hi! So happy to find this video as I was going to make this my last day eating dairy ! I go to a motherhood group and they said I might have a forceful letdown also and just told me how I might be able to slow him down at the breast… never mentioned anything else! He had a great latch when he wants but when he doesn’t - there is clicking of course, he started pulling off the boob frantically and crying around 4-5 weeks old. Starting spitting up, a lot of green poo and once he finally soils a diaper it can be massive! Poor guy wakes up grunting and screaming, flailing his legs !
    I don’t have many symptoms aside from when he pulls off the boob my milk sprays.

      So sorry mommy 😥 Having a forceful letdown/over supply, can be harder to deal with than a low supply. It’s something that you will always need to be aware of.
      Are you exclusively feeding from the breast?
      Do you do any pumping? How much do you pump at one time? And is that in place of a feeding or between feeding?

      @@marycahillclc Hi Mary! Yes I am EBF! at the time of leaving my first comment I was pumping once a day. generally got any thing from 1-3 oz ( both breast combined) and that was in between feeding. Baby was a huge cluster feeder in the first few weeks and I was nervously trying to pump early on also. I have mostly stopped pumping as it was stressing me out and I have a small freezer stash ready for when I return to work already. I was stopping his feeds when I feel my letdown and burping more frequently. It seemed to help a little with what looks like painful BM but has reversed since then. I was attempting to go dairy free but being someone who eats mostly animal based is so hard and my gut kinda tells me it isn't the cows milk allergy anyways , Im just at a loss!

      @@hereforit How is baby’s weight gain going?
      Do you know about how many wet diapers babies having every 24 hours? (If the diaper is heavy, that can count as 2)

    Thank you for this video. It all makes sense now. We have all the baby signs…
    Any advice on how do I help my baby with tummy ache/gas/explosive poos ? Is it going to get better over time ? He also started to bite my nipples (mostly on the right side) so how do I stop that ? I was told that his latch is ok so I am not sure what to do…

      It will take some time but getting it under control will be best. It will be scary to think about purposely reducing supply but it will be worth it. It’s something that you’ll always need to monitor.

    Hi. Thanks for the video. Can I get the OS and FL at around 5months. I recently had abscess too and now the let down is painful

      A FL/OS isn’t usually painful.
      Is the pain on just one side?
      If a mom has a FL/OS, it will often get worse after a clogged duct/mastitis or abscess because of the extra pumping to move the problem but you won’t develop it because of the abscess.
      I would see your local lactation counselor to help determine if there is a clogged duct or see your doctor to check if there is an infection in the breast.

      @@hereforit honestly, I just took a break every time he would start crying. I pumped and bottle fed for a couple weeks and I thought I was going to have to do that from then on, but it was exhausting. I decided to give breastfeeding one last chance and if it didn’t work I’d switch to formula. I would remind myself to be calm every time I started to breastfeed and to expect him to cry, but letting things go and not stressing about it. My baby continued to cry as soon as my letdown slowed down, I realized he didn’t want to work for it, he also didn’t cry like that at night, so that didn’t make sense. So when he’d start crying I’d get up with him and walk around, get him to stop crying and get his mind off it. Then try again a little later. It took about a week or maybe more, but it worked! I breastfed until he was 10 months, I introduced formula at about 9. I started baby led weening at about 5 months, and he loved real food so much he wasn’t really interested in milk as his main source of food anymore, it was only for comfort at night, so I decided to stop then. I hope this helps. Sorry for the long essay 😅

    Well now I am totally confused. I was sure I had a low supply. But baby was definitely making wet diapers. Pooped every other day though. Wasn’t gaining weight as much as the doctor liked so they had me start supplementing. He did have a tongue tie but we got that corrected. A couple months in if I manually expressed I could shoot milk out, hit myself in the face once. Those were the days I thought I was finally making enough. I’m wondering now if my supply was fine but mostly foremilk. I kept asking people if having skim breast milk was a thing. Nobody took me seriously but it seems like a ton of foremilk is kind of like having skim. I didn’t get very much when pumping either which affirmed my thought that I wasn’t making enough….even with power pumping. I thought my stress was causing the low supply but one time I fell asleep while pumping and got nearly nothing out. I thought we’ll I can’t get any more relaxed than that😫. How do you confirm that it’s a forceful letdown? I definitely lived at the lactation consultant’s office but even they gave my baby formula.

      Usually foremilk/hind milk is something that I want moms to ignore or it just confuses them but for a mom with a FL/OS, it’s important for you to understand it.
      It is very common for moms with this condition to believe that they have low supply and there is so much wrong information on the internet and even from healthcare workers.
      If you have more babies, you’ll have this with every baby but each baby will handle it differently.
      If a mom’s condition is really bad, supplementing can actually help because it’s less stimulation on your breast so when you do nurse you don’t have so much of an over supply. When it gets more under control many moms can go back to exclusively breastfeeding by block feeding. ❤️

      @@marycahillclc is it possible to have this and not ever feel engorged?
      I don’t think I’ve ever truly felt engorged but my baby is constantly screaming at the breast and coming unlatched and gagging and needs lots of burping. He has 6 or more pee diapers a day and usually two very explosive poops. My lactation consultant didn’t say anything about a forceful let down at our initial visit but I’m really thinking this is our issue.

      @@morgan3619 most moms with this condition do battle with engorgement, but there are some that don’t have the problem at all. So it is possible.
      How old is baby? Is this your first baby? If not, how did breast-feeding go with the others?

    I had a low supply for the first 3 weeks, and I supplemented with formula and pumped to increase supply. Then, from week 4, completely stopped with formula, still supply on the lower end, but I trusted baby will increase it. So basically, from week 5, I stopped pumping and formula feed, and I did exclusively breastfeeding. Now 12 week old, he is crying on a breast, gaging, choking, my milk is spraying out, so all symptoms of oversupply. How come? Everything that I can read is that baby only by breastfeeding can not produce oversupply.

    I think this may be my problem. I stopped breastfeeding both my boys within 4 weeks due to poor weight gain. I thought I just had low supply. I'm on my 3rd baby and she coughs and pulls off during let down. She also always seems hungry. I don't respond well to a pump but when I do pump my milk doesn't have hardly any fat in it. My baby doesn't want to stay deeply latched and often clamps down. I'm now thinking its not a supply issue but a forceful letdown and too much foremilk. Luckily she hasn't had any weird poop.

      You won’t always see green poop. Every baby is different. A major sign, that your supply is not low, is wet diapers. If you’re seeing 6 or more every 24 hours, then you know it’s not low. Even if your milk doesn’t look fatty, it still has lots of fat. Though, if you condition is way out of control, that can cause it to be low.. do you see lots of wet diapers?

    My baby was born with low weight and because I had problems breastfeeding the first few days, hospital supplemented my baby with formula. I had to do that also at home because they put a lot of pressure on me that he reaches first his birth weight and later three kilos. With all the other troubles I had breastfeeding (him biting my nipples, he got gas/cramps 10 min after starting breastfeeding and didn’t want to let go of my breast treating it as chewing gum, pulling it, was hungry even after being breastfed, staying on the boob for an hour and didn’t want to let go and when I tried to unlatch him he tightened the grip on the nipple as if his survival depended on it, making clicking sounds, etc), I assumed I have low milk supply. I also thought he might have posterior tongue tie because breastfeeding was so painful for me. Now at 6 weeks and almost 4 kilos (his latch finally improved after craneo-sacrum session and some exercises to improve his sucking reflex) I’d like to wean him off the supplement to exclusively breastfeed. But what’s been happening in the last week (not all the time but a couple of times on both breasts) is that he’s gulping the milk down like there’s no tomorrow and choking so that I have to remove him from my breast over and over again so that he can recover. He has most of the signs of forceful let down (not all the time though), i have none. So what do I have- low milk supply or forceful let down?🤷🏻‍♀️

      This can be because of a tongue tie, but Mom’s with a forceful letdown over supply often believe that they have a low supply.
      Do you ever pump your breastmilk?
      If so, how much milk do you pump at time and how much in a day?
      How much formula is baby getting a day?

    My baby starts to choke on my milk and have a lot of burps, she didn’t have this in the first 4 weeks but now it has started for the last 2 weeks. She is 6 weeks old. She usually drinks 8-10 minutes well and then has a choking issue. Might this be forceful let down?

      Is she having any other symptoms? Lots of gas and painful tummy issues? Green goopy stool? How many wet diapers is she having every 24 hours? Do you have any pain when nursing? Does your nipple look flat and smooshed after feeding session or does it look rounded like it did when it went into her mouth?
      Are you doing any extra pumping or just feeding from the breast? If you do extra pumping how many ounces do you get at a time and about how many ounces are you pumping every 24 hours?
      The choking would normally start around the first week. And I would think that it would start before 8 to 10 minutes into a nursing session.
      She could possibly be so full at that point that she starts choking but usually the choking is caused by how fast the milk comes out. We have several letdowns in the feeding, and they are usually not as strong as the first one. 🤔

    So my issue is that my baby does sometimes choke and pull off, sometimes she just stays on breast for 7-15 minutes then I sit her up, and boom she projectile spit ups and it’s out of her nose . She is 2 weeks old 😭 I feel so bad because she looses all the food in her tummy and then she wants to immediately eat after she spits up. So we go through So many outfits and I just feel so bad for her.

      At this age, it can be common for baby to spit up a lot.
      7 to 15 minutes can be plenty if baby is transferring well.

    Oh my goodness! baby has all the symptoms except for crying at the breast. She only does that occasionally, but she chokes a lot and gasps for air and I can hear the flow. She has so much /gas. I’ve been told that she’s sensitive to dairy and I’ve been trying to figure out what else she’s allergic to because she spits up so much and she nurses constantly every evening like she’s hungry and not getting enough milk. She had a high weight gain 1.67 oz/day for over a week to lower than 1 ounce a day gain but the lactation said averaging that out she is OK. my question is if she’s getting way too much foremilk how do you get them more hind milk? i’ve done only nursing on one side that’s what we’ve been doing most of the time, especially the first four weeks but still too much. She is eight weeks old today.

      1 ounce a week is excellent! Not low at all.
      Weight gain should be:
      0-4 months 5.5 - 8.5 ounces per week
      4-6 months 3.25 - 4.5 ounces per week
      6-12 months 1.75 - 2.75 ounces per week

      You will have this condition with every baby but some babies have more problems than others.
      Baby can have sensitives through mom’s milk but it’s not common. Doctors misdiagnose it often because it has the same symptoms. They often check baby’s stool for traces of blood. That could be allergies but it’s more often because of a forceful letdown/over supply. An allergist can do blood work to confirm allergies.
      This condition can be harder than a low supply. It always needs to be kept in check. 🥰

    My symptoms are baby gagging, unlatching and crying at breast. My milk sprays & I am currently dealing with engorgment! So painful & I’m a month postpartum.

      Those are definite signs. It’s important to get that engorgement under control so that you don’t end up with a clogged duct. Engorgement is common for moms with a FL/OS (forceful letdown/oversupply).
      Are you pumping? It’s important to not over pump. That will make it worse. Pump just enough to stay comfortable and as your body adjusts and gets under control, it will help you and baby.

      @@marycahillclc I watched them, so in my case when I go back to work pumping will stimulate my supply.. how do I deal with this?
      Before I found out I had over supply I would get 10oz in 5 minutes when pumping. When engorged I now use the haakaa and get an oz out before feeding baby as it’s hard to latch him when so engorged.

      @@lyzethmartinez3955 pumping at work will actually help because you can pump less often than other moms. At first you’ll have to pump more often but slowly wean down.
      This is something that you’ll always have to work with and keep trying to reduce. Things will happen that will put it back up and you’ll have to work to reduce it. If you ever get a clogged duct, you’ll need to pump often to move it out but then you’ll have to slowly work to reduce again.
      When you are pumping before baby nurses, is that because your breast is too firm for baby to latch on? If that’s so, you can apply heat for just a minute before baby latches and that will soften your breast. If you’re needing to pump because the milk is too forceful. It would be better to latch baby, then take baby off when you start to have the let down and just let your milk spray into a towel for a few seconds. This will remove less milk and with less stimulation.

      Baby will usually be gassy and uncomfortable. Baby might have green or foamy poop. Baby’s weight gain is slow.
      You might not see all of these signs.

      @@liyubi it’s a gradual change from foremilk to hindmilk. There’s usually not even a need to think about it unless you have a Forceful let down/over supply. To be sure the baby gets to the hind milk, let baby be on one breast as long as absolutely possible before switching to the second side. If you do have a Forceful let down/over supply you’ll probably need to block feed. So you’ll only be feeding on one breast for a blocked amount of time. This will help to ensure the baby is getting down to the hind milk. Also, don’t do a lot of extra pumping or baby will have trouble getting to that fattier hindmilk. ❤️

      How are you doing mommy?
      You are more likely to get clogged ducts when you have a forceful letdown. You have to reduce supply very slowly. Apply cold compresses often. Also, taking Sunflower Lecithin supplements help you to not get clogged ducts so easily.

      @@zaincheema8429 you can put ice in a bag. Put a cloth between the ice and your skin. Apply it whenever you feel engorged and swollen.

    My baby has a tongue tie and a high pallet and has all the baby symptoms, but I don't have any of the mom symptoms.

      How old is baby? Do you do any extra pumping, or only feed from the breast?
      Did your doctor say the baby has a severe tongue tie and high palate or is it minor? Many babies have a bit of a tongue tie but nothing that needs to be released.

      @marycahillclc she's just over 6 weeks old. I pump after she eats in the morning and after she eats before bed just to try to build a stash for returning to work. I dont get a lot during my pumping sessions, but I do leak a lot during the day.

      @@moldyvoldy1231 how much do you get at those pumping sessions? How many wet diapers is your baby having every 24 hours?
      Do you plan on breastfeeding once you go back to work?
      A freezer stash should be small and for just an emergency use only. Fresh milk is best when possible. Milk that is frozen starts to lose some nutrients though it is still better than formula. Frozen milk does lose much of its immunological properties.
      If you want to continue breastfeeding full-time, you must be removing milk at about the same time that baby eats. At work you will need to pump at around the same time as feedings. This way baby also gets the freshest milk because baby will get that milk the very next day and your milk supply will be great if you want to nurse on your days off.
      if you don’t plan to continue fully Breastfeeding when you go back to work, then building a freezer stash now will help maybe to get breastmilk a little longer. Some moms choose to partially breastfeed. This is where you might feed from the breast just in the evenings when you’re home with baby.
      If you can wean down off of these pumping sessions, you can see if babies symptoms get resolved. It may take a few weeks to see the improvement though. You may need to go to block feeding. This is where you would feed only on one breast for several hours, and then only from the second breast for several hours. You’ll need to get to that point slowly though so that you don’t become engorged and end up with a clogged duct.

    I may have this condition, my baby always starts off gulping hard and fast (you can hear the clicking) but she also suckles fast too. Could a forceful letdown be the cause from pumping every 3-6hrs? Would a 3 week newborn even get to hindmilk if nursing for only 10-20min (and you hear the gulps)? Is it a good rule of thumb to stick to pumping whenever she gets the bottle or pumping after she feeds first? Thanks!

      Are you pumping for her to get a bottle or are you pumping to store?
      Baby can get hind milk in 10 to 20 minutes if you’re not pumping extra milk to store.
      It’s best to not pump in the 1st few weeks unless you’re pumping to give it in a bottle right away or if you’re uncomfortable from engorgement.
      Is she gassy, fussy, have green poopy diapers or low weight gain?

      Yes, she’s gassy, fussy, sometimes green poop, she gained her weight back from having lost after birth. I started pumping since I brought her home because she did not latch good and I did not want to give her formula (I had to for 1 day until milk came in). The lactation consultant at the hospital said her mouth felt tight and to do mouth stretches. Her pediatrician did not see any issue and said with practice she’ll get better. I used a nipple shield but the 4 holes was drowning her in milk so I stopped and just bit the bullet to keep practicing breastfeeding, even with a bad latch. My plan was to bottle feed at night so hubby can help me and then breastfeed during the day. I’m noticing when I breastfeed she’ll suckle hard for 5-10min and then fall asleep. I try to keep her awake and she’ll sometimes go for 15-20min but then in an hour and half she wants to eat again. I’ll end up doing this for half the day and sometimes get an hour or so break but other times I give in and just give her a bottle. Could it be she’s only getting foremilk?

      I don’t know where to take it from here because she’s only 3 weeks old. Hubby is suggesting for now to just pump and bottle feed her but I’m afraid once she gets used to that I cannot nurse her anymore.

      @@dianaf6055 She is probably getting hindmilk. Maybe not a lot but I wouldn’t let it worry you too much. There’s not a huge difference in fat from foremilk to hindmilk. It’s a gradual change through the feeding, but it can affect baby in the long run but your supply should start to regulate soon. I wouldn’t try to force her to take more than she seems to want.

      Normally a newborn baby needs to eat off of both breast each feeding for the first 4 to 6 weeks when possible but if you have a forceful let down/over supply, it’s best to start block feeding at around three weeks . It looks like that’s what you are doing is block feeding. (if you meant to or not👍)
      Wanting to eat every hour and a half is normal. Some newborns want to eat every hour. In the evening, most babies will eat quite often. This is a cluster feeding time. When baby cluster feeds, they don’t want or need more milk in ounces but they’re getting slightly more fat. The extra fat helps them to sleep a little longer stretch at night. She probably wouldn’t have taken in as much as you gave her in a bottle. You can’t over feed a baby, but when the baby is given a really large amount. It can cause the problems like you are seeing. 3 ounces is a large amount of breastmilk for a baby to take in at this age. Even up through six months, breast-fed babies won’t take in 3 ounces very often. Formula fed babies taking a larger amount because they need more milk to get the same nutrients.
      I’m sure she’s having lots of wet diapers. Six or more wet diapers. Every 24 hours let you know that she’s getting enough. By what she’s eating she’s probably having 8 to 10. Right?
      I think you are doing much better than you realize. It’s OK to give her a bottle if that makes you more comfortable and if that works out better for you.
      When you give her the bottle, it would probably be good if you “pace” fed her. This is where you would offer the bottle for a few minutes, then take it away for a few seconds. If she seems to want it again, give it back to her but if she seems content or falls asleep, then you know she’s had all that she wants. When baby gets a bottle it comes out quickly and they often end up taking in more than they want . ❤️

    I have all these symptoms and my breast milk sprays out, I make 8 oz each breast and my baby spits up so much and she shows symptoms of hunger right after 😭 her belly swells up of gas

      It will take time to get it under control but you can do it. It will be something that you’ll always need to work on. Having a forceful letdown/oversupply can be hard but you can do this mommy! 💕

      @@marycahillclc yes I am trying block feeding because my baby is two months old and all this time I thought she had gerd but I ended up why she’s spitting up so much if she’s mostly breast fed (was supplementing with formula Added rice starch but there was no difference) is there anything that I could be eating that maybe increasing my breast milk? Oh and thank you so much for this video It’s 11:44 and my baby was just so fussy i couldn’t help buy Google what I can do to help her with spitting up

      @@Chocheeze Block feeding will help a lot. And if you can cut down on pumping will help. You more you pump, the more you make.
      Things you eat don’t add to your supply but you can do things to reduce supply. It sounds like your supply is huge. You can apply cold cabbage leaves several times a day. This reduces supply and help your body adjust to making less milk.
