      @@sasuxsakuxfan Nope, PhD is unrelated to both. Psychologists have a degree in Psychology while Psychiatrists are doctors with a specialization in Psychiatry. Both of them can also become Psychotherapists if they do an additional school.

      I'm currently seeing both..
      Bipolar.
      Psychiatrists gives the meds. They're the only that can prescribe stuff like Lithium, Quetiapine, etc
      Psychologists focuses on behavioural theraphy..
      My session is 1v1.
      Stuff like breathing techniques.
      End of session she reports on my behaviour to my psychiatrists via hospital network so yeah ..

    A good chunk of psych consults I saw as a student:
    -Hey this patient is sad because something devastating happened in their lives. Can you fix it?
    -Hey this patient isn't compliant with what we want them to do. Maybe they have a psych problem. Can you fix it?

      Ah, I went into the hospital after having my ovaries out, being seriously sick for two weeks immediately after surgery, having my kid hospitalized and released on the wrong meds, bottle feeding kittens every 3 hours, my dog almost dying from eating xylitol gum, etc, etc. told them I was situationally depressed. They got a court order to shoot me full of Zyprexa because my blood sugar crashed from then giving me Haldol (because I wanted to speak to the patient advocate before being committed and they said it was proof I couldn't take care of myself). Apparently this was psychosis (only it wasn't).
      I wish anyone had recognized my life was swirling down the toilet bowl, my estrogen just tanked and I couldn't take estrogen because it was causing me physical and psychological problems, and maybe I just needed some sleep and a hug.
      I was absolutely in the opposite side of the equation with that one.

      I saw one combined as a resident in oncology. New dx ovarian Ca and the patient didn't want active management due to religious beliefs. Attending forced me to consult psych for ? Depression ? capacity. Easily the stupidest consult I've made 🙃

      @@Twinkie989 sorry you went through that!! I know a few people who have been through pysch wards and to be honest, their experience is similar. A lot of people end up in pysch wards for legitimate issues but get further traumatized by being abused by medical professionals who just want to shut them up.

    This really hurts a lot. While I’m laughing at this skit, I know a lot of people who have been hurt by this thinking. This is a serious issue in healthcare. For some reason, doctors off the clock understand that people sometimes act “crazy” or sad. Many doctors have serious mental issues themselves!
    But on the clock, it’s common to legitimately traumatize patients by stripping them of agency and treating normal emotional responses as being insane. I know of a few people who have tried to seek help, only to be treated as incompetent animals just because they are going through a tough time. This can give a patient medical related PTSD and make them not want to seek medical help in the future.
    On the whole, this is a super important skit and I hope that med students in the comments will keep this is mind as they enter the workforce :)

      Thank you for this very real, very impactful truth! I can personally attest to how accurate and true everything that you've said here really is! Thank you for explaining this and bringing to light not just that these are actually happening in health care settings, but equally as important, HOW deeply these things affect the patients experiencing them, both short and long term!

      Medical ptsd..... what was the trauma? That's not a diagnosis. I don't know how bring assessed by a provider is trauma.

      @@Thinkoutsidethebox15 Lot's of things can be medical trauma. Medical ptsd is just any ptsd receieved via medical treatment. Being neglected in a hospital, being stripped naked, roughly grabbed, yelled at, needles, etc. You could probably look up "what is medical pstd" or "examples of medical ptsd" for a better idea.

      @@salemcrow5078 ptsd is just ptsd you can also have a phobia of things as well. having your clothes ripped of is violently diffrent from needles. There is also acute stress disorder. I mean medically indused ptsd theough medical trauma is trauma that frsters to ptsd, but to say a needle is trauma is a poor choice.

      @@Thinkoutsidethebox15 I mean, I feel like it could be. You might specifically ask to be warned of the prick and then be denied that by the doctor ignoring you. Or maybe the doctor is just stressed out and in such a rush they decided to neglect you in these ways, grabbing you and just kinda taking over. also, some people have a horrible phobia of needles. I have a friend who has terrible panic attacks every time he gets his blood drawn, if he was treated that way, that would definitely result in medical related trauma.

    my mother is a social worker and in her opinion, no one should be assessed for capacity in the hospital. she calls it “hospital psychosis” - stick anyone in a boring, strange place where they can’t get more than 4 hours of sleep at a time and after a few days, they won’t know which way is up

    The amount of weird stares people give me when I say I've got an appointment with the psychiatrist 🙄
    They think psychology is for "normal" mental health issues and psychiatry is for "crazy" people. I always simplify it to "the psychiatrist is the drug dealer of the psychology", they seem to get it

      I was always confused whether it was psychologist or psychiatrist for mental health problems until someone kindly explained to me a while ago that psychiatrists can pescribe medication but psychologists can't. But I absolutely love 'psychiatry is the drug dealer of psychology' and will be using that to remember the difference from now on.

      @@safala Yeah. The medication that I would need can be prescribed by an internal medicine doctor, even though a psychologist can’t (and shouldn’t, because it’s such a different process to become a doctor of psychology), but I want to see a psychologist to try and find someone who can take on someone for a long term, almost more “traditional” therapy, since CBT and DBT techniques and attempts at exposure to things that cause me anxiety aren’t working very well or are actual potential dangers.

    psychiatry is one of the biggest "grey areas" in medicine, it isn't as clear-cut as other somatic specialties, and therefore vastly misunderstood. i'm glad two of my best friends are currently specializing in that field (i work at the ER) bc their perspective helps me realize a lot of things about what psychiatrists do, when to ask for a consult, what to expect in terms of treatment etc

    as someone who was admitted into the psych ward and met a non-psych doctor i can vouch for this. he was like "so.... why do you want to kill yourself? don't you know that ... you'd, like, die???"
    and i was like "yeah no, i think i got that. that's kinda the point."
    laughing abt it now but it was bad lol

      Yeah, I confused a trainee once with my mental health problems. I've been diagnosed with depression for years and went for a check-up, answered the standard "do you ever feel depressed" with "yes", and the new nurse just got this deer-in-headlights look and started acting very worried about me.
      Yes, I'm depressed. Yes, it's on my chart. No, it's no longer a big deal. Please just move on with my appointment

    have you noticed that he agreed to do all these consults? it makes this video sooo real :D psychiatrists rarely refuse consults even tho 50% of em aren't necessary (and it feels like 50% isn't high enough number :D)

    I would love one for inpatient Palliative Care, please! We get asked to perform miracles in terms of goals of care, but then we get a lot of time to meet with patients and family/friends/etc. I love getting "hey! Mr. Jones isn't a candidate for surgery and likely has days to weeks left...can you clarify goals of care (whispers: and tell him?)"

      @@Puglover130 yeah I have a certificate in it. Also my MSW. Why? People with decades in palliative care confirmed this, that often coworkers would want a patient to make certain choices and that they’d call palliative like palliative is a brute squad that makes people fall in line. Like, this patient obviously needs X highly invasive treatment! Convince them to do it! When the reality is it’s up to the patient and probably no one has even bothered to ask their reasoning.

    Just last week I noticed the attending refers to most of the psych patients as "crazy people" and described a person with memory loss / cognitive defects attributed to alcoholism as having "drank themselves retarded." Hmmm. Yep goes to show ya just cuz someone has an M.D. doesn't mean they are enlightened to the use of compassionate speech.

    I feel like this could be my hospital. Putting a 13 yo on antibiotics for 4 weeks bc of a tick bite and then when asked for caffeine bc of headaches they send a psychiatrist bc "the patient seems a bit sad"...no duh who's happy to be in a hospital + being in pain

    I’m currently doing a rotation in a general hospital for my residency. This is so accurate. Medicine doctors know very little of psychiatry and ask consults for the dumbest things 🥲

      makes me appreciate my doctor even more. Dude's a secret cardiologist (actually found out that he did a bunch of courses, no weekend stuff, he went back to uni) and he's also really good with psychology.

      I mean psych consults medicine for a lot of what we consider dumb things, it's hard when stuff that might be outside your wheelhouse comes up. Better to ask for help in some cases than cowboy it.

      Maybe you should stop doing that . If it’s a dumb thing and you admit that to yourself you have a duty to prevent such acts against an innocent don’t you? Why should any person be subjected to an “evaluation” or “examination” of their way of thinking? When there is no reason for such a thing? I can see how people’s reactions to that could be misinterpreted for the benefit of those who are coercing them. That thought ever crossed those brains of you “smart” college kids?

    Oh really? It's not like I've been disabled since childhood and suffered mentally and physically for over half of my life. My symptoms are getting worse too, but I wonder why I could be depressed?
    Like...do you need to ask something with a very obvious answer lmao. My diagnosed depression could also be a hint at that too but I'm not an MD so what do I know. 🤦🏻‍♀️
    (No hate on most doctors but some have really just irked me, especially the ones who mistreated me and called me liars for years)

    This. I worked psych CL and each one of those scenarios happened🤦‍♀️ Another one was the patient that woke up short of breath and anxious at night, and only anxious at night. I tactfully mentioned likely PND secondary to + 10L fluid balance and I would be happy to review the patient once that was rectified. Never heard about that patient again.

    I don’t know anything about being a doctor, nor do I really want to know, but I appreciate how much you dog on your own, and yourself specifically. Makes me feel like you’re a good doctor who takes criticism, learns from others, and accepts when he’s wrong. I have a job where I need to be more like you (or at least my opinion of you) so I’ll try.

    Remember, whether you’re a medical professional or not, when you’re interacting with an individual who is going through a tough time, instead of asking what’s wrong with you? Try, what happened to you? Compassion is critical! ❤️

    Hey Doc, so i work at the same hospital where my homeboy works, he's an attending psychiatrist. This video reminds me of how whenever i spent time at his office, the new residents/fellows always calling him asking for advice in dealing with a patient because they usually dont know what to do. Lol. This video is on point!

    Jesus Christ, I had flashbacks to CL. You missed a couple, though:
    - "alcohol", and you have to explain that no, alcohol withdrawal is a medical thing and psychiatry does not need to be involved. Also, rehab programs should go through social work, and psychiatry does not need to be involved.
    - "suicidal ideation" in a patient with uncontrolled pain because they said "I'm in so much pain, I'm going to kill myself"
    - "competency" but this is kind of a nitpick. For those who don't know, competency is a legal term, not a medical term. It requires a judge to make a ruling.

      In terminal patients a lot of people don't realise that stuff like "I don't want to live like this anymore" or "I want this to end" does not literally mean they want to die and shouldn't be interpreted right away as such...

      @@dreamsinmonochrome I legit have a macro I use for delirium and the "I'm in so much pain I want to die" consults. No matter how many times I write the same things I have multiple consults the next day for the same things 🤦

    Great contentMr. Jones is probably enjoying his stay at hospital and it is like a hotel or he he is loving the attention despite having so many ailments.Dr. Schmit has his interest at heart for certain. Whatever Mr. Jones needs is maybe a mental health check or a family visit . We adore hearing of his progress too. I think hospital is his permanent home.     So this is why, as a young female with a history of mental illness, medical doctors jump straight to psychiatry rather than actually helping me for the problem I’m there for… I legit had a CSF leak, and they were like, “well maybe the 10/10 positional headache and debilitating nausea are all in your head. Have you tried CBT?” 🤦‍♀️🤦‍♀️🤦‍♀️

    Omg! I’m not a psychiatrist, but a therapist who used to work in primary care clinic and was often the go between for doctors and our psychiatrist. This is so true!!! Doctors seem to be so scared of anything psych related. It’s ridiculous really.

    I’m on my consultation and liaison block now as a psychiatry resident, and this is accurate. I do feel that other professionals don’t respect psych consults the way they would respect cardio and gi. When you consult Cardio and gi they are given proper presentations of te patients history, why they were admitted what work up has been done and what clinical question the primary team wants answered. I haven’t felt that respect with psychiatry and it’s pretty sad.

    At least they have in house psychiatrist. Our small hospital has to refer out of county for acute issues. For medical patients having issues the doctors always call the police to evaluate for 5150 holds even though they can do it themselves.

    Saw a psychiatrist once when my life had fallen apart. He said something to the effect of "good news, I don't think you're mentally ill, bad news, I don't have a pill to make your life not shit".

    I went to a hospital and after a 24 or so hour stay that I was heavily medicated or unconscious for, the doctor told me I was bipolar. I asked how she knew because there wasn't any formal test that I noticed and she said she just noticed because of how moody I was. Again, I was medicated, unconscious, and who likes being in a hospital? Anyways I went to a therapist and I never was bipolar, just had untreated ADHD

    This is on point.i work in psychiatry and you have to wonder if my colleagues and doctors from other areas did it as part of their course. They look down on psychiatry and who cares for the patients.newsflash!!! The head controls everything.mental illness should be respected.

    Oh man at the hospital I studied at they would NEVER call psych consults even when it absolutely necessary (except for the dissociative seizures or paralysis). The way I had to convince my seniors to please give a referral.. even in the medical community, psychiatry is such a taboo.. 😔

    I remember going through this runaround.... my PCP said I had to get a ESA approved from my Psychiatrist, and my Psychiatrist told me to either ask my PCP or just get one off line even though my landlord said it had to come from either my PCP or my Psychiatrist. Now I'm pregnant and my OB asked if I had an ESA and I said I used to have one that wasn't documented but my PCP and my Psychiatrist took to long going back and forth and now my ESA is dead. Like no joke, when I need the emotional support I cuddle up to his ashes box now.

    Tbh I think that cancer patients should receive psychological and/or psychiatric support by default.
    Where I live any cancer patient can ask for it and receive it for free.
    I've been receiving it for years now. It's pretty important, especially if you don't have many chances to heal.

    Explaining to people that depression =/= sad and antidepressants=/= "happy pills" is always such a fun time
    I like to say my meds help me feel things other than sadness and anxiety, and help me feel things in general. I still get sad, but it's a tangible feeling of sadness that I can process. Not some weird, looming feeling of emptiness and dread.

    this is what happened to me for years in the er, every time my pain got too unbearable to sleep, causing me to go untreated for Crohn's disease the whole time. they were always just way more interested in selling me drugs I had no business being given, while completely ignoring my abdominal problems and blood coming out of my rear end, because those are such normal symptoms for a "psych" patient.. 🤢
    our system is total garbage.
