Your example of a comparison is misleading. The comparison intervention should be different than the intervention itself. If you are asking, In young, healthy females, is the regular use of a rescue inhaler cause dependency? You can not answer this question without comparing the results to a like population who do not use a rescue inhaler. This is known as a PIO question and is appropriate because not every clinical question requires a comparison. If you want to use a comparison, it should be a second intervention completely different from the first. For example, in a young, healthy adult with asthma, how does regular use of a rescue inhaler compare to the regular use of a maintenance asthma medication at controlling exercise induced bronchiospasms?
It's helpful, but you have to consider that the people who didn't use rescue inhalers might not have had as serious of a case. Medicine is way more subjective than we care to admit.
