Angioplasty is now the most popular treatment for coronary artery disease. Few decades earlier the results of Bypass surgery was superior to Angioplasty in complex cases. But now with the use of superior techniques and hardwires, the results of Angioplasty in complex lesions is much better. Imaging techniques like OCT & IVUS has played a major role in this improvement. The imaging techniques help to take high resolution images inside the coronaries, look for calcium, clots, other complications. The images also help to select appropriate stent size (length & diameter) and ensure proper expansion and apposition of stents.

50% block doesn't need stenting
Sir amar babar 28th sept a permanent pacemaker bose. Sei din e CAG kora hoi and blockage paua jai tar details niche dilam. Doc suggests korlen CABG korte. Akhane ki angioplasty habe? Amar babar sleep apnea ache and sugar er glyciphase 1gm khachen. Weight 98 kg and height 5’4” and age 56years.

Clinical Presentation / History: Chest pain.
ECG: CHB.
ECHO: 65 %
AO: 130/70 (90) mm of Fg.
Approach: Right radial.
Catheters: SF TIG.
Contrast Media: Omnipaque 350 mg.
Flurotime: 02:00 minutes.
Left Main: Normal.
LAD / Diagonal: Type III vessel, proximal segment shows 85% - 90% stenosis, total occlusion from mid segment, distal vessel filling via collateral.
CX / OM: Non dominant, proximal segment shows minor plaque. Distal segment shows 85% - 90% stenosis.
*** Coronary cameral fistula from left system.
RCA: Dominant, proximal segment shows 80% - 85% stenosis, total occlusion from mid segment, distal vessel filling via collateral.
Final Diagnosis: Triple vessel coronary artery disease.
Recommendation: PPI followed by CABG.
