  • In this episode Dr. Spina sits down with Dr. John Saratsiotis Physiotherapist, Chiropractor, Biochemist, and lead FRS instructor to discuss a controversial topic in the world of manual therapy and injury management, namely the topic of soft tissue Fibrosis. What is it? How does it form? How does it affect force transmission and the many misconceptions surrounding both its existence following injury as well as soft tissue management. We also cover related concepts including tissue directionality in the fascial system, the evolution of tissue healing in humans, what pain is and the role manual therapy plays in its management, and selection of appropriate therapeutic outcome measures. Complete with scientific references, this talk is a deep dive into the cellular understanding of fibrosis and how it effects human performance making it an important listen for Strength & Conditioning specialists and Manual Therapists alike.

Every time Dr. Spina interviews someone, I learn more about why what I do works or doesn't.

Loved this podcast-possibly my favourite one so far - could listen to these two particularly all day/ Can’t wait to attend FR and FRA courses.

This is the first time I have heard this podcast. I would have loved to hear more from Dr John, more about how to best integrate this information into a treatment plan with some examples. The venting was a bit much for my taste, if the naysayers got under your skin then present your argument not your gripes. Your subject matter is right down my alley, I will try a few more… time stamps would help.

You can tell these internet clowns really got under Spina’s skin, no pun intended. Great discussion, learned a lot.

So a tissue based outcome measure - could that be ultrasound imaging ?

I’m currently assuming that the “breakdown portion” of scar tissue post an effective manual therapy session is due to stimulation of fibroblasts in that location releasing collagenases, breaking down collagen fibers that are moving at a “suboptimal” direction
And then those same fibroblasts lay down new collagen fibers that are moving in the optimal direction
And this happens with exercise and stretching as well
First off, is that a good way of thinking about the effects of good manual therapy? - and what can help me broaden my scope about this?
My 3rd Question, where should i look for research that looks into indications for manual therapy where the patient is perhaps so severely damaged that exercise and stretching are a bit contraindicated or should be heavily supervised ?

Great question. What do you mean by "Good Force" ?

@@jamessmith4681 maybe I want to much detail, but "good" has as in how much force/duration and frequency during each day?

Since articular cartilage just takes longer to heal if at all perhaps it just take more consistency
