  • After I had my TURP surgery, I thought I had recovered, but after 6 months, I noticed I was having more problems urinating. Two surgeries later and now self-catheterization. A new chapter in my TURP journey.

Thanks for the words of reassurance. I'm doing ok with the TURP, but the darn scar tissue is giving me the problems. Thanks for God's blessings and will certainly ask God to bless you and yours.

I’m an RN And will be sharing this with people I work with, mainly because of the useful insights on what you weren’t told to expect or do. Patient/family education is a huge part of the nursing process and, I’m sorry to say, one that is often back loaded to the final 15 minutes before discharge. You gave me plenty to think about, not just on the subject of your particular procedure, but on how I educate my patients in general. Thank you for your efforts!

Tom, Thank you for the kind words and thanks for what you do to help anyone in need of medical attention. I know you have a difficult job, and like my police career, we usually see the bad and sad side of life. I'm glad I am able to pass along some information to anyone who has a TURP or BNC procedure. Blessings to you and your family.

Appreciate your comment that’s the most uncomfortable thing when dealing with the medical industry they don’t tell you all your options and what could happen they just go ahead and do what they need to do our thing and don’t give you all the information so you can make a informed decision I am so frustrated but I just know I have to eat exercise and do better which t

Thanks again Doug. I’m writing this as I lay in my hospital bed in Calgary Alberta Canada recovering from my TURP procedure earlier today. The heavy bleeding and clots have all but stopped. The discharge is now more pink than red. Fingers crossed that I have a good night and get released tomorrow. I’m a 55 year old male, younger than the average age of 69.

Thank goodness for people like you who care enough about others to do these videos when they don’t have to, especially since my doctor gave only a very cursory and top-sided discussion about this issue. The two earlier videos really helped me get comfortable and confident with the TURP procedure. Doug, thanks for posting. You are not alone. I had a Turp, a week later they took the catheter out, that evening I could not "go", (not a drop) I had to rush to a hospital to get re-catharized, (high retention is a very painful experience). I have been wearing a catheter (ugh) for two weeks now. In five days I will go back and hopefully get the catheter removed for good. Good luck, here's hoping for your full normal recovery. I will share this video to other media which should help promote more views.

Hi Doug, I had Turp a week ago today and prior to surgery watched your earlier video which was very helpful - thankyou. I had no doubts at all after extensive reading and talking to my Urologist that TURP was the right decision for me, life with BPH was miserable . Fortunately I was able to stay in hospital until the catheter was removed (on day 3) as I could not have coped as you did with going home with it, especially living on my own . All the very best Doug with your recovery and thankyou for sharing your journey with us.

LOL on your gleeful wife comment. Married for 30 years I can relate!
Your videos are sooo well done and very generous of you. I made a comment with several questions just now on your initial video following your TURP. Among them was one re whether you researched Auablation and if so, what tipped the scale toward a traditional TURP (if you have time to reply please see my questions in that other video).
That video was very encouraging. But then your relaying of your constricture issue in the subsequent comments as well as the negative outcomes of numerous other commenters really got me down as I prepare for my own TURP. This video though is encouraging. I have to say, your positive attitude and generosity in sharing is an inspiration. Thank you.
Not to be discouraging, but perhaps to just be prepared for the possibility: in addition to BPH, I suffer from a urethral constircutre. Before doing a TURP one doc wanted to do a minor procedure to open the constircutre to rule that out as my source of frequency. I was set to have that quick procedure done where they make incisions in the consticture (scar tissue) and then count on the catheter for a week to serve as a scaffolding to keep it open while healing. My research though came up with studies that showed a nearly 100% relapse. I consulted several more people including an expert in urethral constricton in So Cal before a decision was made that it was not likely to solve my issues due to the BPH.
On the positive side for you though, yours is not urethral but at the bladder neck. And I did not come across the use of steroids to reduce inflammation. Or the use of ongoing daily catheterization. And the fact that I think you said you had the procedure under general indicates they probably removed tissue rather than just making incisions to allow expansion from the catheter. So hoping you have success this time! After all you've been through you deserve a good outcome!!

My research indicates that Aquablation might be the best procedure. It seems to be the newest. Unfortunately no one doing it here in Australia, yet. Nearest practitioner to here is in HongKong. Doctor there replied to my enquiry. very promptly, but the cost was $20k USD ($30k AUD), ~3x local HoLEP laser.

4 weeks ago I had the Urolift procedure. It was very painful. Lidocaine was not administered properly before procedure. I had to have a catheter for 2 days after the procedure. Several hours after the catheter was removed my bladder filled up. Trip to ER that night. I can relate to what you are going through. My provider is looking for a new urologist to take over my care. I can't bring myself to return the Dr. that did the procedure because of the level of pain I endured and his not talking to me the day of the procedure. Not one word before ,during or after the procedure. I have never had a Dr. not talk to me the day of procedure. Hope you get the improvement you need, good luck.

Digger, before you had relapses, did you observe any small clots or specs of blood in your urine? I noticed some pink spots in my overnight pads. I had a major bleed out and bladder full of clots 25 days after initial surgery, and was readmitted for two more surgeries. Thx.

    • @clayconn1242

I had your exact experience with Urolift. The first 24 hours post-operative were some of the most intense pain I have ever had in my life. The doctor who performed the surgery didn't even stick around to see me in post-op. He ran off to vacation and I was miserable (and pissed). Felt like I had to pee every 2 minutes and the pain of peeing nothing but blood for almost 2 days was the worst pain ever. I finally achieved some relief from the procedure but about 3 months later, my syptoms completely returned. Of course, the MD told me he had 'never' had a failed Urolift procedure before and wanted to go back in to make sure they were situated properly. I declined. I could never go through that again. I'm back on prescription meds and have just resigned to accepting my condition. And I'm only 57 so I'm not real happy.

Thanks Doug, I'm self educating before I see my doctor. Your video is very commendable. I not sure how I would handle self cath if that comes up. Not sure of all the options for this problem I have with urination frequency. I wonder if you had other operation options. I see on UA-cam a new procedure using water to open things up. Seems like that is rather new procedure at this time out of Jacksonville, Fla. I'm very glad you have taken the time to post you very informative video. I will be viewing your other videos as you suggested. I see the urologist next month...let you all know. I'm a Marine Vietnam vet with agent orange exposure. Camp Lejeune had a problem with chemicals in the water system back in the '60s. I wonder now what that may have done to us? Maybe a double whammy.

A year out from my turp procedure I have exactly the same problem, repeated visits to my current urologist were unproductive, the last time I questioned him about bladder neck constricture and he said he didn't see any when he scoped me 6 months ago. If I don't take flowmax I shut down almost completely. I have to add that the urologist that did my surgery turned his back on me when I told him I had complications, he said he didn't remember my surgery and didn't know about any complications then hustled me out the door. The guy mentioned above is my NEW urologist. I had the old style TURP and not the laser, big mistake because all of my friends who had the laser came out great.

    Thank you for the explanations.
So far I passed one clot and no need to flush even once. I am hoping my luck holds out.
I did many Kegel exercises overnight which may have helped break up any big clots. Also, some surgeons do not seal up the bleeders well enough and it creates clot fields.
Also.... Guys.... 95% of the time this is an uneventful recovery and is a one and done thing. Many people will post their horror stories to vent. Uneventful surgeries and recoveries rarely give comments, so don't be scared.

Thanks Doug. Informative video. I had a TURP in 2016 so thought I would chime in for the other commenters who have recently had the procedure. Thankfully, I have had no issues with my TURP whatsoever. My diagnosis came in the ER of my local hospital. Although I was seeing my urologists regularly sudden swelling of my prostate shut urine flow down causing extreme ankle swelling. I had TURP done and after the usual recovery time and issues all has been well. I was 66 and extremely active and am so today at 72.5. Regular exercise 4 -5 days a week. So if you have had, or scheduled to have a TURP in the future, hopefully your experience will more closely follow mine than Doug's. In any event, good luck to Doug and all you guys with PG issues. I had PVP laser surgery in February 2022 because Flowmax stopped working on my BPH after 24 years of taking it. For the first two months after, I was peeing like a firehose and was beyond thrilled! Then my stream slowed to a garden hose, and now its back to a trickle. I have to rush to the bathroom every 30 min or so during the day, and am getting up 2-3 times a night which is worse than it was before the surgery. I have been avoiding talking to my urologist because I dont want more surgery, but at this point having a catether is more appealing than my "trickle every 30 min" schedule. But like I said prior, your original post op video eased my mind and apparently many others.!
I had to straight cath for a bit myself but I created a false passage and messed things up

Imagine when one covers the tip of a water hose with your thumb and the water comes out with increased pressure. Told my doctor and he went ahead and tried to insert the camera through my penis to check on that but because of the blockage he was not able to do so. Doug after a radical prostatectomy I had a leak at the connection of the bladder to the urethra. It took about 5 months of wearing a Foley catheter for the opening to heal. Things were okay for about 3 months then the symptoms you describe kicked in. Well that was 2 years ago and after multiple attempts to keep things following the bladder neck contracture has not been resolved. At best I get 3 to 4 months relief. My urologists are concerned that further attempts to open things up will permanently compromise the bladder neck tissue. The neck is totally closed. So after my last emergency room visit in May 2022 a Supra pubic catheter was inserted. So while this is not the solution that I hoped for it is manageable alternative if all else fails.

I had all the same problems, but after radiation treatment (seeds), NOT after a TURP, including the need for chronic self-catherterization for a few months to try to avoid the recurrent scar tissue. But in addition, the seeds caused bladder cancer to occur a couple of years later! Fortunately, all this happened within a four year period some 8 years ago, so I no longer need to self-catheterize nor frequent a urologist, though still needing to urinate once every nighttime. I too was diagnosed with an enlarged prostate, probably with a routine check up at 60yo. Don't worry, the urologist said, we can treat it with meds if you begin having peeing issues. I didn't like the sound of it after reading a few things so started on a saw Palmetto supplement. Year on year, urology man kept checking, asking if had problems, I think I even detected a smirk as I said I had started on SP. My peeing flow did begin to slow although I was never one to have to go through the night, but I read that pumpkin seeds were also recommended, which I have with my breakfast cereal. I take now, a Saw Palmetto, and Calabaza oil tablet twice daily, in addition to a host of supplements and vitaminsfor other purposes. Touch wood, all systems ok, flow not exactly perfect, but it's eight years or so since that initial warning. The interesting thing is, the urologist doesn't do an ultrasound scan any more to check the size, he just goes on the blood results which he thinks is ok.

My story is bit more involved because following open heart surgery sixteen months ago it left me with an inability to urinate more than say 5ml every 20 minutes. No one will believe me that prior to my heart surgery, yes, I didn't exactly urinate every time with a strong stream but I was 'getting by'. But finally, they said I needed TURP since the other procedures like Urolift and the Rezum Vapor steam procedure were not offered to me. Currently, I do urinate, but with some difficulty and some minor discomfort, but after wearing a Foley Cath for sixteen months I'll take anything.....I cannot see myself ever doing Self-catherization and each time they put a 16fr or 18fr cath into me I would scream and when they would deflate the cath and take it out I'd scream one loud yelp but mercifully that is brief. I am sincerely thinking of your situation and I currently as I said only urinate now, maybe 100 to 200ml at a time (and the urine is bloody like red wine but I'm hoping that will clear up over time) ~ I hope I do not develop Bladder Neck Contracture! God bless you -

Thanks Doug, 3 months ago I had my prostate removed and at the moment I'm in the same trouble waiting to go back in to get the blader neck constriction removed I have 6 days to go before the operation and at this stage I don't know if I'll make it before it closes up completely. Them prostates were the most badly designed part of a Man. Thanks again for putting up your videos. Cheers from Ireland.

Ted, I'm so sorry to hear you are experiencing Bladder Neck Contracture too. Not much fun, for sure. We have such high hopes after prostate surgery only to get knocked back down with BNC. I had my third procedure in December of 2022, but this time my bladder specialist injected some type of cancer medicine into the scar tissue. I don't know the name of the medicine, but it is designed to stop cell growth. So far, I'm doing really well, so you may want to ask your doctor if he/she has ever heard of that procedure. Wishing you all the best and stay in touch so we all can help each other. God's blessing to you, my friend. Doug

@@etudigger3340 Thank you so much for your kind words Doug. At least I have another night done. It's diffenitly getting worse, I have to sit on the Toilet for about 5 minutes to get out as much as I can. I will certainly mention it to my Urologist but they don't give away a lot of information and we're not supposed to know anything. With God's help we'll all see a better day. I'm delighted to hear that you are on the road to a pain free life and that's what makes life worth living. A pain free life.

Doug, I'm sorry to hear about your complications and wish you the best of luck. Based on my own experience having had the HOLEP procedure I would encourage folks to look at having that vs TURP. While no procedure is perfect and each has its own complications, I do feel the HOLEP is better. It has a less than 2% retreatment. The issue is finding a good surgeon who has done hundreds of these not just a few. The main drawback of HoLEP is its steep learning curve for the doctors so alot of offices do not even offer it and also finding a hospital that has the equipment. I was fortunate to live in a area where this is available. So far it has been a month since my HOLEP procedure and things are going nicely, I guess time will tell.

Mike, thanks for the support. I still support the TURP and any new and improved procedures. I'm still self-cathing every few days and so far I am not having any issues. Like you said, I guess time will tell. Take care, Brother.

Michael, what do you think the advantages are choosing HOLEP vs other turp procedures? I am trying to decide which procedure in the near future.

@@happycamper5900 I would go with the HOLEP procedure over TURP any time. I am not a Dr. but I had the HOLEP procedure and my prostate was over 200 grams. It has been 3 months since my procedure and my life has changed for the better. I have had no issues what so ever, I can empty my bladder fully and go for hours without having to go. If you want to watch some good videos go to youtube and look for Dr. Amy Krambeck she is one of the best in the country and based on what she says has had to fix many TURP procedures that failed. Always fine the best Dr. in your area if you can. If you do have the HOLEP make sure it is a Dr. that has done many of the procedures and is not just learning. Even if you have to go outside the area you live do that if that what it takes.

Thanks Doug for your report. I'm 77 and had a TURP proceedure about three years ago - one of the best things I have done to improve my life. As you say getting up three or four times during the night is a real intrusion into good healthy sleep and the procedure came just in time for me to avoid some embarrasement when being more than 200 yards from bathroom facilites. This could also be quite painful.
I have been left with one side effect - i'm fit and active and after the procedure still have sex, however the semen is no longer ejaculated normally but travels backward into the bladder as a consequence of the 'weakening' of the bladder sphincter which you are refering to. Nevertheless this is a small price to pay for the comfort of sitting with my friends and my 30 year old son and being the one who doesnt have to keep going to the backroom. And the pleasure of a proper nights sleep and more than five years of broken sleep and medication is a real gift.
    Good luck buddy.

Pete, I'm glad you are doing so well. I pray you continue to have no issues. The retrograde ejaculation is no big deal, unless you are planning to have more children at 77. It's possible. You may have to date a 25 year old women...Darn the bad luck. LOL.

I’m sorry to hear about all your problems after your procedure. I also suffer from BPH. I chose not to go through any of those barbaric procedures. I chose medication prescribed by my doctor. The medication is Alfozosin. I’ve been taking it for two years now and it works great In relaxing the prostate muscle so I’m able to urinate normal. The only way I would go through any of those procedures is as a last resort. I feel it is up to me to educate myself so I I’ve researched all of the different procedures and they all seem to cause some sorts of side effects. Good luck to you on your recovery.

I know your pain, I’ve urethroplasty over 12 times. Because dilation doesn’t work, I’ve even had cold knife with mitomycen injections. I’ve even woke up when I couldn’t go at all. Did the self cath, didn’t make a difference, now I get scoped every six months.

i just got a holep procedure done in japan. its pretty similar to the turp. its just uses a different tool to cut with. the doctor showed me a bottle of all the prostate tissue that was taken out and wow it was much more than i had envisioned. There were some big chunks! but my question to you is did your doctor send any of your extracted prostate tissue to pathology for cancer screening? i asked my dr and he said no

My TURP procedure was done on 1/26/23. After three days my catheter bag looked like the color of Merlot. Was still in the hospital, so they did a "trial" catheter removal for six hour and no go. Did a second TURP and found a few places that needed to be cauterized. After two more days in the hospital, two more removal "trials" but no go. Total of nine days in the hospital! Finally went home with a leg bag and then two more removal "trials" over the next two weeks, but still no go. It's now the end of May and I'm self cathetering four or five times a day. I do get the urge to pee on my own, but it's not a complete drain. Had a urodynamics test done and the Dr says I have a restriction, but he wants to "wait and see" while everything heals up. Now have an appointment with a neurologist to see if there's a connection between the urinary issue and my diabetic peripheral neuropathy. Just FYI.....

Rick, I'm so sorry to read you are having continued issues after your TURP. I know you were as hopeful for good results as all men, myself included, but sometimes things don't go as we hope. I'm praying you will eventually have better results in the near future. Blessings to you.
