Hi mate i really need your help plz
Ive got my medical in 2 weeks
Ive been supplied a d4 form but NOT a d2 form
Will the medical practitioner have the d2 form
What was your experience??

Heya John, I just had a look and going back into my memory, the d4 form will be filled out by the medical doctor, so don't loose the form, take that with you to your medical appointment. I could only advise you to call the recruitment on Monday to double check on the forms. I sure it's a D4 form will be completed by the medical staff

Ive already emailed my manger but due to it being the weekend they wont answer
On your medical test did they fill out d4 and send it for you along with the d2 form??
Did you already bring the d2 form with you at medical or was it provided by the medical team??
