      @@Melissa-in6un they didn't report the death to CMS and it was discovered after CMS did a surprise inspection after being tipped off about it. The doctors who pronounced her failed to mention the medication error to the ME office and because of that they ruled her death natural and didn't notify CMS. As a nurse myself I would have loved to been a fly on the wall when CMS walked in unannounced and asked specifically for this patients chart. Anytime CMS walks through the door it's going to be a bad day. They come yearly, always unannounced, and audit your records. That being said they have a tendency to hit every hospital while their in the area so sometimes you get a heads up.

      @@saramcgaha1406 from my understanding some hospitals include vecuronium in every pyxis in the hospital and others only keep in crash carts. It's used for intubating people so you are correct that it gets used most in a surgery setting but you can use it with any intubation. Not as often as we used to but sometimes we use it. My hospital includes it in the pyxis and in an emergency situation I would have to override it just like she did to retrieve it. However I've never had to do that with versed because we usually don't need it on an emergency basis and we have time to get an order for the pyxis before we pull it.. Also versed is usually under Midazolam and I've wondered if she didn't recognize that. She was a floater nurse and maybe she wasn't familiar with the generic name and that is why she overrode it. I never found out exactly why she had to override it and there could be many reasons and her not recognizing Midazolam as versed could be one. It was a perfect storm scenario with several different factors but the biggest factor hands down was Vanderbilts decision to let nurses override a pyxis in any situation without verifying with pharmacy while their pyxis system was being upgraded. One call would have saved that womans life but since it took roughly ten minutes to have pharmacy verify they decided to save time and give the nurses a work around . Had you told me that prior to this incident I would have laughed in your face and Vanderbilt should have been sitting right beside her on trial. I could be biased because I'm also a nurse but Vanderbilt was the biggest factor in the case.

      This is totally despicable on Vanderbilt I’m in healthcare I would not work for that hospital not if they treat their employees like that terrible she followed protocol she was in very unfavorable circumstances short staff lack of equipment the computer system was down wasn’t functioning properly and this is how errors are made she reported it the hospital lied about it did not report a sentinel event and then threw the nurse out to dry because they have deep pockets this is so disgusting.

      @T She didn’t make 17 errors The computer system that they were using for the medication dispensing was not communicating they were new to epic the EMR system number one they were short staffed number two there was a computer issue number three there was no scanner in radiology at all so to say that she made 17 errors this is why nurses are very upset over this case especially nurses that have worked in these types of situations because they know in addition to that the hospital never reported the sentinel event even though the nurse reported it immediately.

    My God! Thank you for hearing and answering our prayers.. this is the best that could come from the worst situation. I can’t imagine the relief she must feel. As a nurse this hit home but I’m glad God and the Judge were merciful

    Thank you judge. I am 72 retired 6 years. It had become an impossible horrible job. I still have nightmares I am at the hospital and I can't get my work done. Wake up in a panic. I would never ever ever go back. I do not know where I got the strength to do it 40 years. I wish all nurses could just walk out. I have cancer surgery next week so I need a nurse but still enough is enough. It doesn't have to be this way. It was not this way in the past. Corporate hospitals ruined healthcare.

    I'm delighted the Lord and the judge showed mercy to Ms. Vaught. She will live with this error for the rest of her life, and I sure pray she can secure some type of peace and not be shunned by society. I'm an LPN and have always been concerned about this type of unfortunate accident taking place at my hands.
    I pray the Lord extends the patient's relatives peace and someday the ability to forgive Ms. Vaught.

      Very well said. I don’t believe the general public really understand what exactly Nurses do, the responsibility they have. The Nurse/patient ratio is ridiculous (generally speaking) and they are only human, mistakes-human error does happen and will continue to happen.

Yes it was just a mistake..........oh ok no problem........Just does not work that way Someone needs to be held accountable.       @@Melissa-in6unthis is a very late response, but not thanks to nurses, it’s thanks to hospitals administrators and this kind of thing. Nurses are over worked, stressed, understaffed , exhausted. why don’t think they only let truck drivers and pilots fly for a certain amount of hours? Yet there’s not regulations like that for healthcare workers. This kind of thing will only make health care workers second guess coming forward about mistakes due to the fear of criminal prosecution. I guess you are perfect and have never made any errors in life or at work

    The chain of events that lead to this error were due to numerous factors - Having been a nurse for 32 years - working in unsafe environments of staff shortages and lax policies

      It is not about being lax in her job because of burnout. She did not try to hide what she did. She told them. No one is perfect. You have never been on that kind of situation. You know nothing of the medication systems in place. The hospital made her the scapegoat. She followed correct protocol. The hospital did not. She is the fallout.

      I agree with you I’ve been in healthcare almost 30 years myself whenever there is staffing issues lack of equipment or non-functioning equipment which it’s all of the above in this case it leads to errors. This nurse should have never been criminally charged if anybody should be criminally charged it should be the hospital Vanderbilt in my opinion as a nurse I would not work for them because they do not treat their nurses well.

      Why do they make such a big deal out of fatigue & workload with commercial aircraft pilot's then if it doesn't contribute to accidents?
      Pilot's are not allowed to be overworked for a good reason. It's scientific fact that fatigue effects our performance.

    Wow, the judge is impressive. The lawyers not so much. Did she basically tell the prosecution that the case never should have been brought to trial? Or, is that just my bias as a nurse in Nashville coming through? If so, my trust in the judicial system has been somewhat renewed. However, I do not understand how the people of Davidson County can approve of Chad Jackson, Britanni Flatt and their boss Glenn Funk. As long as they have the ability to twist reality to match their misguided political aspirations I'm not so sure I will continue to work in Nashville. There's plenty of better paying nursing jobs.

      Hi I want to let you know I'm afraid of this city and those in power. My husband is is undocumented imigrant was given custody of my kids in a divorce he then molested my 11 year old daughter. They investigated and the Da refused to prosecute him. My daughter went through a forensic interview and to multiple people and children services. She tried to commit suicide too. Da will do nothing so this case came as a shock
      So a nurse get taken to the gallows and an illegal man molest a child and he's running around harrassing.

    Everyone loses. I believe Vanderbilt shares some blame as well as the MD that ordered IV sedation in a non monitored area. I am not second guessing the jury, but it's unfair that she took all of the blame. I commend the judge as she used the power of the law with integrity while showing true humanity.

      Nurses administer medication unmonitored all the time, it’s not like we always have someone looking over our shoulder. She also failed to do her 5 medication rights and ignored several red flags, so the event was indeed her doing. That being said, she was in fact by all intents and purposes tossed under the bus.

What went through your mind when you heard Vanderbilt allowed it's nurses to use a work around with this pyxis because the system was down for upgrades without having to verify with pharmacy? I was shocked however I work in a smaller hospital and maybe that's common place in big hospitals like Vanderbilt. We're not perfect and I've made my share of mistakes but I just don't understand how she missed the huge paralytic warning on the vial. That's probably one of the most important things I learned in nursing school is to always look at the medication before you push it because it's the last chance to spot it and I always check and all of my coworkers check too. If I'm not mistaken that's one of the reasons the board permanently revoked her because there is nothing you could say that could justify that.

      ICUs typically allow you to override most anything in the pyxis, because let’s face it, when a patient is coding, you don’t have time to get a second nurse to override the machine with you. She was an ICU nurse, thus she didn’t require a second nurse to verify, hints why she was able to override without a second nurse. And yes, she should have looked at the vial, that’s just common sense. Hell, she should have read what it was she pulled before dispensing it. That she reconstituted and administered a medication without once looking at the label is beyond me and she should have known better.
      Though I strongly question as to why Norcuron was available in the Pyxis to begin with. That’s typically only used by Anesthesiologists and CRNAs and is normally stocked only in the OR Pyxis and requires special access, given how dangerous it is.
      I work in a nursing home that has no bar code scanners, so I have to be extra vigilant in checking everything and even then, I’ve made a few mistakes, thankfully nothing serious. Med errors rarely result in revocation of licensure, it’s more of the cause of the med error that can result in loss of your license. In her case, she made numerous mistakes that even a rookie nurse wouldn’t have made. Again, it’s just common sense to read the label of a medication before administering it, it’s your last check.

      Yeah she is. That woman who felt unimaginable horror, as her breathing stopped while she unable to call for help, was a human being too. Feel sorry for her. Not this nurse. She is alive, healthy and will continue living her life as a free woman after this is over.

    Why is it considered that "the defendant" is capable of being rehabilitated? And she is immutable to correction? After 18 minutes the judge finally stated what this was. It was a mistake! A mistake brought on by poor rules and over stretched health care workers. This woman is not a criminal!

    First, I wish that guy would move his big fat head. Second, why is RaDonda’s attorney not sitting beside her explaining things to her and/or comforting her? I know there's still a pandemic but I saw others sitting close together and talking face to face with no mask on, so where is her attorney??? 🙄 Finally, YAH only 3 years probation!!! 👏 👏 👏 🙌🙌🙌

      My point was the lack of support from her counsel. I understood it all too but I would expect my attorney to be by my side. I'm obsessed with true crime and watch a ton of court cases and the attorney's are usually by their clients side. 🤷‍♀️

      We would absolutely love that. The problem is no one wants to do our job. They have no interest in staying hours after your shift ends, being yelled at by patients, families and doctors, multitasking at least six issues at a time or being soiled by someone else's body fluids. Now with COVID we have the worst shortage we've ever had. I imagine learning you could be criminally prosecuted for making a mistake didn't help. My state pays for nursing school if you work in the state that paid for it for five years. So if you could round some people up who meet those expectations we would welcome them with open arms.

    Something good hopefully will come from this horrible situation. Eg Providers: never drop OCD behaviors while on the job NEVER. Hospitals: you do not exist to make a buck. Most importantly "staff shortages" are not an option. A minimalist staffing approach (particularly when it comes to the large percentage of staff who are nurses making relatively big $) is one of your go to money saving techniques which most people will accept as a sad reality

    What a terrible thing all around. I can't help but feel compassion for the nurse, and I guess the family of the patient did too, since they forgave her in the courtroom in a different clip that I saw. It really was just a terrible mistake and you can see that the nurse will never really get over it. I can't imagine bouncing back from something like that.

    If we don't forgive others, God will not forgive us. I pray for both family's. I pray for this nurse 🙏. She will have to live with this gor the rest of her life. Just a sad situation. It's so obvious that she is remorseful. If the patients family can forgive, why can't we! May God bless all involved!

    You don’t make mistakes with medical you take your time and focus,,,you don’t play with your cell phone talk to your girlfriends at work and feel like your just doing a job,, and get distracted,,someone died,,,I’m sorry this is prison ,,got off to easy and sends a bad message that’s it’s ok to fCK up,,don’t be a nurse!!!drive a cab,work in the post office,pu garbage,,,but don’t be a nurse!!!and she’s crying because she got away with it also the judge is a waste of tax payer money!!what else is new

You unknowingly are, I give you a prescription to something and you go home and have a severe allergic reaction and die. I should go to prison?
