    between which layers would inguinal hernia repair mesh be placed using the TEP (total extra peritoneal) laparoscopic method?
    i have had the surgery but i cannot come up with a satisfying answer.

      I thought it was placed internal to all layers … however I am not that familiar with surgical procedures

    I believe the Ilioinguinal nerve is not a content of the spermatic cord but runs in the inguinal canal beside it!
    The genital branch of genitofemoral nerve does pass as a content of the cord!
    correct me if i am mistaken please

      +Samir Seginy Hi Samir, you are correct in that there is something unique about the Ilioinguinal nerve ... it is a content in the spermatic cord but does not enter the inguinal canal at the deep inguinal ring. The genitofemoral nerve enters the inguinal canal at the deep inguinal ring, courses the length of the inguinal canal and enters the spermatic cord at the superficial inguinal ring. However, because the ilioinguinal nerve courses within the body wall (between the internal oblique and transverse abdominis muscles) it merges into the inguinal canal and enters the spermatic cord and becomes its contents at the superficial inguinal ring. Does that make sense?

    I recently had a hernia operation after being told I had a hernia for about a year. Turned out to be Cord Lipoma, and I now can have kids naturally. I was told before surgery I was infertile. 2 problems sorted in one surgery. The lump in my groin is now gone and so is the blockage stopping my sperm getting out. And that's the short version!

      Erikcardi Cardenas a lipoma is a benign lump of fat, and it was on my spermatic cord. Was blocking sperm so I can now have children naturally

    You haven't mentioned the Tunica Albuginea in your video, also the cremaster muscle is formed by the lowermost parts of the internal oblique and the transversalis abdominis is that correct?

      +ahmadharb13 The cremaster muscle is an extension of the internal oblique muscle and the transverse abdominis muscle and its associated aponeurosis do not contribute to the spermatic cord. The tunica alb. is part of the testis and the video was focusing on the inguinal canal and spermatic cord primarily, which is why it was not mentioned. I hope this is helpful.

    While mentioning the contents of the spermatic cord, you just mentioned testicular arteries.
    What about cremateric arteries and arteries to ductus deferens...?
    Should those be mentioned or not...?

      Hello Sumit, thank-you for your comment. I found that when i tried to cover every detail of anatomy for each topic the videos run to long so my focus is on the content that is most clinically relevant as opposed to anatomically exhaustive. I have found that this works well within a medical school curriculum.

    I'm not sure that sperm are propelled from the testes through the ductus deferens during ejaculation. I think the sperm may just swim (or be moved gradually) up the ductus deferens, "contaminating" the prostatic urethra and surrounding areas, and are then expelled with the semen during ejaculation. I've gotten confusing and differing explanations regarding this, even amongst urologists!

      +Mitchell Wilson Hi Mitchel, thanks for your question. Yes, ejaculation is the movement of sperm out the urethra. Emission is the process of sperm moving from the testis to the ductus deferens. I have found that most anatomy textbooks just group "emission" and "ejaculation" together for simplicity. Other references (like some physiology textbooks) separate "emission" and "ejaculation". Does that help?

    This is just wrong. He fails to explain how the spermatic cord becomes covered by the layers passed thru by the fetal testes on their way to the scrotum. There are misstatements, including some egregious ones - "the gubernaculum pulls down the testes", etc., and very misleading illustrations.

      +Silverman ze'ev Hello, thank-you for watching the video and your comment. You are correct in that i could have used different vocabulary other than, "pulls down". The testes are guided by and follow the pathway of the gubernaculum from the posterior abdomen into the scrotal sac. Will you please expand on what you mean by "misstatements"?
