  • IMPORTANT 2023 UPDATE re: IUD DURATION OF USE- US FDA now approves 52mg levonorgestrel IUD (Mirena, Liletta) for 8 YEARS use for contraception. When treatment of heavy menstrual bleeding is the primary indication, best results are usually seen in the first 5 years. Kyleena (19.5mg) is still 5 years. See video #35 for info on copper IUD extended use.
    ****
Dr. Kristina Dervaitis, a gynecologist in Newmarket, Ontario (Canada) answers your questions about intrauterine contraception.
Can you get pregnant while using an IUD? (ie, what is the failure rate of an IUD?)
    What happens next?
Key messages covered in this video:
-IUD failure rate compared to failure rates of other contraceptive options
-risk of tubal and intrauterine pregnancy with IUD failure
    -possible reasons for IUD failure
-(brief outline) treatment options for tubal pregnancy
-management options for intrauterine pregnancy with IUD in place (termination versus continuing pregnancy)
-possible signs of pregnancy to watch for
-possible risks when continuing with an intrauterine pregnancy with an IUD in place
-future contraception after an IUD failure

Thank you so much for this fabulous series of videos!! I had a pretty terrible experience having my IUD inserted - the gynecologist i was referred to where i live in Southern Ontario had a very "drive-thru-medicine" approach and it was all very rushed and painful on the day of insertion. I wasn't walked through the procedure very well (in fact the doctor practically inserted the uterine sound when i was mid-sentence with the nurse and that was very startling and painful because i had thought he was going to freeze my cervix first. Panicked by the severity of the pain, i asked about freezing but they insisted we were almost done anyways if i could just breathe through the 2nd big cramp because the freezing would take a lot more time to do than finishing the procedure). I didn't get to ask many questions, or if i did they were brushed-off, so it is fabulous to be able to listen to your video series and learn what to expect with my body post-IUD insertion! Thank you so much for sharing your guidance and easing my worries ☺ watching them has made me feel so much better!

That was excelente Dr. D! You covered everything. I wonder about the possibility of a defective IUD or an IUD that all of sudden stops releasing the daily hormone amounts. I guess watching for changes in the cicle is important to make sure the IUD is working properly. In some videos I've watched about IUDs, I've read that a lot of women commented they got pregnant while having one placed. It's surprising and a little concerning. Thank you again Dr.!

Hi! Thank you for the informative videos. You say you have seen several women had a full-term pregnancy with the IUD in place in your practice. This concludes there must also be plenty of others with ectopic pregnancies, who choose to remove the IUD, have an abortion etc. Do you think the failure rate really is just 0.2%, considering you had several cases just in your practice?

    • @dr.kristinadervaitis789
      @dr.kristinadervaitis789  3 роки тому +12

I have seen several “just” in my practice because I insert close to a thousand IUD’s per year currently, and so I have inserted literally thousands of IUD’s over the last 16 years. My practice numbers are consistent with the failure rate data that is well established in the scientific literature (studies based on experience with many more thousands of women). In my practice, I have also seen unplanned pregnancies (intrauterine and ectopic) in patients using other contraceptive methods such as the birth control pill and condoms, these numbers also consistent with established failure rates (9% for the pill, 18% for condoms).

    Hi Dr,
Can you make a video on male contraceptives? Or is this out of your scope? I feel like men also need to play a part in prevention of pregnancies and I know that there male contraceptive injections out there. Thank you!

    • @dr.kristinadervaitis789
      @dr.kristinadervaitis789  4 роки тому +6

I agree 100% that contraception needs to be a joint responsibility of BOTH male and female partners!- and I did do a video about this called “It Takes Two” (video #22). According to the stats I receive from UA-cam, about 10% of the viewers of this channel are male, which is great to see (but could improve even further!)
Regarding male (hormonal) contraceptives: although there is research into this field, currently there are not any male contraceptives that are commercially available or approved by the FDA (and so I can’t really comment further). I would be happy to do a video on this topic in the future if there is a product that becomes approved and available.
Thanks for watching and for your comments!

Hi there doc! I don’t know if you’ll see this but I have a question... my mother and grandmother both got a iud, got pregnant but they both tested negative on a pee test. I got mine in January, haven’t had my monthly since. I’ve been having cramps not as bad I usually would with my pcos and endometriosis. I’ve been getting morning sickness. Super light headed to the point I have to sit down to do the dishes. I went to the doctor. Told them my concern and family history. I told them I wanted a blood test but they told me it wasn’t necessary. They did a bunch of other test which were all negative and they left it at that... I’m still having cramps, lower back pain, dizziness, the morning sickness or getting sick over the smell or the taste of things and my energy has completely gone down the drain. What is your opinion on this? Should I get a second opinion? A ultrasound?

I've had the paragard copper iud since Aug 19. My cycle has always been on time only difference was it was heavier than normal and I had really bad cramps. Last month for about an hour I had cramps similar to contractions coming in waves. I was so stressed out at the time I figured that was why. I got my period on time this month but no cramps or heavy bleeding. Also it only lasted 2 days. Now I'm wondering if I may be pregnant because my periods have been normal the entire time.

      Hello-
Unfortunately I can’t give personalized medical advice over the internet. I will say that “generally speaking”, although bleeding can happen in pregnancy, it would be very unusual for it to occur in a predictable (monthly) fashion. If any patient is concerned about a possible pregnancy while using an IUD (IUD failure) I’d recommend they do a pregnancy test or see their doctor.

Hi. I've got 2 things I'm wondering about right now and would be very thankful for your reply.
    So the first one is:
I have a hormonal IUD (Mirena) inserted in Febuary. I was bleeding then and then quite a lot, it must be like half a month until the bleeding really stopped.
So the next month, I had my period, I was bleeding on and off again for quite a long time but I think it was a little less than the last time.
Then another month came again and my period is totally missing until today with no sign of breast tenderness at all, which I always use to have before my period arrives.
Is it normal that the period will stop coming that early?
    Another:
So right now I'm starting to have more and more symptoms popping up such as becoming easily tired at a certain time, having strong but short cramps at my lower part, I think it's because of the IUD but don't know why the pain relies at the back, like at my anus/rectum part instead of the utesrus. My nipples are also starting to hurt too, which I've never had this kind of symptom before.
Could these reasons that I've told you possibly be a sign of pregnancy?/Should I take a pregnancy test just to be sure?.

A woman Pap smears was done and also an IUD is

Get an appointment the following week. The OBGYN check on the IUD finds out she’s 7 weeks pregnant. A request for a removal of the IUD but string is not visible for the OBGYN to removal. Hello ma'am
Have you ever had a case where the IUD has moved waaayy up into the uterus. Where the patient can actually feel the IUD poking thru the surface of the skin? I'm assuming close to the fallopian tubes?

    • @dr.kristinadervaitis789
      @dr.kristinadervaitis789  4 роки тому +3

No, I have not witnessed this phenomenon. In the rare instance that an IUD “migrates” out of the uterine cavity, it is still contained within the abdominal cavity (well below the surface of the skin).

I am pregnant with the copper iud in situ. It was perfectly in place and they couldn't find any reason for the failure. They couldn't remove the iud as the strings had risen into my cervix due to my womb expanding. At first, it was very close to the amniotic sac so I was monitored closely and it was consistently imaged. I'm now 38 weeks and having a cesarean on Friday (for unrelated reasons). Thankfully none of the expected complications happened. I'm having a hormonal iud fitted at the same time.

Hi! I would absolutely love a response to this just simply to reassure myself. I have the copper coil but since beginning to being sexually active I constantly panic I’m pregnant. I see these stories in the media of women getting pregnant with periods and I’m just wondering what kind of blood it would be like? I’m usually pretty heavy because of the coil but these past months it is a little lighter (bright red and then the dark bright red with small black clumps) so sorry for the tmi but i read these articles and constantly panic. I don’t know why but it’s an anxiety trigger of mine and was just wondering what kind of bleeding it is for when women have periods when pregnant.

I also see these articles about failing pregnancy tests. How rare is it to not show hcg in urine and only needing blood tests? Is there a way to know if you’re one of those people?

    • @dr.kristinadervaitis789
      @dr.kristinadervaitis789  3 роки тому +3

      Hello-
The failure rate of the copper IUD is 6-8/1000. If this risk seems too high for you and is causing you anxiety, you may want to consider using an additional method of contraception such as condoms.
While pregnant women can sometimes have bleeding in the first trimester or implantation bleeding, it is usually not like a period. Pregnancy tests vary in accuracy based on type (urine versus blood) and when they are taken (at the time of a missed period? Sooner? ) This channel is not intended to provide personalized medical advice and is not intended as a substitute for pregnancy testing. If you are worried you may be pregnant take a test at home or with your doctor.

This channel is not intended to provide personalized medical advice, nor is it a substitute for a pregnancy test. There is a 6-8/1000 chance of pregnancy with the copper IUD. If you are concerned about a possible pregnancy, do a test, see your doctor, or both.

I never had a pregnancy test done before my IUD insertion. My gyno is very very bad. I haven’t bled since I’ve had it put in, and haven’t been able to have my checkups because of COVIID. I’ve been having all pregnancy symptoms the past month, and my belly has popped and I’m getting stretch marks. I’m scared to make an emergency apt because I’ve had prior miscarriages, and it’s the reason why I got an iud. Not asking for an answer since you can’t diagnose someone over the internet, but what would you recommend I do? If I am pregnant, do you know if I can sue my doctor for not testing me beforehand? This is my second iud, first time I got one they did a test. I have the Kyleena. I’ve had it for almost 4 months now. No checkups since I got it. They all cancelled and wouldn’t rebook due to the COVID. Thanks for the video :-)! Hope you’re doing well with everything otg.

      Hello-
Every IUD inserter practices differently, and there is variation in how each clinic attempts to establish that the patient is not pregnant at the time of insertion. It is also a patient responsibility to be confident she is not already pregnant when she attends for an IUD.
This channel provides general information only and I cannot comment on your specific case. But if you think you are pregnant, see a doctor! (There is a 2/1000 chance of pregnancy with a hormonal IUD, and a rare chance of tubal pregnancy- which could be an emergency).
Also wishing you well during this stressful pandemic!

      Hi Nicole-
If you mean, will it still work as a contraceptive if malpositioned: it depends on how far out of place, and what kind of IUD (hormonal vs. copper). Unfortunately I can’t give personalized medical advice over the internet so I can’t comment on your particular situation.

I went from super heavy periods to suddenly only light bleeding for maybe a couple hours and then it stopped. I was waiting for the rest of my period to come but it hasn't and it's been a week. Not sure if I should take a pregnancy test right away or wait a bit longer. I haven't been keeping good track of my cycle, so I don't know if it was a month since my last real period or not.

I recently got my Liletta IUD removed and wasn't put on a new form of birth control to see if the cramping I am having is from the IUD. However, my husband and I had unprotected sex the day before and 3 days before without being told or being aware that sperm could live for as long as it does. I frequently track my cycle and it looks like I was at peak ovulation. Could we end up being pregnant?

    • @dr.kristinadervaitis789
      @dr.kristinadervaitis789  Рік тому

The hormonal IUD prevents pregnancy by thickening cervical mucous to prevent sperm entry, and by thinning the lining of the uterus. The thin lining effect on particular is likely to persist in the several days after removal. However, technically pregnancy is possible (even with the IUD still in place the chance of pregnancy is 2/1000).

The 52 mg levonorgestrel Liletta has been tested and approved for use up to 7 years. The 52 mg levonorgestrel Mirena is approved for 5 years, it has not yet been approved for use up to 7 years (unless inserted at age >44). If your IUD is expired, I’d recommend that you see your doctor for replacement.

The hormonal IUD is effective immediately if inserted in the first 7 days of the menstrual cycle, otherwise it will be effective 7 days after insertion. Because of the small risk of expulsion, and the very rare risk of uterine perforation, I usually advise my patients to use a back up method of contraception until I’ve had the chance to do a post-insertion check up in the office (I usually do a check about one month after insertion).

Yes, it’s possible for bleeding to return after not having a period for some time. If there is a change in bleeding, I recommend having the IUD position checked (either by checking strings at home- see video #27- or seeing a doctor for a check).

Most women with the hormonal IUD still ovulate. Not having menstrual bleeding is due to the progesterone thinning the lining of the uterus. To learn about how the IUD prevents pregnancy, see video #9: ua-cam.com/video/N-YEdEAfL54/v-deo.html

This is a random question for this video but I couldn’t find much on this. Is the hormonal IUD suitable for people with PCOS? I live in England and I read on the NHS website that it can have a side affect of making cysts grow and I was on the mini pill for a few months then I had to stop because of a non stop break through bleeding and through ultrasound they also found a 17cm dermoid cyst on my right ovary that wasn’t there before that they had to remove through key hole surgery... it was a horrible experience and I’m scared of repeating, it I’d love to know your thoughts. Also, in the uk they tend to offer mirena or Jaydess, which do you recommend? I hope you see this before Wednesday because that’s when I’m getting this done and I’m so scared :S

      Hello-
While I can’t comment directly on your personal medical history, I will make the following general comments:
-the hormonal IUD is a good option for PCOS patients, because these patients are generally deficient in progesterone (which the IUD provides in the form of levonorgestrel) I usually prescribe Mirena to my PCOS patients.
-the sorts of cysts that can occur with hormonal IUD use tend to be functional cysts (not dermoid cysts, not the same cysts referred to in the term polyCYSTic ovarian syndrome)
      Hope this helps!

Unfortunately, I can’t give personalized medical advice over the internet. You will obviously need to follow up with your doctor for further investigations in this situation. If the IUD is not seen on ultrasound and not seen on the X-ray, it is likely that it has been expulsed, and there is a risk of pregnancy. Definitely I would advise a patient in this situation to begin using an alternative form of contraception immediately.

There is no evidence of teratogenic effects (ie, any negative effects on the baby’s development) from either a copper or levonorgestrel IUD ( in the event of an IUD failure and ongoing pregnancy) regardless of whether the IUD is able to be removed.

Most women with mild to moderate prolapse will not have any problems using an IUD. If the prolapse is very severe (ie, the cervix is actually protruding out of the vagina...) it might be problematic. Your doctor will advise you as to whether an IUD is a safe option, based on your anatomy and degree of prolapse.

Also my cervix is low and hard, is pregnancy still possible or is it safe to say I’m not pregnant? I don’t expect my period to come for another week and a half. Thanks!

The failure rate (chance of pregnancy) with a copper IUD (even one that is in proper position...) is 6-8/1000. So yes, there is always a small chance of pregnancy. If you are late for your menstrual cycle, do a pregnancy test and/or see your doctor.

I have a silly question as a bloke I honestly had never heard of IUDs to such a point that I googled it! So my missus has had an IUD for years, she's 48 and still has her menstruations. Is there a chance of being pregnant - not the ectopic one? Pardon me if it's very personal but I can't have a condom as it causes massive pain and irritation, but we've had no issues whatsoever. I was just wondering if that can occur and also infer on her perimenopause? Thus causing painful periods. Cheers for that! This coming from a concerned man. I guarantee that I'm no freak so I'd appreciate a clear answer as it'd be awkward to make an appointment with a gyn for such 'daft' questions. Cheers and thanks for such a clear vid.

Hi I know this is late but I’m on the Mirena and the Levlen pill that I take continuously for my severe endometriosis. Is there much of a benefit using condoms with my husband? • @dr.kristinadervaitis789

Most health care providers would say “no”. The chance of pregnancy with Mirena is 2/1000. Add a birth control pill on top of that, and your chance is even lower. If a patient is comfortable accepting that very low theoretical risk, they don’t use condoms (unless they are using them to prevent sexually transmitted infections with a new partner)… if they aren’t comfortable with that very small risk, they use condoms concurrently.

It is important for a copper IUD to be in proper position within the uterus in order for it to function to prevent pregnancy. Usually it is most concerning if the copper IUD is too low in the uterus (or especially if it is so low that it is in the cervix instead of the uterus).

Dr., let's say I were to have an intrauterine pregnancy with a Mirena IUD after nearly 5 years of use, and the pregnancy was further along, what possible side effects or abnormalities could an embryo's exposure to levonorgestrel experience?
I've been randomly reading about the effects of progestin on the virilization or masculinization on female embryos and am interested in knowing if continued exposure in vitro by not being able to remove an iud might cause birth defects and, respectively, if iud removal later in term can make mistakes more likely?

    • @dr.kristinadervaitis789

      Hello-
There is no evidence that the progesterone exposure from a levonorgestrel IUD would cause any birth defects (it is not “teratogenic”). Similarly exposure to an oral birth control pill (either progestin only or estrogen + progesterone) is not teratogenic.
If an intrauterine pregnancy occurs with an IUD in place, we usually only try to remove the IUD as soon as possible (as early as possible in the first trimester) and would not attempt a “later” removal.

Tip Of The Iceberg I’m wondering if there were signs indicating that her IUD may have moved. I’ve had Mirena for 3 years successfully!! And Mirena has two ways of prevention, physical barrier AND hormonal. I’ve seen more “horror stories” with Paraguard, which, while I can’t predict the causes for the failure, would make sense as it only has one method of prevention (physical, no hormonal method). IF you are considering birth control, there’s plenty, but I know that the efficacy rate is higher for the IUD than other methods. Of course, some women may find it more “invasive.” Look into other options or you could always do IUD + another method! (condoms, possibly withdrawal)

      Hi CL-
You’ve responded to several viewers comments now. Can I ask what your background is? (IUD user? health care practitioner?) While I appreciate your input, at the same time- this channel is not intended to give personalized advice, and is not meant to have any industry bias. Can you expand on your perspective here?

I got the copper IUD because I have 4 kids already. November 2021 I found out I was pregnant with the copper IUD . Ultrasound showed the IUD didn't shift or move, strings still in place, so I made the 1% lol . Currently 5 months due the end of July

Thank you Dr for such a thorough and clear explanation 💖 Just found out that I am pregnant with a Mirena in place. This was very helpful in understanding "what now".

is it normal to dont have menstruation even if you are using iud??

If it is a levonorgestrel (hormonal) IUD, 20-30% (depending on the type/dosage) may not have any menstrual bleeding. (But with a copper IUD, one would expect to have their normal menstrual cycle). See also video #7, video #10.

Unfortunately, there is no contraception option that prevents pregnancy 100%. Sorry to hear about your stressful situation. Best wishes to you Ivy! I hope that things go well.

This channel is not intended to provide personalized medical advice. If you are concerned about about a missed cycle and chance of pregnancy, see your doctor. The chance of pregnancy with an IUD is 2-8/1000.
