Discarded towel incorrectly after drying. Opened gown with sterile side facing him. Did not tie gown. Second towel on mayo stand went below sterile field. Did not count gauze in descending order. Arm below waste. Cough in hand. He removed instruments out of tray for the count. He counted to fast. The count was not in an organized manner. The way the basin stand was moved, his hands were not on the top plane. His hands should have been wrapped and tucked behind the drape as he opened it up. The circulator was over sterile field and made contact swaying. Turned his back. Some of these I’m not sure are real mistakes but since you say there is 25 I pulled what I thought would be mistakes. Love these kind of exercises and you videos !
tons of mistakes! -When drying with towel, goes from elbow to hand while drying + when discarding -Touches outside of sterile gown with hand -Opens gown the wrong way and still uses it -Holds open hands to sterile gown while waiting for circulator -circulator walks between two sterile fields -doesn't glove while circulator is tying him -never turns with circulator -grabs gown wrapper after touching it with ungloved cuffs -completely tucks in mayo stand cover before putting on towels (big waste of time) -touches instrument set with exposed back -doesn't cuff fingers when draping ring stand -fingers touch underside of back table while moving ring stands onto table -completely sets up back table before touching instruments(first priority) -while toweling mayo stand, the towel falls below table level and is brought back up -very sloppy mayo stand toweling -touches gown with instrument set while waiting for filters to be checked -also probably touched ring stand with gown but I can't really see -brings glove to face to cough -spends a gazillion hours perfecting the instrument setup while the circulator is waiting for him -touches back of gown with gloves and then goes back to field -circulator walks right up to the field and is holding paper over the sterile field -picks up each raytec/lap from the top to count. hard to type it out but theres a much easier -and faster way to count sponges. -touches each sharp with glove instead of pointing at it with something -doesn't show the bovie tip -idk if its a big deal but he counts "1 2 3 4 kelly clamps" instead of "kelly clamps 1 2 3 4" -doesn't count one of the knife handles -never sets up the mayo stand -never gets any medication -never loads the scalpels -count is never written down anywhere
I appreciate this list, I agree with basically everything you put. I also noticed that his towel dipped below the edge of his back table when setting it down, not sure if that was intentional or not. The cough into the glove and touching the back of the gown were especially egregious 😂
The first thing he did wrong was when drying his hands he went back up the forearm. Thats a no no. He didn’t spin to close the gown, moved multiple items multiple times on the back table, when placing the towels on the mayo he dropped it too low and then pulled it up higher, coughing on gloves and contaminated the table, popping the tag on the rays & laps before the count, (can’t tell if the count is correct because it’s hard to see) but he said that 1 knife handle was wrong.
Hands were super high walking in….towel drying, didn’t pass the tag off from gown, opening the instrument tray(it wasn’t on a sterile surface, so circulator should have opened for you to remove the inner contents…the count, back table towel too close to edge of table, lifted hand to face to cough(you can cough at sterile field, as turning head causes particles to fly out the sides) but you can’t lift hands above nipple line…
He touched the sterile napkin with both hands when he was about to discard it, coughed on his hands, hands below the table when he moved the ring stand
He touched his face, his hand went below his waist when he scratched. he counted things that didn’t need to be counted and instead of waiting on the circulator to get done tying him up, he should gone ahead and put on his gloves, and the last thing he didn’t turn with the circulator.

He forgot to blow his nose with the towel before rolling it up for the instruments. I don’t know if you know this but he was suppose to fart through the syringe and closing it.
Oh, and I forgot to mention, he didn’t pick up his suture to show the circulator and he also dropped his blue towel that he was putting on his mayo below the sterile area.
